Plasma auto-antibodies to angiotensin II receptors are correlated with blood pressure and inflammatory factors in hypertension patients
Auto-antibodies against to angiotensin II type 1 receptor (AT1R-AA) have been discovered in patients with hypertension and they have a close relationship with inflammatory factors. However, auto-antibodies to angiotensin II type 2 receptor (AT2R-AA) are seldom investigated in hypertension. Subjects (n = 138) were enrolled and divided into three groups according to their blood pressure levels by using the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC-7) criteria: normotensive (≤120/80 mmHg, n = 31), pre-hypertensive (120–139/80–8...

Clinical outcomes and risk factors of periprocedural myocardial injury after successful percutaneous coronary intervention for chronic total occlusions
Limited published data about periprocedural myocardial injury (PMI) after successful chronic total occlusion (CTO)-percutaneous coronary intervention (PCI) were available. The study aimed to investigate risk predictors and clinical implications of PMI after successful CTO-PCI and drug-eluting stent (DES) implantation. Between 2009and 2012, a total of 437 patients who underwent successful recanalization with DESs were included. All the cardiac troponin T (cTnT)-positive patients were excluded. As the benchmark in PMI defined as cTnT >5 x the 99th percentile ULN, the independent predictors and major adverse cardiac events...

Efficacy and safety of serial atorvastatin load in Chinese patients undergoing elective percutaneous coronary intervention: results of the ISCAP (Intensive Statin Therapy for Chinese Patients with Coronary Artery Disease Undergoing Percutaneous Coronary Intervention) randomized controlled trial
Although several studies have suggested that intensive statin pretreatment could reduce the incidence of procedure-related myocardial infarction in western population, the data on the effect in Asian patients have been still limited. The aim of the study was to investigate the efficacy and safety of intensive atorvastatin load in Chinese patients undergoing elective PCI. A total of 1202 patients with stable angina or non-ST-segment elevation acute coronary syndrome (NSTE-ACS) scheduled to undergo PCI received either intensive statin treatment (80 mg atorvastatin daily x 2 days before PCI and 40 mg daily x 30 days after PCI...

Gender differences in lipid goal attainment among Chinese patients with coronary heart disease: insights from the DYSlipidemia International Study of China
Despite great advances in lipid-lowering therapy, the gender-based disparities in lipid goal success among Chinese coronary heart disease (CHD) patients receiving lipid-lowering treatment remained uncharacterized. Furthermore, little is known about potential causes for this gender imbalance. This cross-sectional trial included 25 697 patients treated with lipid-lowering agents from 122 centres between April 2012 and October 2012; data for 9420 participants with CHD were analysed in the present study; all underwent clinical examination and had their latest fasting lipid profiles while on lipid-lowering therapy recorded. Log...

QTc interval prolongation predicts the ablation outcome in hypertensive patients with paroxysmal atrial fibrillation
This study sought to determine the relationship between abnormal QTc interval and the recurrence after RFCA in hypertensive patients with paroxysmal AF. Two hundred and forty-two hypertensive patients with paroxysmal AF who underwent first time RFCA were retrospectively evaluated. Baseline data and QTc before the procedure were collected. Ninety-eight patients had AF recurrence after a mean follow-up of 17.0 ± 9.4 months. Those experiencing recurrence had a longer QTc interval than those without recurrence (429 ± 26 vs. 419 ± 23 ms, P = 0.004). Multivariate Cox regression analysis demonstrated the QTc ...

Optimal- vs. standard-antiplatelet therapy on platelet function and long-term clinical outcomes in patients with high on-treatment platelet reactivity: 2-year outcomes of the multicentre, randomized Optimal-antiPlatelet Therapy (OPT) trial
Optimal strategy for the management of high on-treatment platelet reactivity (HPR), an independent predictor of ischaemic events, is not yet clear. The aim of this study was to explore the safety and efficacy of personalized antiplatelet therapy based on platelet function testing in patients with HPR. A total of 840 patients with HPR who underwent coronary stenting for acute coronary syndrome (ACS) were randomly assigned in a 1:2 ratio to receive standard dual-antiplatelet therapy (n = 280) or platelet function-guided optimal-antiplatelet therapy (n = 560). In the optimal group, patients initially received clopidogrel 150 ...

