Lung Carcinoma Dural Metastasis and Meningioma Mimicking Bilateral Falcine Meningiomas
We present a case of bilateral occipital lesions which, on the imaging, mimic bilateral falcine meningiomas. However, the histopathologic findings confirmed that the left-sided lesion originating from the falx cerebri was meningioma, whereas the right-sided falcine lesion was lung carcinoma metastasis. The bilateral falcine neoplasms are generally the single histopathologic type, mainly meningioma. It should be emphasized that the bilateral falcine mass lesions occurring in patients with known potential malignant tumors elsewhere in the body have a small but real chance of being 2 distinct types, which is helpful for treat...

Effective Use of Multimodal Intraoperative Neuromonitoring in Neurosurgery
Conclusions: We acquired the experience of multimodal neuromonitoring techniques, especially in high-risk neurosurgeries in coordination with surgery and anesthesia crews. Our preoperatory data and postoperative outcomes suggest that intraoperative monitoring provides benefits to the surgeon for minimal neural injury. (Source: Neurosurgery Quarterly)

Major Complications and Sequelae After Total En Bloc Spondylectomy for Malignant Spinal Tumors
This retrospective study looked at 33 consecutive patients with malignant, aggressive tumors in the thoracic and lumbar spine who underwent TES (24 men and 9 women; mean age 49.9 years; mean follow-up 3.6 years; 14 primary tumors and 19 metastatic tumors). TES was performed by a posterior-only approach in 15 patients and by a combined anterior and posterior approach in 18 patients. We classified complications as intraoperative, early (within 1 month after surgery), or late (>1 month after surgery), and examined the relationships between major complications and potentially related factors. Ten died of disease at 22.3 months...

Double Meningomyelocele: Unusual Case
A full-term newborn boy was born with 2 meningomyelocele defects. The first one was a 2.5×2 cm defect located in the upper dorsal area and the other defect was a 12×10 cm defect in the lower dorsal-lumbosacral area. Neurological examination of the lower extremities showed paraplegia with hypotonia. Lesions were operated upon and closed by advancement flaps. Clinical and intraoperative findings of this extremely rare case are discussed. (Source: Neurosurgery Quarterly)

Unusual Cause of Interhemispheric Subdural Hematoma: A Case Report
We report a rare case with ISH caused by migration of subdural hematoma (SDH). Serial computed tomography scans demonstrate rapid resolution of a right-sided acute SDH and equally rapid presentation of an ISH with obvious midline shift. Emergent right-sided decompressive craniectomy was performed and an intraparenchyma sensor detected an intracranial pressure as 46 mm Hg before removal of skull bone flap. Three months after surgery, this patient could spontaneously open eyes, alert to noise, and localize painful stimuli. The case shows that an extremely high intracranial pressure could result in migration of a SDH to an IS...

Effect of Previous Herpes Zoster on Radiofrequency Thermocoagulation of Gasserian Ganglion for Trigeminal Neuralgia
Objective: The aim of this study is to investigate the clinical characteristics, outcomes, and complications of patients with trigeminal neuralgia who underwent radiofrequency thermocoagulation of gasserian ganglion with and without previous herpes zoster. Materials and Methods: The authors analyzed 269 patients with trigeminal neuralgia treated by radiofrequency thermocoagulation of gasserian ganglion in Xinhua hospital between January 1, 2010 and December 31, 2012. A total of 37 patients were secondary to herpes zoster (group I), and 232 patients were primary to trigeminal neuralgia (group II). Clinical outcome data eval...

Clinical Study of a New Transpedicular Nonfusion Posterior Dynamic Stabilization System for Treating Herniated Lumbar Intervertebral Disks
In this study, we evaluated the 12-month clinical outcomes of patients treated with the K-Rod, a pedicle-based dynamic stabilization system. Patient participation was based on the presence of a herniated lumbar intervertebral disk complicated with endplate osteochondritis at L4-L5 or L5-S1. The clinical outcome at each assessment was assessed by Visual Analogue Scale to measure back pain, and the Oswestry Disability Index and range of motion to measure functional status preoperatively and postoperatively. Thirty-three patients (20 men and 13 women) with a mean age of 45.5 years were included. The mean Visual Analogue Scale...

Second Impact Syndrome: A Rare, Devastating Consequence of Repetitive Head Injuries
Sports-related concussion has been viewed as a developing public health crisis in recent years. Underrecognition of concussions can lead to premature clearance for athletic participation. Second impact syndrome (SIS) represents a rare, yet devastating, potential outcome of premature return to play. SIS is a condition in which rapid brain swelling occurs as a result of a repeat head injury sustained before symptoms of a previous head injury have resolved. Within minutes of the second impact, diffuse cerebral swelling, brain herniation, and death can occur. There are (Source: Neurosurgery Quarterly)

Percutaneous Reconstruction of the Stenotic Initial Segment of the Right Subclavian Artery Using the Simultaneous Kissing Stent Technique
We report a rare case of stenosis of the initial segment of the right subclavian artery treated successfully using the simultaneous kissing stent technique. A 61-year-old man with a history of systematic hypertension, hyperlipidemia, and alcohol abuse suffered recurrent and aggravated vertigo. Angiography showed severe stenosis of the initial segment of the right subclavian artery and steal from the right vertebral artery. Thus, subclavian and carotid artery stents were placed simultaneously using the simultaneous kissing stent technique. Angiography revealed excellent dilatation of the stenotic segment of the right subcla...

Simultaneous Occurrence of Chiari Type I Malformation and Classic Medulloblastoma
We present here a rare case of an adult patient with Chiari type I malformation and classic medulloblastoma. A 22-year-old woman underwent magnetic resonance imaging (MRI) because of a variety of symptoms. Results from this MRI led to the diagnosis of Chiari type I malformation. Two years later, she was readmitted to the hospital with complaints of gait disturbance. She received another MRI that showed herniation of the cerebellar tonsils and a tumor mass in the right cerebellar hemisphere. Microsurgical treatment was performed for both the bilateral herniated cerebellar tonsils and tumor excision. Postoperative pathologic...

Evaluation of Visible Physiological F-18 FDG Uptake Patterns in Spinal Cord on PET/CT
Conclusions: Visible mild FDG uptake in the spinal cord is a common finding in PET/CT imaging. No significant association with age or sex was observed. Such physiological uptake can appear mostly at the cervical and T11-L1 vertebral levels. At these levels, moderate uptake is also frequently encountered. However, in the upper thoracic regions, only mild uptake is observed. (Source: Neurosurgery Quarterly)

Neuroendoscopic Removal of Astrocytoma in Unilateral Ventricle in a Patient With Enlarged Perivascular Spaces: A Case Report and Review of the Literature
The authors report a case of astrocytoma, located in the right lateral ventricle in a 22-year-old man who presented with headache. Magnetic resonance imaging (MRI) also showed that the patient had multiple enlarged perivascular spaces. The patient underwent endoscopic removal through an aquiline incision placed 3 cm from the midline and 3 cm in front of the coronal suture. The histologic examination of tumor revealed a WHO I-II glioma. Despite the postoperative intraventricular hemorrhage and catheter infection, he was discharged home without neurologic deficits. The endoscopic fenestration rather than excision should be c...
