The role of video games in facilitating the psychomotor skills training in laparoscopic surgery
AbstractSeveral studies have evaluated the impact of video games (VGs) on psychomotor skills in laparoscopic surgery. Our review of literature revealed that prior VGs experience ameliorates the initial skill level of novice trainees and accelerates their learning curve by facilitating a better perception of 3D images, ambidexterity, and tone reflexes. The published studies are unfortunately limited by the small number of participants, the heterogeneity of the sample, the endpoints, and the evaluation tools of the studies. Randomized control studies with strict inclusion criteria and valid evaluation tool are required befor...

ESGE survey on tissue power morcellation complications other than leiomyosarcoma
AbstractThe exact incidence of power morcellation complications (PMC) is unknown and probably underestimated. Medical literature mainly describes case reports and the vast majority of complications after tissue power morcellation are not reported. ESGE has run a survey among its members about complications emerging after laparoscopic electromechanical morcellation including the risk of leiomyosarcoma (LMS). The reported risk of a sarcoma after myoma or uterus morcellation is low and presented in a separate article. The Central office using the ESGE server and website, activating the ‘Survey Monkey’ programme, sent a re...

Ovarian remnant syndrome: a retrospective evaluation of surgical management
The objective of the study is to report on patient characteristics, surgical findings, pathology, and recurrence of ovarian remnants. This is a retrospective case series completed at an academic tertiary care hospital. Seventeen patients were identified between September 2005 and December 2015 with ovarian remnant syndrome using a diagnosis code search at one institution. All patients underwent surgical excision. Pathology confirmed ovarian tissue. Three recurrences were treated non-surgically. All patients had a history of endometriosis and previous surgeries. The average number of laparotomies, laparoscopies, and cesarea...

The diagnosis of endometrial atypia by hysteroscopy with guided biopsy in postmenopausal patients with hyperplasia: a reliable practice?
This study assesses the accuracy of histological diagnosis of endometrial atypia in hysteroscopy with blind biopsy, in postmenopausal women with an endometrial thickness (ET) greater than 5  mm. In order, to determine the relationship between ET and atypia more precisely, ET was subdivided in three categories of 5–7 mm, ≥7–9 mm and>9  mm. Ninety-nine postmenopausal patients, aged 51–79 years, with abnormal uterine bleeding and an endometrial thickness>5  mm in whom a diagnosis of endometrial hyperplasia, with or without atypia, was established with hysteroscopy with biopsy, underwent subsequent hysterec...

An unusual clinical presentation and histological findings provide insight into the pathogenesis of endometriosis
AbstractThe aetiology of endometriosis is poorly understood resulting in a number of different models of pathogenesis. None of these models completely explains all of the available evidence. As such, there remains significant uncertainty about the origins and development of the disease, and in particular the involvement of the lymphatic system in that process. A 37-year-old patient with known endometriosis presented with a rapidly growing endometrioma following previous surgical drainage. The cyst enlarged over 5  months to extend above the umbilicus, necessitating a mid-line laparotomy and right salpingo-oophorectomy for...

Posterior colpotomy for the retrieval of ovarian specimens in laparoscopy: a large case series
AbstractEffective tissue retrieval techniques continue to be a challenge in laparoscopic surgery. We aimed to determine the safety and efficacy of ovarian specimen retrieval via posterior colpotomy. This is a large case series study conducted in a large district general hospital. All patients in our unit who had ovarian specimens retrieved via posterior colpotomy over a 6-year period were included for analysis. A retrospective case notes review was performed. Information was collected on patient demographics, risk of malignancy indices, radiological size of the ovarian specimen, surgical spillage, postoperative complicatio...

Minimally invasive technique for the reconstruction of the cervix in cervical atresia
We describe a case of cervical atresia who had multiple surgeries before this case was treated by using a minimal invasive technique by inserting gastrostomy tube with guide wire through the vagina and inflate the gastrostomy balloon inside the uterus to create a patent cervical tunnel, under ultrasound guidance. The patient recovered and was observed for 10  months after the procedure and developed a regular cycle with no cyclical pain. (Source: Gynecological Surgery)

Effectiveness of thermal balloon ablation versus NovaSure endometrial ablation in different age groups
In conclusion, NA ablation is more effective than TBA in achieving amenorrhoea in younger women and should be the preferred treatment option. Failure of abla tion and/or hysterectomy rates were higher in younger women, especially those with dysmenorrhoea and after TBA. This highlights the necessity for careful selection of the treatment modality especially for younger women with HMB and for clear patient counselling. (Source: Gynecological Surgery)

Pfannenstiel Incision scar characteristics: relation to pelvic adhesions after cesarean section
AbstractCurrent evidence shows that depressed scars are positively associated with intra-abdominal adhesions. Abdominal wound characteristics following cesarean section can predict the presence of abdominal adhesions. However, given the small number of published studies, further research is needed. The purpose of this report is to show the relation between cesarean scar characteristics and pelvic adhesions. Depressed scars are associated with intra-abdominal adhe- sions, while flat scars are correlated with its absence. (Source: Gynecological Surgery)

Uterine artery embolization for symptomatic uterine myomas using gelfoam pledgets alone vs. embospheres plus gelfoam pledgets: a randomized comparison
AbstractThe aim of the study is to evaluate the feasibility, tolerability, and clinical outcomes of uterine artery embolization (UAE) using gelfoam (G) alone versus embospheres  + gelfoam (E + G) in women with symptomatic uterine fibroids. Prospective, patient blinded, randomized controlled pilot study (Level-I). University Affiliated Teaching Hospital. Fifty-nine women with symptomatic uterine fibroids; G (n = 31) vs. E + G (n = 28). Fluoroscopy-guided trans-femoral artery UAE was performed under intravenous sedation, local anesthesia, and overnight patient-controlled analgesia (PCA) pump using either gelfoam pl...

Therapeutic hysteroscopy in an outpatient office-based setting compared to conventional inpatient treatment: superior? a cohort study
AbstractSince the introduction of smaller instruments, hysteroscopy is increasingly performed in an office-based setting. The aim of this cohort study was to compare operative hysteroscopy in an office-based setting with inpatient procedures to evaluate differences in procedure and analgesia-related parameters. All office-based hysteroscopic procedures during February 2014 to October 2015 were entered for analysis. Included were morcellation of fibroids, polyps and pregnancy remnants, synechiolysis, diagnostic hysteroscopy, and endometrial ablation. Comparative cases of patients undergoing hysteroscopic surgery in the oper...

Resumption of menstrual cycles after 14  years of amenorrhea in a woman infected with Mycobacterium africanum
We report a woman with a narrow and tubular uterine cavity due toMycobacterium africanum. The endometrium was fibrotic leading to amenorrhea  of over a decade. Despite treatment with multiple anti-tuberculosis medication and hysteroscopic adhesiolysis, menstrual cycles resumed only a few years later. It shows that the endometrium regenerates overtime.
