How hypothyroidism affects the ovaries
A 15-year-old girl who was previously diagnosed to have primary hypothyroidism presented to the outpatient department in our tertiary care centre in Southern India with abdominal distension and pedal oedema. On evaluation, she was found to have elevated lactate dehydrogenase (LDH) and CA 125 levels, enlarged multicystic ovaries, ascites, and pleural and pericardial effusions. A diagnosis of spontaneous ovarian hyperstimulation syndrome (SOHSS) was made based on the characteristic soap bubble-like imaging appearance of ovaries in the background of hypothyroidism and she was started on thyroxine. Diagnosing intravascular large B cell lymphoma
Intravascular large B cell lymphoma (IVLBCL) is a rare condition with a predilection for central nervous system involvement and is often misdiagnosed. This case report describes a 58-year-old gentleman who presented with paraparesis and subsequent dramatic neurological deterioration initially attributed to acute disseminated encephalomyelitis. Upon post-mortem examination, the correct diagnosis of intravascular large B cell lymphoma was reached. This condition has a poor prognosis but can be chemotherapy responsive. Conclusion We demonstrate the challenges and illustrate the heavy workload involved in receiving widespread ethics and governance approval across Australia. We highlight the need to simplify, homogenise, and nationalise human ethics for non-clinical trial studies. Sedation in Dentistry
Conclusion Formal training is essential to achieve the safe practice of sedation in dentistry or medicine. The appropriate setting for sedation should be determined as there is an increased risk outside the hospital setting. A rare presentation of HBV infection
We report a case of a 44-year- old male with HBV infection, who underwent kidney biopsy for rapidly progressive renal failure and nephrotic range proteinuria. Histopathological evaluation of the kidney biopsy was consistent with immune complex mediated crescentic membranoproliferative glomerulonephritis (MPGN). The patient achieved complete renal and virological remission with steroids, plasmapheresis and antiviral therapy. This case report summarises the importance of early initiation of immunosuppression and plasmapheresis under antiviral coverage for improved clinical outcomes. Dancing eyes and dancing feet in scrub typhus
A 26-year-old male, presented to us with complaints of fever for five days and breathlessness for one day. During the hospital stay, he developed myoclonic jerks in all four limbs, head titubation, and saccadomania. Magnetic resonance imaging (MRI) of the brain ruled out structural lesions and cerebro spinal fluid (CSF) analysis ruled out meningo-encephalitis. Weil Felix was strongly positive, OX K titres were one in 640, and IgM for scrub typhus was positive. He was treated with doxycycline for one week.
