Restoring microcirculation in anesthesia: Impact, usefulness and controversies
Publication date: Available online 3 April 2016 Source:Colombian Journal of Anesthesiology Author(s): Diana Patricia Ñáñez-Varona, Adolfo León Tróchez-Zuleta, William Andrés Vargas-Garzón Mentioning microcirculation and endothelial dysfunction to explain the pathophysiology of diseases whose relationship was not well understood is a recent phenomenon. Microcirculation is not only compromised by disease but can be altered by conditions that the anesthesiologist faces (agents and anesthetic techniques). There is significant and increasing evidence that anesthetic agents may alter it. The importance? In a sta...

Case history. “Coral snake” Micrurus mipartitus bite in 1968. A herpetologist's ordeal
Publication date: Available online 24 March 2016 Source:Colombian Journal of Anesthesiology Author(s): Bernardo Ocampo Trujillo Historical clinical case that presented back in 1968 at a time when respiratory support and intensive care techniques were just emerging, with many shortcomings in hospital care areas and monitoring devices. The case is of a 58 year-old patient, outstanding citizen, recent winner in a television contest on snakes, who was bitten accidentally by a coral snake Micrurus mipartitus. The poison of this snake is a macromolecule that induces complete depolarizing muscle blockade which, if not rever...

Successful systemic thrombolysis in a patient with massive pulmonary thromboembolism after prolonged cardio pulmonary and cerebral resuscitation. Case report
Publication date: Available online 21 March 2016 Source:Colombian Journal of Anesthesiology Author(s): Gustavo Chicangana, Lorena Zapata, Juan Camilo Gómez, Juan Pablo Zuluaga Massive pulmonary thromboembolism is a condition with high morbidity and mortality if not treated early. A case of a young man with a history of knee trauma that was admitted to the emergency department with sudden dyspnea and syncope is discussed. During the clinical evaluation the patient experienced 5 episodes of cardiac arrest that required prolonged cardiopulmonary-cerebral resuscitation. The diagnosis of massive pulmonary thromboem...

Multicenter study on effectiveness of controlling postoperative pain in Colombian patients
Conclusion None of the clinics had acceptable percentages of patients with pain controlled 4h after surgery. Clinical practice guides should be incorporated, institutional policies should be defined, health personnel should be trained, and the outcomes of the interventions should be evaluated. Design and performance evaluation of the “iTIVA” algorithm for manual infusion of intravenous anesthetics based on effect-site target
Conclusions The performance of the algorithm to achieve effect-site concentrations during induction and maintenance for remifentanil and propofol was excellent, with a low systemic deviation versus the desired target concentrations. (Source: Colombian Journal of Anesthesiology)

Retained epidural catheter: A rare complication. Report of two cases
Conclusion To prevent these potential complications, refrain from passing excessive catheter length and do not leave more than 5cm of catheter into the epidural space. If a catheter cannot be easily removed, try using several maneuvers and if these fail or there is any pain or paresthesia, diagnostic imaging is required to locate the catheter and request neurosurgery support. Horner syndrome after epidural analgesia for labor. Report on three cases
Publication date: Available online 16 March 2016 Source:Colombian Journal of Anesthesiology Author(s): Eduardo Rodríguez-Sánchez, Juan Manuel Vadillo, Pablo Herrera-Calo, Maria Luisa Marenco de la Fuente Epidural analgesia is assumed to be the technique of choice for the relief of pain in labor. Multiple adverse neurological effects have been reported, one of which is the so-called Horner syndrome (ptosis, myosis, anhidrosis). Its evolution is usually benign and does not require specific management, except clinical monitoring for the more than probable cephalic spread of local anesthetic. Most of the cases th...

Do not resuscitate orders and anesthesia
Publication date: Available online 16 March 2016 Source:Colombian Journal of Anesthesiology Author(s): Fritz Eduardo Gempeler-Rueda, Ana María De Brigard-Pérez Given the poor results derived from cardio-pulmonary resuscitation (CPR), some decades ago, so-called do not resuscitate orders were established. These include unilateral medical decisions taken in extreme situations when the survival rate or recuperation of the patient is considered nil. Currently, and given the development of individual guaranties and their adoption in clinical practice, do not resuscitate orders are understood as agreements between phys...

Vasectomy surgery: Where one should not underestimate the risk
Publication date: Available online 16 March 2016 Source:Colombian Journal of Anesthesiology Author(s): Juan David Ramírez-Pimiento, José Ricardo Navarro-Vargas Vasectomy has been considered a minor surgical procedure, even more so in view of the fact that, most of the time, it can be performed under local anaesthesia in a doctor's office. However, a particular phenotype is frequently found among those cases in which the urologist decides to use general anaesthesia, that poses a challenge for the surgeon as well as the anaesthetist.
