A near-fatal case of emphysematous pyelonephritis: Embracing the new management ‘gold standard’ – Saving the life while saving the kidney!
We report on the presence of EPN (Grade 3a) in a patient with poorly controlled diabetes, who presented in septic shock due to EPN. Although previous older treatment algorithms would proceed with an immediate nephrectomy, based on her grade, she was successfully managed by incorporating the new treatment ‘gold standard’: medical management with percutaneous drainage. A high index of suspicion is required to detect and manage this life-threatening condition associated with diabetes, since the presenting symptoms may be non-specific. Significance and clinical value of the transitional zone volume (TZV) or index (TZI) in assessing the degree of lower urinary tract obstruction: Revisited
Conclusion Estimating the transition zone volume during TRUS is a reasonable way to obtain the required information about the TZI. Calculating the TZI could not be directly correlated with any of the different parameters, making the clinical value of such an index questionable. The observation that the obstructed and the retention groups both had a TZI above 0.5 deserves further research that can help in the classification of patients into obstructed and non-obstructed. Living donor kidney transplantation in the hemodialysis-naive and the hemodialysis-exposed: A short term prospective comparative study

An unusual presentation of advanced prostate cancer in a 56-year old Nigerian
We report an atypical case of a 56- year old Nigerian male who had bowel obstruction, multiple peripheral and intra-abdominal lymphadenopathies. The patient had a normal initial urological evaluation but his diagnostic conundrum was resolved to be prostate cancer by immuno-histochemistry of the cervical lymph node biopsy and he did well after antiandrogen monotherapy. Retrocaval ureter with vesicoureteric reflux, a very rare entity
Conclusion In a child presenting with retrocaval ureter we should look for associated VUR. The problem is similar to pelviureteric junction obstruction and ipsilateral reflux. Preliminary study of the efficacy of the combination of tamsulosin and trospium as a medical expulsive therapy for distal ureteric stones
