Endoscopic treatment with wrapping by a vascular prosthesis for internal jugular vein phlebectasia
Publication date: June 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 3 Author(s): Yu-Yuan Chen , Yu-Hao Huang , Yen-Ting Sheen , Jui-Ying Lee , Yu-Tang Chang The management of jugular vein phlebectasia through a transverse cervical incision provides good direct exposure. However, patients having such a procedure will have a scar in the neck, which may be cosmetically unsatisfactory. The authors describe in this communication a transaxillary subfascial endoscopic treatment of internal jugular vein phlebectasia with wrapping by a vascular prosthesis in a 6-year-old boy. The wrapping prevented the vein fr...

Axillary cystic hygroma in an adult
Publication date: June 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 3 Author(s): Yu-Hao Huang , Ya-Wei Lai , Tung-Ying Hsieh , Su-Shin Lee , Kao-Ping Chang , Sin-Daw Lin , Chung-Sheng Lai Cystic hygroma (CH) is an uncommon congenital anomaly of the lymphatic channels that occurs mainly in children. CH is usually found in the cervical, axillary, inguinal, retroperitoneal, or thoracic region. CH is rare in adults, and to the best of our knowledge, there are only three adult case reports in the English literature (searched via PubMed) that discuss the development of CH in the axilla during adulthood. T...

Meta-analysis of randomized studies of surgery for supratentorial intracerebral hemorrhage
Conclusion In terms of mortality and dependence, MIS had significantly better results than medical treatment for STICH, deep ICH, and putaminal ICH. In the present review, craniotomy showed no significantly better outcome than medical treatment. (Source: Formosan Journal of Surgery)

Surgical safety margin of gastroenterological cancer surgery: A truth or a dream?
Publication date: June 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 3 Author(s): Chen-Guo Ker Most surgeons dream about performing an extensive resection with a wide resection margin and extensive lymph nodes dissection, which will yield a favorable prognosis. Previous studies have reported varying lengths of the margins based on different clinical profiles. The so-called safety margin is not completely safe because limited scientific evidence exists for nonrecurrence, even after the patient has had a pathological examination to prove a negative cancer invasion at the resection margin. The safety margins ...

Obtainment of mesenchymal stem cells by surgeons
Publication date: August 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 4 Author(s): Chien-Yu Ou , Nyuk-Kong Chang , Tsung-Han Lee , Yu-Hua Huang , Pei-Wen Chang Chien , Ta-Ting Tang , Tsung-Ming Su , Yu-Jun Lin , Nan-Fu Chen , Tao-Chen Lee Biological and clinical interest in mesenchymal stem cells (MSCs) has risen dramatically over the past two decades. This paper introduces the methods we used to obtain bone marrow MSCs by means of locally available materials. Rabbit bone marrow contents were harvested from rabbit femurs. After lysing red blood cells, the fibroblast-like cells adhering to the fla...

A huge extradural ganglioneuroma of the lumbar spine
Publication date: August 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 4 Author(s): Feng-Ji Tsai , Keng-Liang Kuo , Rong-Dar Tzou , Yu-Hsin Cheng , Shiuh-Lin Hwang , Ann-Shung Lieu Ganglioneuromas are benign tumors that arise from the ganglion cells of the sympathetic nervous system. They are most commonly localized in the posterior mediastinum, followed by the retroperitoneum, cervical region, and adrenal gland. We encountered a rare paraspinal neurogenic tumor with extension into the spinal extradural space that presented with a dumbbell shape and bony indentation. The clinical features, radiographi...

Spontaneous spinal epidural hematoma: Early surgical intervention provides ideal neurological outcome
We report a case of SSEH, and discuss its early diagnosis and rapid surgical intervention. A 46-year-old man was brought to our emergency department because of sudden-onset, rapidly-progressive paraplegia and acute urinary retention. Magnetic resonance imaging revealed an epidural mass lesion in the posterior epidural space from T9 to T11 in the spinal canal. The patient underwent emergency decompression laminectomy within 4 hours of symptom onset and recovered fully within 1 week. In patients with SSEH, early surgery is crucial to ensure a favorable outcome. (Source: Formosan Journal of Surgery)

Simultaneous pectoralis major-latissimus dorsi myocutaneous flap: An option of reconstruction for open sternal wounds
We report a retrospective series of cases with regard to the management of sternal wounds according to our own experience and a review of the related literature. A retrospective chart review from June 2007 to February 2010 of patients with infective and open sternal wounds after surgery was performed. There were seven patients with a previous history of type A aortic dissection treated with the Bentall operation, thymoma with excision, mitral and tricuspid regurgitation with double valve replacement, and acute myocardial infarction with a ventricular-assist device or coronary artery bypass graft. All wound cultures of the ...

Repeated intracerebral hemorrhage as a presentation of pulmonary giant cell carcinoma with brain metastases
We present a patient with progressive impairment of short-term memory and apathy. Multiple hemorrhagic metastatic tumors in both cerebral hemispheres were found on image examination. Chest computed tomography (CT) showed suspected bronchogenic carcinoma. The left frontal brain tumor was removed, and pulmonary giant cell cancer was proved by pathology. Unfortunately, intracerebral hemorrhages in different locations were found to increase in the following brain computed tomography. A second and a third brain tumor excision were performed. Pathology showed the same metastatic giant cell carcinoma. The clinical course rapidly ...

Necrotizing fasciitis in liver cirrhosis
Conclusion NF in the vast majority of cirrhotic patients was caused by Gram-negative bacilli. Hypoalbuminemia (serum albumin <2.5 g/dL) was associated with mortality in patients with liver cirrhosis and NF. Further studies are needed to assess whether resuscitation with albumin-containing solutions lowers the mortality rate in such patients. (Source: Formosan Journal of Surgery)

Nonintubated video-assisted thoracoscopic pulmonary resections
Publication date: August 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 4 Author(s): Ming-Hui Hung , Jin-Shing Chen Video-assisted thoracoscopic surgery (VATS) has become a common and globally accepted mode of resection of pulmonary tumors. One-lung isolation using double-lumen endobronchial tubes or endobronchial blockers has been traditionally considered mandatory for VATS. However, recent reports showed that VATS pulmonary resections can also be performed safely using regional anesthesia without tracheal intubation. Mostly, nonintubated VATS pulmonary resections are performed using regional anesthesia, ...

Mixed endocrine-glandular neoplasm of cecum
Publication date: Available online 10 August 2014 Source:Formosan Journal of Surgery Author(s): Tung-Jung Liang , Tuan-Ying Ke Colorectal mixed endocrine-glandular neoplasms, a subtype of neuroendocrine tumors, are mainly diagnosed based on pathological characterization. The rarity and unusual presentation of mixed endocrine tumors makes their prognosis relatively poor, and an optimal management strategy for the tumors has yet to be devised. Here, we report a case of cecal mixed adenocarcinoma and neuroendocrine carcinoma with ileal tumor seeding, lymph nodes, and left iliac crest metastasis. After undergoing right h...
