Ivabradine in the management of coronary artery disease with or without left ventricular dysfunction or heart failure
Heart rate reduction is an integral part of antianginal therapy. In this article, we examine currently available data related to the use of ivabradine in patients with angina pectoris with and without left ventricular dysfunction (LVD) or heart failure (HF). We explore results from large morbidity–mortality trials of ivabradine in stable coronary artery disease (CAD) without HF (SIGNIfY), stable CAD with LVD (BEAUTIfUL), and systolic HF (SHIfT). While ivabradine did not improve outcomes in the main population in SIGNIfY, analyses of the antianginal effect of ivabradine in a pre-specified angina subgroup (n = 12 049) ...

The role of ivabradine in improving myocardial perfusion, adding to the antianginal benefits
This article discusses the role of ivabradine in improvement of myocardial perfusion based on data drawn from relevant preclinical and clinical investigations. Experimental data suggest that heart rate reduction with ivabradine causes greater increase in duration of diastolic perfusion time and coronary flow compared with β-blockade due to absence of impairing of left ventricular contractility and coronary vasomotion. In consequence, the full benefits of heart rate reduction for improved coronary perfusion could be realized with ivabradine. Following experimental data, clinical studies demonstrated anti-ischaemic and ...

Angina treatments and prevention of cardiac events: an appraisal of the evidence
Angina pectoris is the symptomatic manifestation of transient myocardial ischaemia. At the most fundamental level, angina arises when myocardial oxygen demand exceeds the ability of the coronary circulation to provide adequate oxygen delivery to maintain normal myocardial metabolic function. In vivo, the balance of oxygen demand and delivery is a complex physiological process that can be altered by a variety of interventions. Lifestyle modification is a cornerstone of cardiovascular disease management, with or without angina. Additional pharmaceutical and physical interventions are usually applied to patients with angina. ...

The ivabradine journey
Large body of epidemiological evidence showed strong correlation between heart rate and total or cardiovascular (CV) mortality. These data do not, however, clarify the role of increased heart rate in CV disease, basically because they do not distinguish whether increased heart rate is a risk marker or a risk factor. The distinction is relevant for therapeutic value, since if it is the cause—or one of the causes—of the disease and its progression; its reduction corresponds to a prognostic benefit. Ivabradine, a selective heart rate-lowering agent, provides an opportunity to assess the effects of lowering heart r...

Image quality of 256-multidetector computed tomography in patients with atrial fibrillation: an initial experience
The objective of this study was to evaluate and compare the quality of 256-MDCT images obtained in patients with AF and in a control group of patients in sinus rhythm. Two reviewers were blinded to the patient groups and evaluated images of 15 coronary artery segments for each patient using 256-MDCT. The images were scored subjectively as 1 or 2, meaning of diagnostic quality, or 3, meaning poor quality. The effective doses of all patients were calculated. No statistical difference between the groups was noted in patient age (P = 0.318). The heart rates were 74.1 ± 8.5 (SD) and 66.4 ± 7.5 (SD) beats per minut...

Impact of coronary calcifications 12 months after everolimus-eluting stent implantation: an optical coherence tomography study
To assess the endothelial coverage and neointimal proliferation at 12 months after everolimus-eluting stent (EES) implantation in patients with coronary calcifications by optical coherence tomography (OCT). Seventeen patients who received an EES in culprit coronary lesions of acute coronary syndrome (ACS) underwent follow-up OCT examination at 12 months. Eleven patients in whom spotty calcifications were detected in stent segment vessels by OCT were included in the final analysis. The percentage of uncovered struts (6.0 vs. 2.0%, P < 0.001) and the stent symmetry index (0.883 ± 0.040 vs. 0.913 ± 0.026, P =...

Electroacupuncture pre-treatment ameliorates myocardial ischaemia/reperfusion injury through regulation of cannabinoid receptor type 2
We reported that EA pretreatment significantly ameliorated MI/R injury, evidenced by increased cardiac function, reduced infarct size, and decreased apoptosis. This was associated with a reduction of oxidative stress in the infarcted myocardium, which was through CB2. The CB2 expression was up-regulated in the heart after EA pretreatment, as well as heart tissue content of N-arach-idonoylethanolamine-anandamide and 2-arachidonylglycerol. Furthermore, the expression of adenosine monophosphate-activated protein kinase (AMPK) and peroxisome proliferator-activated receptor coactivator-1α (PGC-1α) in the left ventr...

Comparison of early and delayed transplantation of adipose tissue-derived mesenchymal stem cells on pulmonary arterial function in monocrotaline-induced pulmonary arterial hypertensive rats
The aim of this article was to investigate the effects of early and delayed transplantation of adipose tissue-derived mesenchymal stem cells (ADMSCs) on pulmonary vasomotion in monocrotaline (MCT)-induced pulmonary arterial hypertensive (PAH) rats. Adipose tissue-derived mesenchymal stem cells were isolated from inguinal subcutaneous adipose tissue of Sprague-Dawley (SD) rats. Eighty male SD rats were divided into four groups (n = 20 per group): normal control (Ctr), PAH, early (at day 7, ADMSCs-7), and delayed (at day 14, ADMSCs-14) ADMSC intervention. PAH was induced by a single dose intraperitoneal injection of 40 mg/kg...

Expert panel of the Chinese Society of Cardiology recommendations on beta-blocker therapy in patients undergoing non-cardiac surgery
The initiation of beta-blockers in the perioperative period of non-cardiac surgery remains controversial. After reviewing all the evidence, the expert panel of the Chinese Society of Cardiology (CSC) believes that there lacks high-quality randomized trial and the use of perioperative beta-blocker can only be considered on a case-by-case basis. Five practical recommendations are presented. (Source: European Journal of Heart Failure Supplements)

Gulf Heart Assocation 2015 Conference Proceedings Abstracts
Conclusion In the field of management of coronary artery disease percutaneous coronary intervention (PCI) is the internationally recognized standard treatment worldwide for more than last 3 decades. Our result of PCI correlates well with the other studies worldwide, though the study population is not big enough & there are many lacks of logistics in our country. Complications during and in-hospital period are very few. (Source: European Journal of Heart Failure Supplements)
