Dilemmas in management of bilateral ectopic pregnancies—report of two cases and a review of current practice
We report two such rare cases of simultaneous bilateral ectopic pregnancies with different management and outcomes highlighting the fact that these cases not only pose diagnostic and management challenges but also has complex ethical issues associated with it. (Source: Gynecological Surgery)

Role of prophylactic antibiotics in endoscopic gynaecological surgery; a consensus proposal
Abstract Surgical site infection can result in increased morbidity for the patient, prolonged hospital stay and hospital readmission. Preoperative antibiotics reduce the incidence of such infections, particularly in open surgery. Universal use of antibiotic prophylaxis, however, is not recommended due to the risks of adverse reactions, generation of resistant bacteria and additional cost. Endoscopic procedures carry low risk of wound contamination and infection. Limited data suggest wide variability in antibiotic prophylaxis in gynaecological surgery and potential overuse of antibiotics in gynaecologica...

Laparoscopic ovarian reconstruction without suturing after cystectomy for endometrioma
Abstract The primary aim of this study is to evaluate the technique of ovarian reconstruction without suturing after laparoscopic cystectomy of endometrioma. The secondary aim is to find the pregnancy rate following this technique. The study is a prospective observational study (Canadian Task Force classification II-3). The interventions used in the study are laparoscopic ovarian cystectomy and reconstruction without suturing. Laparoscopic ovarian cystectomy was performed in 240 patients between May 2007 and April 2012 of which 182 consecutive patients who met the selection criteria were enrolled in the...

A comparative study of Essure® hysteroscopic sterilisation versus laparoscopic sterilisation
This study includes a retrospective case–control comparative study of 70 patients who had laparoscopic or hysteroscopic sterilisation performed. Systematic chart review for the documentation of preoperative counselling, operative time, intraoperative complications, documentation of correct application of Essure® and Filshie® clips and duration of hospital stay was also done. Patient follow-up was arranged and a questionnaire completed including details of postoperative pain, satisfaction of procedure, recovery time and compliance with confirmatory hysterosalpingogram attendance and associated pain. The main outcome mea...

Laparoscopic management of an 11-week rudimentary uterine horn pregnancy using extracorporeal Roeder knot to secure the dilated vascular pedicle
We report a case of an unruptured 11-week rudimentary horn pregnancy that was diagnosed and treated laparoscopically. We have performed a literature review using PubMed, Embase and Cochrane Database of Systematic Reviews to identify relevant cases and draw conclusions with regards to their management. We have collated 20 published cases of rudimentary horn pregnancies that were managed by laparoscopy. The surgical technique appears consistent among these cases with few variations. In advanced gestations, feticide may need to be performed. Morcellation has been shown to be possible without compromising patient safety from t...

A randomised trial of Medgyn Endosampler® vs Endocurette® in an outpatient hysteroscopy clinic setting
Abstract Outpatient hysteroscopy and endometrial biopsy are increasingly being used in the investigation of abnormal uterine bleeding. In our unit, both Endocurette® and Endosampler® endometrial biopsy devices are available in the outpatient hysteroscopy clinic. Literature comparing these devices is lacking. This was a prospective, randomised trial involving women attending the outpatient hysteroscopy clinic at Cork University Maternity Hospital. Women were randomised to endometrial sampling with either Endosampler® or Endocurette® devices. A number of device insertions, pain scores, ease of handlin...

Efficacy of ovarian suspension to round ligament with a resorbable suture to prevent postoperative adhesions in women with ovarian endometrioma: follow-up by transvaginal hydrolaparoscopy
Abstract The aim of this study was to assess the effect of ovarian suspension to the round ipsilateral ligament with a resorbable suture, performed during laparoscopic surgery for endometrioma, on postoperative ovarian adhesion formation. The tool used to assess this effect was not conventional laparoscopy but outpatient transvaginal hydrolaparoscopy. Fifty women with single ovarian endometrioma were divided in two groups (group A, 24 and group B, 26). All patients underwent laparoscopic ovarian cystectomy for endometriosis. In group A, the ovary was suspended to the ipsilateral round ligament. In group B...
