Perspectives: Rationale and design of the ISACS-TC (International Survey of Acute Coronary Syndromes in Transitional Countries) project
During the past 10 years, the health of people in Eastern Europe and the former Soviet Union has undergone changes very different from the health patterns seen in their Western counterparts. Mortality from cardiovascular disease has been decreasing continuously in the USA and many Western European countries, but it has increased or remained unchanged in many of the states of Eastern Europe. Analysis of this phenomenon has been hindered by insufficient information. The International Registry of Acute Coronary Syndromes registry study in Transitional Countries (ISACS-TC) is both a retrospective—over a 1-year period&mda...

Potential cardiovascular effects of dipeptidyl peptidase-4 inhibitors in patients with type 2 diabetes: current evidence and ongoing trials
Cardiovascular disease (CVD) is a major cause of morbidity and mortality in patients with type 2 diabetes mellitus (T2DM). Cardiologists, who often treat patients with CVD and T2DM, are faced with the unmet need for an agent that provides glycaemic control yet does not pose CV risk. No antidiabetic therapy is currently indicated to improve macrovascular outcomes. Results of studies assessing the association between intensive antidiabetic therapy and a reduction in the risk of major CV events in patients with T2DM have been inconsistent, and independent reports have linked certain T2DM therapies (e.g. rosiglitazone, sulphon...

Beyond glucose: cardiovascular effects of incretins and dipeptidyl peptidase-4 substrates
The incretins are naturally occurring peptides that are pro-glucagon derivatives that have been implicated in the control of appetite and satiety. The class includes both glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). Incretins, particularly GLP-1 (7–36) amide, have been exploited clinically by virtue of their insulinotropic properties that are glucose dependent, thus mitigating the risk of hypoglycaemia that accompanies conventional secretagogues and exogenous insulins. However, the short circulating half-life of the native peptide limits their use as a therapeutic option. Di...

Cardiovascular disease and diabetes: the vulnerable patient
Patients with diabetes exhibit an increased risk for cardiovascular complications, such as acute coronary syndromes, stroke, heart failure, or arrhythmias, as diabetes facilitates changes in the cardiovascular system of these patients. Atherosclerotic lesions have long been recognized as a cause for these events. However, recent findings support the idea that alterations in the circulating blood and the myocardium may be equally or even more important contributors to the development of life-threatening cardiovascular events in patients with diabetes. These alterations in the myocardium, vasculature, and blood characterize ...

Heart failure in Asia: the present reality and future challenges
Heart failure (HF) is the final pathway of multiple cardiac diseases. It exerts its clinical burden globally. In this brief review, its epidemiology in Asia is described, followed by an overview of the evolution of past therapies to present strategies available in Asia, and finally, future challenges are identified. Transcatheter therapies for congenital heart disease in Taiwan
Transcatheter therapies have been widely applied in the treatment of congenital heart disease in Taiwan since 1985. Balloon dilation was routinely performed in the treatment of congenital as well as post operative stenotic lesions. Stenting of branch pulmonary arteries was performed in patients who did not achieve successful balloon dilation. We applied a strategy to close patent ductus that is using Gianturco coil for ductus >2.5 mm and Amplatzer duct occluder for ductus <2.5 mm. Amplatzer septal occluder is used to close secundum type atrial septal defect with very rare complications. Balloon sizing is not routinel...

Congenital heart disease in Japan
As same as other developed countries, prevalence of ACHD in Japan has already dramatically increased. The establishment of the special facilities and resources for ACHD patients is crucial, also the establishment of education and training systems for adult cardiologists in this evolving field has been discussed in Japan. In the field of catheter intervention, the Society of Japanese Pediatric Interventional Cardiology (JPIC) contribute all area of interventional procedures, all procedures for congenital heart disease and procedure related complications have been registered in the society. Brief communication: current and future challenges in electrophysiology in the Asia-Pacific region
Seminal developments in clinical electrophysiology have expanded the range of therapeutic options available to manage patients with cardiac arrhythmias, including in the Asia-Pacific region. This review examines some of these developments and looks at the future challenges confronting the field. (Source: European Journal of Heart Failure Supplements)

Why should cardiologist care about peripheral arterial disease in their cardiac patients?
Peripheral arterial disease (PAD) is becoming an increasingly important health issue and is associated with significant cardiovascular morbidity and mortality. There are many reasons why cardiologists should care about PAD in their cardiac patients but unfortunately, cardiologists tend to focus upon atherosclerosis of the coronary arteries and PAD is inadequately addressed in most cardiology training programmes. To optimize outcomes of patients with global atherosclerotic disease, the modern cardiologist should have a good foundation of knowledge about PAD. The overall effect of this paradigm shift would reduce the inciden...

Clopidogrel pharmacogenetics of east, south and other Asian populations
The pharmacogenetics of clopidogrel metabolism show striking interethnic diversity. Apart from clear differences between white and Asian populations, unique differences in the genetic control of clopidogrelbioactivation exist between major Asian ethnicities. More studies are clearly needed to help us understand the clinical impact of these genetic differences and their environmental interaction in ethnically and genetically diverse Asia. Challenges in the management of hypertension in Asia
Hypertension is common in Asian populations and is a major cause of cardiovascular diseases. Even a small reduction in blood pressure can lead to a substantial decrease in the risk of stroke. The prevalence of hypertension appears to be increasing in many countries, partly because of ageing of the population, but also because of increasing obesity. While much effort has gone into promoting the awareness of hypertension, the rates of detection, treatment, and control remain low. As hypertension is very prevalent in the general population, it makes sense to adopt population approaches towards the detection and treatment of h...

P122 * Relationship between retinal microvascular complications in metabolic syndrome (MS) and its association with serum uric acid level
Conclusion: It is concluded that there is a direct association of MS components with retinal microvascular complications. There was synergistic effect of MS on retinal microvascular changes beyond its individual components. Incidence of retinopathy was higher in patients with hyperuricemia than normouricemic patients.
