Rifaximin versus metronidazole in management of acute episode of hepatic encephalopathy: An open labeled randomized clinical trial
Conclusion Rifaximin and metronidazole are equally effective in management of acute episode of overt HE, therefore, re-auditing of treatment protocols of HE are warranted especially in limited resource settings. (Source: Arab Journal of Gastroenterology)

The gut microbiome and irritable bowel syndrome: State of art review
Publication date: Available online 20 June 2018 Source:Arab Journal of Gastroenterology Author(s): Ahmed E. Salem, Rajdeep Singh, Younan K. Ayoub, Ahmed M. Khairy, Gerard E. Mullin Irritable bowel syndrome (IBS) is a functional disorder of the gastrointestinal tract, the physiology of which is not very well understood. There are multiple factors and pathways involved in pathogenesis of this entity. Among all, dysmotility, dysregulation of the brain-gut axis, altered intestinal microbiota and visceral hypersensitivity play a major role. Over the last years, research has shown that the type of gut microbiome present i...

Hepatitis C virus antibodies are absent among high risk group of health care workers in Damascus Hospital
Conclusion HCV is not a main issue regarding its prevalence among HCWs in Damascus Hospital. Nevertheless, it is still necessary to develop a mandatory well-organized program to increase the awareness among HCWs and develop stricter prevention policies especially about bloodborne diseases transmitted occupationally. Impact of Franseen needle on rapid onsite evaluation and histological examination following endoscopic ultrasonography-guided tissue acquisition in patients with splenic malignant lymphoma
Publication date: Available online 30 May 2018 Source:Arab Journal of Gastroenterology Author(s): Tesshin Ban, Hiroshi Kawakami, Yoshimasa Kubota, Yuichiro Sato Rapid onsite evaluation (ROSE) following endoscopic ultrasonography (EUS)-guided fine-needle aspiration contributes to the establishment of a diagnosis for various organs. Newly designed three-plane symmetric needles for EUS-guided fine-needle biopsy (EUS-FNB), such as the Franseen needle, have been developed to enable histological core tissue acquisition. However, EUS-guided tissue acquisition for hypervascular splenic lesions remains challenging. Tissue acq...

Oxidative stress is bane in chronic liver diseases: Clinical and experimental perspective
Publication date: Available online 28 May 2018 Source:Arab Journal of Gastroenterology Author(s): Devaraj Ezhilarasan Oxidative stress plays an important role in the pathogenesis of various chronic liver diseases (CLD) and increasing evidence have confirmed the contributory role of oxidative stress in the pathogenesis of drugs and chemical-induced CLD. Chronic liver injury is manifested as necrosis, cholestasis, fibrosis, and cirrhosis. Chronic administration of anti-tubercular, anti-retroviral, immunosuppressive drugs is reported to induce free radical generation during their biotransformation in the liver. Further, th...

Globular amyloidosis of the colon
Publication date: Available online 24 May 2018 Source:Arab Journal of Gastroenterology Author(s): C. Díaz del Arco, M.J. Fernández Aceñero Gastrointestinal involvement is frequent in systemic amyloidosis. However, amyloidosis can rarely be confined to the gastrointestinal tract or appear as a tumour mass. There have been few reports describing amyloid globular deposits in a variety of locations, as opposed to the usual linear ones. We herein report a rare case of globular amyloidosis involving the large bowel, which to the best of our knowledge is the second reported in the world literature. A 74-year-old man consul...

Antibiotic prophylaxis in transarterial chemoembolization of hepatocellular carcinoma
Conclusion Third generation cephalosporin, levofloxacin or ciprofloxacin all are effective as prophylaxis against post-chemoembolization infections. No significant difference between oral and intravenous administration among the 3 groups. Oral ciprofloxacin is an effective, safe and relatively inexpensive prophylaxis regimen. The prevalence of occult hepatitis B in chronic hepatitis C patients treated with interferon-based antiviral therapy
Conclusion According to the results of this study, the prevalence of OBI is lower than expected amongst CHC patients in Turkey and it may not be necessary to apply routine screening to IFN non-responders for OBI infection before DAA therapy. However, there is a need for multicentre studies with larger patient series. (Source: Arab Journal of Gastroenterology)

Determinants of survival following hepatocellular carcinoma in Egyptian patients with untreated chronic HCV infection in the pre-DAA era
Conclusion Survival following HCV-HCC in Egyptian patients undergoing HCC management in a specialised clinic setting is poor, although similar to high income country settings.
