Can laboratory based research regarding type 1 diabetes and exercise be applied into the real‐life environment?
Abstract The aim of this study was to determine whether results from laboratory based research examining glycaemic control during and after exercise can be applied to a real‐life (non‐laboratory) environment. A comparative study of individuals with type 1 diabetes (n = 9) using basal bolus analogue insulin regimens was undertaken. Glycaemic control before and after two 40‐minute runs at 70% VO2 max, in both laboratory and real‐life environments, was measured across 10 time‐points during and up to 12 hours after exercise. Insulin was adjusted in all participants following a self‐management algorithm. Pooled mean...

Diabetes education: a global perspective
Abstract Diabetes education has been acknowledged as the cornerstone of good diabetes care and management. Although evidence to support the need for diabetes education has grown in recent years, many people with diabetes remain unable to access or choose not to attend diabetes education for a variety of reasons. In order to improve the likelihood of success, diabetes education needs to be adequately resourced and tailored to meet the needs of specific populations. This paper seeks to offer a global perspective on the current status of diabetes education. Insight is provided into recent evidence suggesting poor integration ...

Managing factitious disorder and type 1 diabetes
We describe the impact of the disorder on their glucose control and general health, the challenges faced in diagnosing the psychiatric condition, and how the resulting situations were managed in a secondary care setting. We highlight the strategies often employed by patients in factitious disorder, and suggest counter‐strategies that can be implemented to minimise risk to patients. Ultimately, timely therapy for the factitious disorder could aid glucose control, and the overall health of the patient. Copyright © 2015 John Wiley & Sons. (Source: Practical Diabetes)

Train driving and diabetes – keeping safety on track
Abstract Despite disability legislation, some people with diabetes (particularly those taking insulin) still experience discrimination in the work that they are allowed to perform. Some occupations have strict medical standards that people with diabetes may not be able to meet. These usually apply where a job is unusually risky, e.g. because it involves operating heavy machinery or vehicles, with a risk of harm to the employee or others in case of sudden incapacity. People with diabetes have successfully challenged employers who sought to exclude them from jobs simply because they were ‘diabetic’, rather than assessing...

Improving the effectiveness of short‐term courses for multidisciplinary health care professionals
The objective of the study was to evaluate the short‐ and mid‐term impact of seven PD courses held between March 2011 and March 2012. Three methods of assessment were carried out: an end of course evaluation; pre‐ and post‐tests to assess gain in knowledge and change in attitudes; and self‐assessment of action plan implementation at six months to determine level of behaviour change. In total, 327 multidisciplinary HCPs participated in the seven PD courses. End of course evaluation indicated that the PD course was valued by course participants in terms of relevance of the topics and learning experience. A signific...

The challenges of type 1 diabetes in older people
Abstract Over 200 000 people in England and Wales have type 1 diabetes. After the discovery of insulin, it became clear that people with diabetes had a reduced life expectancy and in the 1950s someone with type 1 diabetes might only expect to live until their sixth decade before they died from diabetic complications. Since then, longevity has increased and significant numbers are living into their eighth decade. In 1985, Diabetes UK introduced the Nabarro medal for those who had lived with the condition for more than 50 years and, as evidence of increased life expectancy, the Macleod medal has been awarded since 2004 to su...

Insulin induced tan: a dual diagnosis
We present the case of a young male patient diagnosed with type 1 diabetes whose skin noticeably darkened with insulin therapy. Minimal insulin requirements, an unexpectedly low HbA1c and ongoing symptoms of weight loss and anorexia prompted further investigations. He was found to have a suboptimal response to synthetic ACTH (Synacthen) confirming a diagnosis of Addison's disease (primary adrenal failure). We postulate that the lowering of the blood glucose with insulin therapy for type 1 diabetes unmasked the diagnosis of Addison's disease. The case highlights several important lessons: (1) beware the dual diagnosis – h...

Bacillus cereus causing widespread necrotising skin infection in a diabetic person
We report a unique case of B. cereus cutaneous infection causing extensive skin necrosis in a person with type 2 diabetes mellitus. Examination revealed erythema, pitting oedema and right ankle tenderness. Elevated inflammatory markers supported a diagnosis of cellulitis, and intravenous flucloxacillin was initiated as per local hospital guidelines. Clinical response was poor with the development of spreading cellulitis, desquamated blisters and superficial necrosis. In response to swab culture and sensitivity results, vancomycin was commenced resulting in gradual resolution. Cutaneous necrotising B. cereus infection in a ...