Traditional Chinese medication Tongxinluo inhibits inflammatory angiogenesis via Bmx/NF-{kappa}B/MAPK pathways
The aim of this article is to examine the effect of the traditional Chinese medication Tongxinluo (TXL) on atherosclerotic plaque angiogenesis and stabilization and to explore the mechanism involved. Apolipoprotein E-deficient (apoE–/–) mice were fed a high-fat diet for 20 weeks, and then divided into four groups for 16 weeks' treatment: control (equal volume of buffer) and low-, medium-, and high-dose TXL (0.38, 0.75, and 1.5 g/kg/day, respectively). Vasa vasorum (VV) were identified by perfusion of biotinylated Lycopersicon esculentum (Tomato) lectin. The cellular effect of TXL on tumour necrosis factor &alph...

Chinese experts recommendation on the monitoring and management of variability in responsiveness to antiplatelet therapy
Antiplatelet agents are widely used in patients with coronary artery disease for the prevention of thrombotic events. However, individual responsiveness to antiplatelet agents is greatly varied, which is deemed as one of the most important risk factors for adverse clinic events, such as thrombosis and bleeding. Although studies on this aspect have lasted for more than a decade, the appropriateness of responsiveness variability monitoring and management for antiplatelet treatment remains controversial. Based on current clinical evidence, expert recommendations by a working group of Chinese Society of Cardiology of Chinese M...

The Notch pathway: a new therapeutic target in atherosclerosis?
The Notch signalling is a fundamental pathway involved in the modulation of cell proliferation, survival, and in stem cells maintenance. The crucial role played by Notch in tumour onset and progression has made it amenable to cancer therapy. Several clinical trials are currently testing safety and efficacy of combination treatments of existing cancer therapies with Notch inhibitors. Notch receptors and their ligands play a major role in the modulation of a plethora of biological functions of macrophages, endothelial, and vascular smooth muscle cells, particularly under inflammatory conditions. Data obtained by us and other...

The use of physical energy for tissue healing
Oscillatory patterns permeate the universe and are essential requisite in living cells. Circadian clocks exist at the subcellular and single-cell level. Chromatin is assembled into rhythmic oscillatory domains driving stem cell growth/differentiation up to higher hierarchical embryo development. The cytoskeleton is organized as rhythmically oscillating networks, producing radioelectric fields that may turn local events into non-local, long-ranging paths. For decades, scientists have used chemical tools to affect cell behaviour. However, this view is now deeply challenged. Consonant with a major role of physical forces in l...

Overview on diabetic foot: a dangerous, but still orphan, disease
Diabetic foot disease is a major health problem that concerns 15% of the 200 million patients with diabetes worldwide. More than 60% of non-traumatic amputations in the western world are performed in the diabetic population. Many patients who undergo an amputation have a history of ulceration. Treatment of foot complications is one of the main items of consumption of economic and health resources in diabetic patients. Over recent years, knowledge about the physiopathological pathways of this complication has increased significantly, together with improvements in diagnostic techniques and, above all, the establishment of a ...

An innovative minimally invasive technique for aortic valve replacement
Minimally invasive aortic valve replacement (MIAVR) by right mini-thoracotomy is used by some centres as an alternative to conventional full sternotomy. Whether this technique may replace full sternotomy and become the routine approach for aortic valve replacement remains an open question. The surgical technique, complication rates, learning curve, and patient outcomes collected during a 4-year experience by three centres were reviewed to answer this question. We retrospectively analysed the results obtained in 354 patients (212 males, 59.9%) who, between January 2010 and July 2014, underwent MIAVR through a right mini-tho...
