Hospital transfusion medicine committees – problems and pitfalls
Hospital Transfusion Medicine Committees (TMCs) are multidisciplinary committees that provide oversight of a variety of clinical activities as they pertain to transfusion medicine and patient care in hospital environments. Across the globe, there are many different standards that provide guidance on the goals, structure and function of hospital transfusion medicine committees but these are not always implemented in the same manner. Screening for fetal and neonatal alloimmune thrombocytopenia: is it possible and what are the potential outcomes?
Fetal and neonatal alloimmune thrombocytopenia (FNAIT) is a rare but potentially very serious bleeding condition affecting the fetus/newborn. The vast majority (>80%) of FNAIT cases among Caucasians, and the most serious one, are caused by maternal antibodies to the human platelet antigen 1a (HPA‐1a). The clinical consequences of FNAIT span a continuum from no symptoms to intracranial haemorrhage (ICH) and intrauterine death. Mitochondrial damage ‐associated molecular patterns in blood transfusion products
Mitochondria are organelles in charge of energy supply and the control of apoptosis. Owing to their similarities with bacteria, however, extracellular mitochondria are considered damage‐associated molecular patterns (DAMPs) capable of activating the immune system and non‐immune cells. Studies revealed that diverse blood products contain extracellular mitochondria, which could account for the adverse reactions that can occur in transfusion. Effectiveness of predonation health and travel screening in reducing the risk of transfusion ‐transmitted infections
Early in the history of transfusion it became evident that blood can transmit infectious diseases from the donor to the recipient. Predonation screening is performed by a Donor Health Questionnaire (DHQ). The DHQ complements the blood screening programme, when it fails to detect transfusion‐transmitted infections (TTIs), or when TTIs are not covered by the blood screening programme.
