Huge tumoral calcinosis of the buttock
We report a rare case of TC secondary to end-stage renal disease in a 65-year-old female patient presenting with a huge, painful, buttock area tumor involving the sacrum that required extensive surgical excision and gluteus maximus myocutaneous flap reconstruction. Complete resolution was achieved after a wide surgical excision followed by local flap reconstruction and medical control. Surgical excision alone may result in a high recurrence rate when TC is secondary to other diseases. Combination with medical treatment to achieve phosphate deprivation is more effective. Comparing the effectiveness of endoscopic carpal tunnel release between idiopathic and long-term hemodialysis patients
Conclusion ECTR was effective in both groups of patients, but the severity of their pre-operative status, especially with advanced involvement, may compromise the objective results. Although the results from the self-administered questionnaires and the electrophysiological findings were inconsistent in these hemodialysis-related patients, it is possible to have satisfactory results in symptoms and questionnaires but with the less satisfactory electrophysiological outcome. Anal canal melanoma misdiagnosed and treated as prolapsed hemorrhoids in a male patient
Publication date: April 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 2 Author(s): T.A. Mala , R. Gupta , S.R. Ahmad , S.A. Malla , V.B. Gupta , I. Shah Malignant melanoma of the anal canal is a rare, but aggressive, tumor with a poor prognosis. It represents less than 1% of all melanomas and 4% of anorectal tumors. We are reporting this case because of its rarity in men and because, in this instance, the tumor was misdiagnosed as prolapsed thrombosed hemorrhoids and excised. Examination showed a visible exophytic irreducible growth measuring about 3 × 3 cm and protruding outside the anal verge an...

Spontaneous rupture of a congenital umbilical hernia in an infant: A rare complication
Publication date: April 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 2 Author(s): Rahul Gupta , Tariq Ahmed Mala , Atul Gupta , Lila Dhar Agrawal , Arun Kumar Gupta , Avinash Suhkdev Jadhav Umbilical hernia is not uncommon in children. Most of these hernias close spontaneously as the children grow; they are often remarkably free from complications. Although this did not affect the accepted principles of management of umbilical hernia, we feel that this case of spontaneous rupture is worth reporting because of the severity of the evisceration of the small intestines in a 45-day-old female infant. The ...

Amputation of lower limb for necrotizing soft-tissue infection in an ultramarathon runner
We present the case of a 51-year-old Taiwanese woman, a female ultramarathon runner who had a medical disaster after her participation in La Trans-Gaule French ultramarathon. After completing the competition, she was diagnosed to have septicemia and a potentially life-threatening NSTI of both lower limbs. Therefore, she underwent emergent right above-the-knee amputation and left foot transmetatarsal amputation. Then she came back to Taiwan to receive further treatment. After meticulous reconstructive surgery and continuous rehabilitation programs, she was instructed to wear prostheses and finally resumed her daily activiti...

Hybrid procedure for pyeloplasty in infants and young children with ureteropelvic junction obstruction is a safe and effective alternative
Conclusion In infants or young children (<1 year old or weighing <10 kg) with UPJO, the hybrid procedure of pyeloplasty may be considered as a safe, effective, and less time-consuming alternative to laparoscopic surgery, and most importantly, confirms the security of the anastomosis. Cervical total disc replacement
Publication date: April 2014 Source:Formosan Journal of Surgery, Volume 47, Issue 2 Author(s): Jau-Ching Wu Cervical total disc replacement (CTDR) has been accepted as a viable option for surgical management of cervical spondylosis or degenerative disc disease. Current indications for CTDR are one- and two-level cervical spondylosis and degenerative disc disease causing radiculopathy or myelopathy that is refractory to medical treatment. Conventionally, these patients could be managed surgically with anterior cervical discectomy and fusion (ACDF) as the standard of care. In recent years, there have been several large-...

Fibrous dysplasia of the thoracic spine

She complained of severe pain of the left arm, numbness on the ulnar side of the left forearm, and weakness of the middle to little fingers. Computed tomography and magnetic resonance imaging showed the characteristic appearance of fibrous dysplasia, which includes ground-glass opacity, an expansile nature, and lytic lesions with sclerotic rims. She underwent surgical intervention with radical tumor resection combined with stabilization and fusion ...
