Oral Candidiasis and Inflammatory Response: a Potential Synergic Contribution to the Onset of Type-2 Diabetes Mellitus
In this study, we explore the hypothesis that yeast colonization of the oral cavity directly influences the tendency of an individual to develop type 2 Diabetes. According to this hypothesis, oral yeast colonization may influence the pathway in which the immune response handles subsequent responses to glucose intake. The first part of this work presents data concerning oral Candida carriage in subjects with pre-diabetes, the second part explores the potential role of oral Candida infection, related to inflammatory response in promoting insulin resistance. Fluconazole-Warfarin Interaction: A case report with deadly consequences
Adverse drug-drug interactions are more common in the elderly because of the commonality of polypharmacy. When one of the interacting drugs has a low therapeutic index, the consequences can be life-threatening or fatal. This case describes a fatal cerebral haemorrhage in an 80-year old male on stable doses of warfarin prescribed by his cardiologist, who was prescribed fluconazole 200mg, once daily, for 14 days by an oral and maxillofacial surgeon to treat an intraoral fungal infection. Trichobezoar - A hair-raising surgical emergency
We present a case of an intellectually impaired 14-year-old girl with a gastric trichobezoar discovered during an emergency laparotomy. We will discuss the limited but salient findings on history and examination. Through a review of the literature, imaging modalities and treatment options for the early versus late stage of trichobezoar presentations will be identified. Hypothyroidism in a 45, X phenotypic male with a Y/18 translocation
This is the first case report of a 45, X male with autoimmune thyroid disease. Chromosome analysis revealed that all cells were 45, X without chromosome Y. The entire short arm and the centromere of Y chromosome were translocated onto the short arm of chromosome 18. Fluorescence in situ hybridization revealed that the SRY gene was present in all cells. At the age of 3 years he was diagnosed with hypothyroidism, and treatment was initiated. Painful shoulder? Remote clinical management of a Field Guide with shoulder pain and loss of shoulder function in Antarctica
We report the case of a female Field Guide based at the British Antarctic Survey ’s Rothera Science Research Station on Adelaide Island, Antarctica who independently contacted a physiotherapist specialising in climbing related injuries (GJ) located in the UK. for a second opinion. The Field Guide was experiencing significant work difficulties due to shoulder pain and subsequen t loss of function particularly in overhead activities. Pre-operative immunonutrition therapy in upper gastrointestinal cancer patients
Conclusion We found no difference in post-operative outcomes; however, the pre- and post-implementation comparison was limited by poor adherence to the protocol as a consequence of late attendance at pre-admission clinic. Of those who did attend as planned, patient acceptance was high. Future studies incorporating prospective, quantitative pre-operative immunonutrition intake are needed.
