Anesthesia for interventional neuroradiology
Publication date: Available online 4 March 2015 Source:Colombian Journal of Anesthesiology Author(s): Chanhung Z. Lee Interventional Neuroradiology (INR) is firmly established in the management of cerebrovascular diseases. The aim of this manuscript is to present the author's critical review of the literature and interpretation emphasizing perioperative and anesthetic management strategies to prevent complications and minimize their effects if they occur. Planning the anesthetic and perioperative management is predicated on understanding the goals of the therapeutic intervention and anticipating potential problems. (...

Cardiopulmonary resuscitation beyond the technique
Publication date: Available online 24 February 2015 Source:Colombian Journal of Anesthesiology Author(s): Fritz E. Gempeler R. This reflective article presents the current state of cardiopulmonary resuscitation (CPR) and reviews it from a bioethical standpoint. It starts with the ineffectiveness of CPR and the reasons why today it is a universally applied procedure, sometimes without taking into consideration the wishes or condition of the patient. Possible courses of action for the continuous improvement of cardiopulmonary resuscitation are proposed, especially from the humanistic point of view. Greater involvement o...

A comparative analysis of 3 sedation guidelines for patients undergoing subarachnoid anesthesia. Randomized, single blind clinical trial
Conclusions The three sedation guidelines presented were effectively used in subarachnoid anesthesia; the results were more favorable with the use of midazolam+fentanyl or midazolam+ketamine. p-2011-1682 Colciencias. The special abilities of the healthcare professional
Publication date: Available online 23 February 2015 Source:Colombian Journal of Anesthesiology Author(s): Geovana Andrea Vallejo Jiménez Introduction One of the parameters used when analysing the occurrence of an offence of negligence is the so-called ideal average individual scale. 1 Thus, in order to determine whether medical negligence has occurred, the criteria that apply to the average ideal practitioner are generally used. 2 Nonetheless, there are situations in which healthcare professionals have abilities that are superior to those of the average ideal practitioner. This essay reflects on whether these super...

Nasotracheal intubation with Airtraq optical laryngoscope in two cases of oral brachytherapy
Publication date: Available online 23 February 2015 Source:Colombian Journal of Anesthesiology Author(s): Miguel Castañeda Pascual , Eva Turumbay Ramírez , Alejandro Bilbao Ares , Juan P. Jimeno García , Mikel Batllori Gastón , Esther Hijosa Basarte Nasotracheal intubation is usually the selected airway approach for oral and maxillofacial surgery to enable a free surgical field. It is an unusual intubation technique with a high success rate. However, it is not free from difficulties and complications in certain groups of patients, like those with oral, facial or cervical tumors. The Airtraq® laryngoscope (Pr...

Cerebral salt wasting syndrome in the posterior fossa surgery post-operative period: Case report
We present a case of a patient taken to microvascular decompression (Janetta surgery) for the treatment of trigeminal neuralgia who developed a symptomatic magnification of basal hyponatremia in the immediate post-operative period. Cerebral salt wasting syndrome was diagnosed. The management of this condition poses a challenge for physicians involved in postoperative neurosurgical care. Anaesthetic implications in Von Recklinghausen disease: A case report
We describe the anaesthetic management of a patient with NF1 under general anaesthesia for facial neurofibroma excision. We performed a brief review of the literature with the aim of optimizing the anaesthetic management and reducing the number of complications associated with the systemic manifestations of this syndrome. Intrathecal opioids and respiratory depression: Is it myth in obstetrics?
Publication date: Available online 11 December 2014 Source:Colombian Journal of Anesthesiology Author(s): Pedro José Herrera Gómez , Juan Federico Garzón The addition of opioids to bupivacaine for spinal anesthesia has been shown to improve quality of anesthesia by the action of fentanyl, and extend postoperative analgesia by the effect of morphine. Side effects, particularly respiratory depression, have prevented their widespread use. Studies are not consistent regarding the incidence of respiratory depression due to the variety of definitions of this complication and the doses of opioids used. Low dose regimens ...

Anaesthetic management for craniotomy in a pregnant patient with rupture of a cerebral arterio-venous malformation: Case report
We describe the anaesthetic management of a 32-year-old woman at 22 weeks gestation with intracranial haemorrhage due to a ruptured arteriovenous malformation (AVM). Managing these patients requires a complex approach with a highly individualised plan involving neurosurgeons, neuroradiologists, anaesthetists, obstetricians and neonatologists to assess the risk and benefit of all the different therapeutic alternatives. Given the high risk of further bleeding during pregnancy and the location of the AVM, the best therapeutic option in this case was considered to be a craniotomy and complete removal of the lesion.
