Transvaginal hysterotomy for cesarean scar pregnancy in 40 consecutive cases
Abstract To propose a novel procedure as a safe and effective treatment for cesarean scar pregnancy (CSP), a cohort study was initiated in patients diagnosed with CSP and treated with transvaginal hysterotomy from December 2009 to March 2013, either as a primary or secondary therapy. All diagnoses were confirmed by both sonography and pathology, either a gestational sac or residual tissue after termination of pregnancy or miscarriage in the cesarean section scar. Basic clinical characteristics and perioperative data were collected and analyzed. A total of 40 patients were included. The mean age was 32.88â...

Outcome and medium-term functional results of anterior rectopexy for rectocele repair
Abstract This retrospective observational study was designed to assess patients’ satisfaction after anterior rectopexy, with or without sacropexy, for treatment of symptomatic rectocele in a group of women with anterior rectoceles. Between February 1995 and March 2011, all female patients who underwent an anterior rectopexy for large (>grade 2) anterior rectocele were included in the study. Preoperatively, the rectocele was assessed at rectovaginal examination and a defecography was performed. Rectovaginal examination was repeated in all patients 6 weeks postoperatively and a control defecography wa...

Hysteroscopic diagnosis and excision of myometrial cystic adenomyosis
Abstract In 1908, Cullen described the first cases of cystic adenomyosis in his textbook on adenomyomata. Although not very common, with the introduction of noninvasive imaging techniques such as magnetic resonance imaging (MRI) and 3-D transvaginal ultrasound, an increasing number of cases have been reported. Patients primarily complain of severe dysmenorrhea, chronic pelvic pain, and dysfunctional uterine bleeding. Currently, it is unclear whether adenomyosis and, more specifically, cystic adenomyosis can be an underlying reason for impaired fertility and reproductive outcome. With the postponement of c...

Locoregional treatment of breast cancer during pregnancy
Abstract The management of patients with breast cancer during pregnancy is very demanding and it should be better performed in highly qualified and experienced centers. Referral to institutes and physicians trained in this special clinical scenario allows reducing the risk of both overtreating and undertreating the patients. Moreover, patients can receive appropriate information regarding safety of treatments without old-fashioned taboo. The purpose of the current paper is to discuss the main issues concerning surgical management and in general locoregional treatment of patients diagnosed with breast canc...

Cryptomenorrhea with cervicovaginal aplasia: endoscopic transfundal development of the lower genital tract
In conclusion, TFH is a safe and feasible procedure as a harmonizing technique during endoscopic canalization of cervical atresia with or without BV in cases of obstructive MĂĽllerian anomalies. The procedure accelerates the drainage of uterine contents and localizes the correct site of draining catheter. (Source: Gynecological Surgery)

An unnecessary ouch: is routine outpatient endometrial sampling indicated where hysteroscopy reveals an atrophic cavity?
Abstract There is currently little guidance in the literature as to whether routine endometrial sampling is indicated after a normal or atrophic cavity has been visualised at outpatient hysteroscopy for post-menopausal bleeding. Retrospective electronic database review of women with post-menopausal bleeding attending an outpatient hysteroscopy clinic was performed. Seventy-nine patients underwent outpatient hysteroscopy for post-menopausal bleeding. Forty-four of these (56 %) were found to have normal or atrophic cavities with no other visible pathologies. Forty of these women (89 %) underwent endometri...

Ampullar ectopic pregnancy following ipsilateral partial isthmic salpingectomy
We present a rare case of a recurrent ectopic pregnancy, occuring in the distal remnant of the ampulla of the fallopian tube, after a previous partial ipsilateral salpingectomy. This case shows that a strict adherence to the Dutch guideline would probably have led to an earlier detection. (Source: Gynecological Surgery)

The risk of uterine rupture after myomectomy: a systematic review of the literature and meta-analysis
This study seeks to review the risk of uterine rupture during pregnancy and/or labor after laparoscopic myomectomy in comparison with laparotomic myomectomy. A systematic search of the literature through search strategies in MEDLINE (PubMed) and Embase (Embase.com) from January 1970 up to March 2013 was performed. We used the following MeSH terms and free text words: gynecologic surgical procedures, gynecologic surgery, leiomyoma, fibromyoma, fibroids, myoma, uterine tumors, and pregnancy complication. Our data show that the risk of uterine rupture during labor and delivery is low (0.75 %). Compared with traditional open ...

Ultrasound-assisted intraoperative localization and laparoscopic management of a previously missed unruptured retroperitoneal ectopic pregnancy
We report the case of an early first-trimester retroperitoneal broad ligament live pregnancy occurring after spontaneous conception in a patient who had a history of an ipsilateral tubal ectopic pregnancy, previously treated with laparoscopic right salpingectomy. Current gestation had been missed during initial laparoscopy, and was located and removed during a repeat laparoscopic procedure under intraoperative ultrasonographic guidance. Closed entry technique for the laparoscopic management of adnexal mass during pregnancy
Abstract Management of adnexal masses during pregnancy is challenging for most gynecologists. When surgery is needed, a minimally invasive approach should be preferred. The aim of this study was to evaluate the safety and feasibility of the closed entry technique for laparoscopic management of adnexal masses during pregnancy. We reviewed clinical records and videos of laparoscopic procedures performed during pregnancy. Seventeen pregnant patients with diagnosis of adnexal mass that required surgery underwent laparoscopic surgery using the closed entry or Veress technique. We searched for complications r...

Proposal of a modified transcervical endometrial resection (TCER) technique for menorrhagia treatment. Feasibility, efficacy, and patients’ acceptability
Abstract The aim of this study is to evaluate the feasibility, efficacy, safeness, and patients’ acceptability of a modified transcervical endometrial resection (TCER) technique for the treatment of menorrhagia. Eighty-four premenopausal women with menorrhagia after careful investigation and 2 months therapy with GnRHa underwent a modified TCER. It was performed with a standard dual channel, 26 French irrigating resectoscope (Karl Storz, GmbH, Germany) after cervix dilatation to 10 mm and sorbitol mannitol solution used as distension medium. The modified technique was based on the resection of the e...

Routine vaginoscopic office hysteroscopy in modern infertility work-up: a randomized controlled trial
This study aims to estimate the safety, efficacy, and patient acceptability of adding vaginoscopic office hysteroscopy (VOH) to the routine infertility diagnostic work-up for evaluation of the endometrial cavity. This study is a prospective comparative diagnostic trial. This study was conducted in a tertiary care referral facility and university hospital. This study comprised a total of 156 infertile patients scheduled for laparoscopy. Seventy-eight patients had VOH on one stop bases in addition to the usual infertility work-up and were assigned as group B while a similar number was examined by the usual diagnostic work-up...
