Copper concentrations in Egyptian infants with cholestasis: A single center study
Conclusion Serum and hepatic Cu concentrations were markedly elevated in patients with cholestasis and positively correlated with each other and with S. ceruloplasmin. S. Cu level can predict hepatic Cu concentration. (Source: Arab Journal of Gastroenterology)

Dendritic cell-based cancer immunotherapy for pancreatic cancer
Publication date: Available online 8 March 2018 Source:Arab Journal of Gastroenterology Author(s): Wei Li, Xiujun Song, Huijie Yu, Manze Zhang, Fengsheng Li, Cheng Cao, Qisheng Jiang Pancreatic cancer (PC) is a lethal disease and remains one of the most resistant cancers to traditional therapies. New therapeutic modalities are urgently needed, particularly immunotherapy, which has shown promise in numerous animal model studies. Dendritic cell (DC)-based immunotherapy has been used in clinical trials for various cancers, including PC, because DCs are the most potent antigen-presenting cell (APC), which are capable ...

Double-balloon enteroscopy (DBE) in patients presenting with obscure gastrointestinal bleeding (OGIB)
Conclusion DBE is an excellent endoscopic procedure that has a relatively high diagnostic and therapeutic yield. The procedure is feasible and exhibits a high safety profile with a low complication rate when performed by an experienced endoscopist. (Source: Arab Journal of Gastroenterology)

Sustained improvement of psoriasis associated with HCV after virologic response to sofosbuvir/ribavirin
Publication date: Available online 8 January 2018 Source:Arab Journal of Gastroenterology Author(s): Asem Ahmed Elfert An 18 year old boy attended a dermatology clinic outside our hospital and diagnosed with psoriasis. His skin lesions over both ankles (Fig. 1) did not improve at all with potent local steroids twice for 3 months, then 3 times daily for another 3 months. His dermatologist talked to him about using methotrexate and asked for liver function tests before starting the systemic therapy. His ALT and AST were doubled. The patient came to our Hepatology clinic worried about his elevated liver enzymes. We a...

A difficult diagnosis of coeliac disease: Repeat duodenal histology increases diagnostic yield in patients with concomitant causes of villous atrophy
We report a case of a woman with anaemia, weight loss and diarrhoea with an initial diagnosis of seronegative CD and a histological documented villous atrophy who did not improve on gluten-free diet due to the concomitant presence of common variable immunodeficiency (CVID) and Giardia lamblia infection. This case report confirms that CD diagnosis in CVID patients is difficult; the combination of anti-endomysial antibodies (EmA-IgA), anti-tissue transglutaminase antibodies (tTG-IgAb) antibodies and total IgA is obligatory in basic diagnostic of CD but in CVID are negative. Furthermore, the typical histological aspects of th...

Colon cancer with rapid growth in 16  months confirmed by computed tomographic colonography
This report describes a case of rapidly growing colon cancer confirmed by computed tomographic colonography (CTC). A 75-year-old woman presented at our clinic with complaints of abdominal discomfort and melena. Colorectal examination was performed via CTC. The findings were normal. Sixteen months later, she was referred to our clinic again for positive findings on a faecal immunochemical screening test. A repeat CTC was performed, which revealed a 3 cm ulcerated tumour at the ascending colon. Colonoscopy with biopsy revealed tubular adenocarcinoma. Laparoscopic right hemicolectomy was performed and the TNM classification...

The effect of premedication with peppermint oil capsules (Colpermin) prior to colonoscopy: A double blind randomized placebo-controlled trial
Conclusion This randomized controlled trial does not support the routine use of peppermint oil capsules prior to colonoscopy as a tool for procedure optimization, and patients’ and endoscopist’s satisfaction. (Source: Arab Journal of Gastroenterology)

Systematic gastric biopsy in iron deficiency anaemia
Conclusion Intestinal metaplasia was frequently observed and was mostly related to Helicobacter pylori infection. These patients require monitoring, especially if they are young because it represents a pre neoplastic condition. However, in our study autoimmune gastritis often described in the literature in case of iron deficiency anaemia was rarely seen raising the question of relative cost-efficiency of fundic biopsies during iron deficiency anaemia. Gastric submucosal haematoma caused by an amyloidoma in the setting of multiple myeloma
Publication date: Available online 15 December 2017 Source:Arab Journal of Gastroenterology Author(s): Stella C. Pak, John-Phillip T. Markovic This is a case of a 67-year-old woman with a history of multiple myeloma with amyloidosis who presented with massive hematemesis. Emergent upper endoscopy revealed a mass on the greater curvature of the stomach, which measured approximately 3 cm in width and 7 cm in length. The patient underwent a wedge resection of the gastric mass without complication. Microscopic examination of the gastric mass revealed amorphous deposits that were congophilic in nature and stained positi...

Prevalence and risk factors of hepatitis D virus infection in patients with chronic hepatitis B infection attending the three main tertiary hospitals in Libya
Conclusion Although the study demonstrated that Libya has low to moderate prevalence of HDV (2,5%), it is important for policy makers and health care providers to continue the preventive measures for HDV spread, and HBV prevention program including utilization of HBV vaccine. Furthermore, it is imperative to screen chronic HBV patients for HDV for close observation for early diagnosis of subsequent development of liver cirrhosis. Moreover, further epidemiologic and genetic studies are needed to explore the trend for HDV infection in Libya. (Source: Arab Journal of Gastroenterology)

Simple classifiers for molecular subtypes of colorectal cancer
Conclusion There is still a need for a consensus gene expression-based subtyping classification system for CRCs, thereby allowing the categorization of most CRC tumours. This study reveals that a simple and rapidly accessible process could replace the complicated, costly and mostly inapproachable methods clinical practices that have been introduced in the majority of previous studies.
