Management of a large CBD stone, beyond the scope of ERCP

We present the case of a 77-year-old lady admitted with obstructive jaundice. Magnetic resonance cholangiopancreatography (MRCP) performed identified the presence of a large common bile duct (CBD) stone measuring 50.22 mm by 20.62 mm. Due to the size of the stone it was not possible for removal via endoscopic retrograde cholangiopancreatography (ERCP) or use lithotripsy to aid its removal. Attempts to insert a stent past the stone to drain bile had failed. Thus the stone had to be removed surgically by an open common bile duct exploration.
