Recent advances in postoperative free microvascular flap monitoring
Publication date: October 2013 Source:Formosan Journal of Surgery, Volume 46, Issue 5 Author(s): Cherng-Kang Perng Microvascular free flap surgery is one of the most important operations in reconstructive surgery. With the advancement of microsurgical technique, the success rate of free flap pedicle anastomosis is now >90%. The key to further enhance the free flap success rate is early detection of vascular thrombosis and salvaging of the failing flap with early intervention. The gold standard of postoperative free flap monitoring is clinical observation of flap color and capillary refill; however, it is subjec...

Laparoscopic ureteral repair for iatrogenic ureteral injury following lumbar disc surgery
Publication date: December 2013 Source:Formosan Journal of Surgery, Volume 46, Issue 6 Author(s): Ping-Ju Tsai , Hay-Yan J. Wang , Yung-Shang Lin , Tzu-Ming Yang Iatrogenic ureteral injury is a rare yet severe complication in lumbar disc surgery. In our hospital we encountered only one such incident in 1946 prior to this case. We hereby present a novel approach in which laparoscopic end-to-end ureteral anastomosis was adopted for the repair of ureteral injury after lumbar disc surgery. A 60-year-old female underwent minimal invasive transforaminal lumbar interbody fusion (TLIF) for spondylolisthesis of the L4–5 l...

Evaluation of a revised in-hospital trauma activation protocol according to clinical outcome for patients with major trauma
Conclusion The study shows that our TPA criteria are easy to follow and have a high accuracy rate. For patients with ISS ≥16 who did not meet our TPA criteria, escape was mainly due to head trauma. Additional factors besides GCS scores should be incorporated in the protocol to identify patients with significant head trauma. (Source: Formosan Journal of Surgery)

Dedifferentiated liposarcoma can induce a leukemoid reaction
In this report, we present the rare case of a patient who had de-differentiated liposarcoma and elevated G-CSF levels that resulted in a leukemoid reaction. The patient was a 65-year-old man who had been lame for one month due to right thigh swelling. His body temperature was slightly elevated at 38°C and leukocytosis with an elevated white blood cell (WBC) count (41500/μL) was noted. The findings of computed tomography of the lower extremities indicated the presence of a malignancy. Therefore, an incision biopsy was performed. Based on the finding of magnetic resonance imaging (MRI) and the biopsy pathology report, we d...
