Usher introduced the first polypropylene mesh in 1968 as a method to achieve tension-free hernia repair. It took less than a decade for the worldwide adoption of the mesh to replace most anatomical repairs. Mesh is one of the most commonly used implants in surgery today. Several meta-analyses showed that laparoscopic incisional hernia repair is associated with lower surgical site infection (SSI) rates compared to open repair. However, the efficiency of antibiotic prophylaxis (AP) in laparoscopic incisional hernia repair alone is unknown and needs evaluation. Due to increasing antimicrobial resistance, a major global health care problem, AP needs to be critically evaluated.The aim of this study was to investigate the impact of AP on the rate of SSI and complication-related reoperations in patients undergoing laparoscopic incisional hernia repair. In this September issue of the Editor ’s Perspectives, I am going to talk more on endoscopic polypectomy, and its recent developments in the upper and lower gastrointestinal tract (GIT) using flexible endoscopy. Polyps can develop in the upper and lower GIT, and some of these polyps have the potential to become malignant. For a small polyp, polypectomy can be done using a hot biopsy forceps. The specimen obtained can then be sent for histopathological examination.
