Circulating microRNA-155 is associated with insulin resistance in chronic hepatitis C patients
ConclusionOur findings indicate that the insulin resistance and T2D in HCV are strongly related to miR-155. This may suggest a role for miR-155 in the pathogenesis of IR caused by HCV. However, further large-scale studies are required to confirm our findings. (Source: Arab Journal of Gastroenterology)

Prognostic factors in patients with active non-variceal upper gastrointestinal bleeding
ConclusionUndesirable clinical outcome in patients with acute non-variceal upper gastrointestinal bleeding has a significant statistical association with longer hospitalisation, chronic underlying disease, anticoagulant administration, packed red blood cell infusion, higher Forrest stage, low systolic blood pressure, higher age, low haemoglobin, low platelet count, high INR and high BUN at the onset of diagnosis. Postanal minimally invasive surgery “PAMIS” assisted extra-levator abdominoperineal excision “ELAPE” for cancer: A novel approach in supine position
Publication date: Available online 12 February 2019Source: Arab Journal of GastroenterologyAuthor(s): Ahmed FaragAbstractLocal recurrences are more common after abdominoperineal excision (APE) than after anterior resection of rectal cancer. Extralevator APE was introduced to address this problem. The post anal minimally invasive approach had been used by other authors for transperineal mesh rectopexy but not in cancer management. Our aim is to use the post-anal minimally invasive approach for better visualization and division of the levator Ani during the Extralevator abdominoprineal excision in 2 cases of carcinoma of the...

Paraoxonase-1 and arylesterase levels in patients with ulcerative colitis
ConclusionThis study found that the ARE values of UC patients were lower than those of healthy subjects. The same results could not be determined for PON-1. The data suggest that the antioxidative capacity of UC patients may be reduced. (Source: Arab Journal of Gastroenterology)

Fiducial placement for recurrent gastric cancer
Publication date: Available online 8 February 2019Source: Arab Journal of GastroenterologyAuthor(s): Mohamed M. Abdelfatah, Eric M. GochanourAbstractA 53-year-old male was diagnosed with invasive adenocarcinoma of the pre-pyloric region. Imaging studies ruled out metastatic disease. The patient underwent neo-adjuvant chemotherapy followed by a partial gastrectomy. CT scan at 18 months’ post-resection demonstrated right upper quadrant lesions suspicious for metastatic disease. EUS exam shows two round hypoechoic lesions. Fine needle aspiration with suction was performed with on-site cytology confirming malignant cells i...

Primary omental malignant solitary fibrous tumour, an extremely rare malignancy: A case report and review of the literature
We present the case of a 57-year-old male patient diagnosed with primary omental malignant SFT. Neither tumour origin nor pathologic diagnosis were possible based on preoperative information. Nevertheless, tumour resection was performed successfully.Several SFT cases involving omentum without malignant potential have been reported. However, primary omental malignant SFTs are extremely rare with only 3 cases reported in the literature. (Source: Arab Journal of Gastroenterology)

Colonization of intestinal microbiota with carbapenemase-producing Enterobacteriaceae in paediatric intensive care units in Cairo, Egypt
ConclusionPrevalence of carbapenem resistant Enterobacteriaceae was 24%, various genes of carbapenemases were detected in 80% of carbapenem resistant Enterobacteriaceae with dominance of blaOXA-48. Understanding the colonization status of our patients with strict infection control measures can reduce the risk of horizontal gene transfer of carbapenemases. The association between blood urea nitrogen to creatinine ratio and mortality in patients with upper gastrointestinal bleeding
ConclusionsA BUN/Cr ratio of>30 was found to be an independent risk factor for mortality and may be useful for pre-endoscopic assessment. Development of future risk scoring systems might warrant consideration of including BUN/Cr ratio as a parameter for estimating risk. (Source: Arab Journal of Gastroenterology)
