Early‐onset dementia in type 1 diabetes mellitus
Abstract This case displays the difficulty of managing type 1 diabetes with young‐onset dementia, withco‐existing medical challenges that have an effect on each other. We outline current evidence on how hypoglycaemia could have potentially led to the patient's cognitive impairment, and demonstrate the difficulties that may be encountered in the management of recurrent hypoglycaemia in diabetic patients with dementia due to both declining cognition and associated psychiatric symptoms. In the light of this we would call for additional research to aid in the management of this complex and increasing cohort of patients. Co...

Addition of SGLT2 inhibitor to GLP‐1 agonist therapy in people with type 2 diabetes and suboptimal glycaemic control
In conclusion, patients with type 2 diabetes who require further improvement in glycaemic control despite the addition of a GLP‐1 RA to their regimen can be considered for an SGLT2 inhibitor. The mechanism of action may be synergistic and has the advantage of promoting further weight loss. Copyright © 2016 John Wiley & Sons. (Source: Practical Diabetes)

Coconut water drink and the risk of hyperkalaemia in diabetes
Abstract Coconut water is available as a substitute for table water and as a sports rehydrating drink. Because of the nature of its micronutrients, it may lead to biochemical changes that may not be beneficial for all groups of people. A patient with type 2 diabetes mellitus consumed daily around one litre of coconut water drink. As a result, there was a gradual increase in serum potassium. On cessation of beverage consumption, serum potassium decreased to within the reference interval. However, an increase in urea and creatinine concentration did not revert to the level seen prior to coconut water consumption. There was a...
