Structured diabetes patient education in Scotland
This article describes how Scotland is addressing the need to review structured diabetes education to support self‐management and record this on the national database. A National Education Coordinator (NEC) was appointed to lead the development of structured diabetes education. The NEC was supported by an education lead from the 14 health boards and representation from paediatric, minority ethnic and patient groups. Through literature searching, discussion, workshops and a consensus approach, the criteria for structured patient education were developed appropriate for the Scottish context. A process to review education p...

Dealing with diagnosis of diabetes
This article reviews the evidence of factors that facilitate a more helpful diagnosis conversation, describes the importance of this initial conversation on long‐term clinical outcomes, and offers suggestions drawn from psychological models on how to more effectively manage the diagnosis conversation. ‘Empathic willingness’ has been found to both assist the professional to feel less frustrated, and improve the patient's experience and clinical outcomes in the long‐term. ‘Empathic willingness’ can be taught and fostered by clinical psychologists (or their equivalents) yet are absent in the majority of settings i...

Type 1 diabetes and pregnancy: a phenomenological study of women's first experiences
This study has shown that most women with diabetes who become pregnant are resigned to the fact that their pregnancy is considered high risk, and are willing to play their part to achieve a positive pregnancy outcome. However, they would also like to ‘do the normal pregnant bit as well’, ‘normalise it and make it a nice experience’ and make it feel ‘less fragmented’. This woman‐centred experience of pregnancy care, in women with type 1 diabetes mellitus, may motivate health professionals to revise their approach to care, prompt them to utilise the skills of each individual member of the multidisciplinary team...

Overview of complementary and alternative medicine and diabetes
Abstract People with diabetes frequently use complementary and alternative medicine (CAM) and other non‐medicine CAM therapies, often combine various CAMs, and combine CAMs with conventional care. They use CAM for a range of reasons, not necessarily to manage blood glucose. The evidence base for CAM is improving, but there is still a lack of quality evidence for many CAMs, and the combination of CAMs and conventional care. CAMs have both risks and benefits: some CAMs, like some conventional treatments, are higher risk than others. Conventional practitioners should ask about CAM use in a non‐judgemental way, document it...

Social support, self‐efficacy and motivation: a qualitative study of the journey through HEALD (Healthy Eating and Active Living for Diabetes)
This study aimed to describe the influence of social support on participants’ self‐efficacy and motivation to perform physical activity before, during and after the Healthy Eating and Active Living for Diabetes in primary care networks (HEALD) programme. HEALD was a controlled trial of a 24‐week primary care‐based walking programme for people with type 2 diabetes, proven effective in increasing physical activity. Data used in this qualitative sub‐study were derived from semi‐structured interviews with purposefully selected HEALD completers (n = 13) six months after the programme ended. Qualitative data were ana...

Hearing loss and type 2 diabetes: is there a link?
Abstract Recent meta‐analysis has confirmed an association between hearing loss and diabetes. The cause remains uncertain and open to academic debate. This five‐year retrospective study examined the outcomes of patients referred for audiological investigations from a large primary care diabetes clinic. Audiological assessment included pure‐tone audiometry, and the presence of neuropathy was identified by foot examination and risk stratification. Referral rates of patients with diabetes were nearly twice those seen in the non‐diabetic population (7.5% [107/1428] vs 4% [499/12422]). Hearing loss was identified in 77 ...
