Association of major cardiovascular risk factors with the development of acute coronary syndrome in Lithuania
This study aimed to compare the prevalence of major CV risk factors in general population and acute coronary syndrome (ACS) patients and to determine their association with the development of ACS. Five hundred and twenty-three ACS patients and 645 age- and gender-matched control subjects were enrolled in the study. Smoking, dyslipidaemia, diabetes, and hypertension, but not overweight or obesity, were significantly more prevalent in the ACS patients, compared with control group. The prevalence of smoking, overweight or obesity, and dyslipidaemia were significantly higher in younger patients. Hypertension was highly prevale...

Perspectives: Coronary microvascular dysfunction in post-infarct remodelled myocardium
Myocardial infarction (MI) results in alterations in cardiac structure and function that not only impact the infarcted area, but also result in changes in the remote, remodelled myocardium. These changes also encompass structural changes in the coronary vascular tree and an increase in extravascular compressive forces acting on the microcirculation, thereby blunting myocardial flow reserve. Moreover, regulation of microvascular tone in the remote coronary vasculature is altered after MI. The alterations in regulation of microvascular tone appear to be principally the result of a loss of NO bioavailability in conjunction wi...

Clinical profile of patients with no-reperfusion therapy in Bosnia and Herzegovina and Serbia
The objective of this study was to evaluate the clinical profile and mortality of patients who were hospitalized with a diagnosis of STEMI, but did not receive reperfusion therapy in Bosnia and Herzegovina and Serbia. This was an observational study using registry data from the International Survey of Acute Coronary Syndromes in Transitional Countries (ISACS-TC; ClinicalTrials.gov, NCT01218776) on 633 STEMI patients admitted to 14 hospitals in Bosnia and Herzegovina (both Republic Srpska and Federation of Bosnia and Herzegovina) and Serbia from October 2012 to September 2013. Of these, 61 (9.6%) received fibrinolytic thera...

Management of heart failure complicating acute coronary syndromes in Montenegro and Serbia
It is essential that context-appropriate health research and health interventions take place in countries with economy in transition. The aims of this study were to describe the clinical characteristics, management, and in-hospital outcomes of acute coronary syndrome (ACS) patients with heart failure (HF) in Montenegro and Serbia. The data of this study are a framework of the International Survey of Acute Coronary Syndromes in Transitional Countries (ISACS-TC; NCT01218776), a multi-national and multicentre registry of patients hospitalized with ACS in the European countries that emerged from the Socialist era. The present ...

Gender differences in case fatality rates of acute myocardial infarction in Serbia
We sought to investigate sex differences in clinical characteristics, treatment and in in-hospital mortality in patients admitted with ST elevation myocardial infarction (STEMI) in Serbia. From October 2010 to September 2013, a total of 2348 patients have been hospitalized and received medical treatment for STEMI in 19 hospitals, referring data to the International Survey of Acute Coronary Syndromes in Transitional Countries (ISACS-TC) registry. There were 773 women and 1575 men. Women were older than man, with a higher prevalence of family history of coronary artery disease, hypertension, and diabetes. They were less like...

Perspectives: How to evaluate healthcare systems in primary angioplasty
Primary percutaneous coronary intervention (P-PCI) is the gold-standard treatment for ST-elevation myocardial infarction (STEMI). The main goals of a healthcare system in the setting of STEMI are two: first of all to reduce the time delays between diagnosis and P-PCI; secondly to increase the per cent of STEMI patients treated with P-PCI. The aim of this article was to propose a quality evaluation of healthcare system, identifying and defining measurable elements in the setting of P-PCI. The prevalence and outcomes of transradial percutaneous coronary intervention for acute coronary syndrome. Analysis from the single-centre ISACS-TC Registry (International Survey of Acute Coronary Syndrome in Transitional Countries) (2010-12)
The aims were to compare the prevalence and short-term outcomes of transradial (TRA) percutaneous coronary intervention (PCI) for acute coronary syndrome (ACS) in dedicated radial centre. This was a single-centre observational study of 3484 consecutively enrolled patients with ACS in a 3-year period (2010–12). There were 1648 patients with ST-elevation myocardial infarction (STEMI) and 1836 with non-ST-elevation ACS (NSTEACS). All patients underwent diagnostic coronary angiography within 24 h (STEMI) or within 72 h (NSTEACS). Percutaneous coronary intervention was performed in 84.9% patients (96.6% with STEMI and 74....

Perspectives: The Ca2+-dependent K+-channel KCa3.1 as a therapeutic target in cardiovascular disease
The endothelium is an interesting target to modify cardiovascular disease. In addition to the well-known autacoids such as nitric oxide and prostaglandins, a third endothelial pathway exists which relaxes vascular smooth muscle through hyperpolarization (endothelium-dependent hyperpolarization-type dilation). Herein, calcium-activated potassium channels (KCa) are crucial in which two types are expressed in endothelial cells (KCa3.1, KCa2.3). Specifically, KCa3.1 is selectively activated by small molecules that are potentially useful as pharmacological compounds which lead to overall vascular dilation including the coronary...

Hypertension and acute coronary syndromes in Romania: data from the ISACS-TC registry
There is little information on the incidence and prognostic significance of arterial hypertension (HTN) in acute coronary syndromes (ACSs), especially in the east European countries. We sought to investigate a registry of ACS patients in Romania, in order to better elucidate whether hypertensive patients are at higher risk of death and deserve a tailored approach for management and follow-up. The data of this study are a framework of the International Survey of Acute Coronary Syndromes in Transitional Countries (ISACS-TC) (ClinicalTrials.gov, NCT01218776). The present analysis focused on 2286 retrospective patients admitte...

Clinical characteristics, hospital care, and prognosis of patients with ST elevation myocardial infarction: Hungarian Myocardial Infarction Registry
The authors analyse clinical data, hospital care, and short- and long-term prognosis of 4981 ST segment elevation myocardial infarction (STEMI) patients enrolled in the web-based Hungarian Myocardial Infarction Registry. It has been found that the mean age of STEMI patients was lower by 5.3 years than that of patients treated for non-STEMI (NSTEMI). In the group of NSTEMI patients (n = 3601), the occurrence of diabetes, hypertension, peripheral vascular disease, and previous history of myocardial infarction and stroke were significantly more frequent. The proportion of smokers among STEMI patients was remarkably higher. Pr...

Perspectives: The burden of cardiovascular risk factors and coronary heart disease in Europe and worldwide
Cardiovascular disease (CVD) is estimated to be the leading cause of death and disability-adjusted life years lost worldwide being coronary heart disease the most prevalent form of heart disease. The underlying cause of most CVD is atherosclerosis, a process mainly governed by lifestyle factors. Several epidemiological studies have enabled the recognition of the major risk factors for CVD development, ‘the so-called conventional risk factors’. Identification of such global risk factors have led, in turn, to the development of risk-prediction algorithms and cardiovascular risk models for men and women. Indeed, d...
