Unusual Cause of Interhemispheric Subdural Hematoma: A Case Report

We report a rare case with ISH caused by migration of subdural hematoma (SDH). Serial computed tomography scans demonstrate rapid resolution of a right-sided acute SDH and equally rapid presentation of an ISH with obvious midline shift. Emergent right-sided decompressive craniectomy was performed and an intraparenchyma sensor detected an intracranial pressure as 46 mm Hg before removal of skull bone flap. Three months after surgery, this patient could spontaneously open eyes, alert to noise, and localize painful stimuli. The case shows that an extremely high intracranial pressure could result in migration of a SDH to an ISH. This relocation of hematomas may also be an important indication for irreversible brain damage and poor prognosis of the patient.
