Paediatric and adolescent diabetic ketoacidosis
Abstract Diabetic ketoacidosis (DKA) is one of the two acute emergency situations in those who have diabetes mellitus. DKA should be identifiable and is often preventable. Any associated complications of DKA should also be preventable with proper education and treatment. Awareness of presenting signs and symptoms, on the part of the general public, primary care providers and emergency department personnel, should help minimise the severity of DKA through earlier diagnosis and focusing on appropriate physiologic treatment as well as close, ongoing monitoring to minimise potential lethal DKA complications such as cerebral oe...

A simple method for introducing care planning into specialist diabetes clinics. The WICKED project
Abstract Diabetes care planning is a systematic way of establishing a partnership between people with diabetes and health care services. We piloted a systematic patient‐driven care planning consultation process in our routine specialist diabetes clinics. A document based on all key care processes was given to and completed by patients prior to their consultation. The completed document was used to structure the clinic consultation. Patient and doctor outcomes were assessed by post consultation questionnaires. Of 148 patients, 101 responded; 55 male, age 60 ± 12 years, duration of diabetes 12 ± 9 years, 67 on in...

Observations on age at diagnosis of type 1 diabetes and family history in a small population: the Winchester cohort
Abstract Clinical and demographic data from the records of 694 white Caucasian patients with type 1 diabetes attending a secondary care diabetes clinic between 1983 and 2010 have been audited to give information on age at diagnosis of type 1 diabetes and frequency of a family history of diabetes in their first degree relatives over five chronological decades between 1961 and 2010. All patients in the cohort lived in 12 postcode areas (six urban and six rural) in west Hampshire, and, although ascertainment is thought to be at least 90%, the exact at‐risk populations are not accurately known, so no incidence nor prevalence...

Monogenic diabetes and type 1 diabetes mellitus: a challenging combination
In conclusion, it is important to recognise the co‐existence of T1DM and GCK‐MODY as symptoms of hypoglycaemia at BG levels usually considered ‘within target’ pose management challenges. A combined diagnosis warrants careful consideration of BG target. Copyright © 2014 John Wiley & Sons, Ltd. (Source: Practical Diabetes International)

The Bermuda triangle of diabetes
Abstract Achieving better patient outcomes is something that often eludes us. We sail together with our patients through the rough seas of diabetes and its complications. As in the Bermuda triangle, both patient and health care professional can get lost at times; however, this process of being lost and finding our way back again can provide us with valuable lessons to become better at what we do and thus improve outcomes for the person with diabetes. This paper addresses the key elements of effective treatments for diabetes, the qualities needed to be a clinical champion, and the components needed to deliver high quality c...

‘A Safer Ramadan’: developing an integrated approach to support safer fasting and feasting for people with type 2 diabetes
Abstract In the United Kingdom, it is estimated that there are over 300 000 Muslim people with diabetes. Observance of Ramadan is an integral part of being a Muslim that involves abstinence from food and drink from dawn till dusk. In the UK over the next 10 years, Ramadan will fall in the heart of the summer months, resulting in longer fasting periods. For people with diabetes fasting can be problematic, yet few individuals receive advice from health care professionals on how to manage their diabetes during the Ramadan period. A ‘whole systems’ approach, involving community awareness, health care professional training ...
