Ureteric Suture Urolithiasis Following Open Emergency Ureteric Repair
We report a case in a 30-year-old man who sustained a gunshot injury to the left ureter in Burundi and underwent open ureteric repair in a regional hospital. He migrated to the UK and presented one year later with left loin pain. An intravenous urogram revealed two foci of calcification to the left of L3, within a peri-ureteric position. The patient underwent left-sided ureteroscopy where two calculi each formed around a nylon suture were discovered within a ureteric pseudo-diverticulum, and basketed. This is the first reported case of suture urolithiasis occurring following ureteric surgery.Curr Urol 2015;9:44-46 (Source: Current Urology)

Clinical Effect of Switching from a Luteinizing Hormone-Releasing Hormone Agonist to an Antagonist in Patients with Castration-Resistant Prostate Cancer and Serum Testosterone Level ≥ 20 ng/dl
Conclusion: Switching to a luteinizing hormone-releasing hormone antagonist in patients with testosterone levels ≥ 20 ng/dl may be an option in sequential androgen deprivation therapy for some patients.Curr Urol 2015;9:31-35 (Source: Current Urology)

Urinary Retention: Catheter Drainage Bag or Catheter Valve?
The management of patients with intractable urinary incontinence presents as a challenging priority in the ageing population. To preserve the antibacterial mechanisms of the bladder, a urine collection that enables the bladder to fill and empty regularly and completely, should be used. This mimics the action of the healthy bladder. We compared the success rates of two widely used urinary-collection systems (catheter with drainage bag or a catheter valve) at our institution for those patients undergoing a trial of void.Curr Urol 2015;9:28-30 (Source: Current Urology)

Can the Free/Total PSA Ratio Predict the Gleason Score Before Prostate Biopsy?
Conclusion: According to our study, there is a relationship between higher Gleason score and decreased f/t PSA ratio. Therefore, f/t PSA can be an indicator for predicting the Gleason score.Curr Urol 2015;9:24-27 (Source: Current Urology)

Impact of Robotic Fellowship Experience on Perioperative Outcomes of Robotic-Assisted Laparoscopic Partial Nephrectomy
Conclusion: Fellowship experience may allow for treating more challenging and posterior tumors in initial practice and significantly more comfort performing retroperitoneal robotic-assisted laparoscopic partial nephrectomy.Curr Urol 2015;9:19-23 (Source: Current Urology)

A Comparative in Vitro Study of Power Output Deterioration over Time Between Ho:YAG Laser Fibers from Different Manufacturers as a Function of Deflection and Power Input
Conclusion: Fiber fracture and decline in performance is not due to deflection and continuous use. Frictional forces that occur between the fiber tip and the stone fragments may be responsible.Curr Urol 2015;9:12-18 (Source: Current Urology)

Post-Operative Pain Management in Patients Undergoing Robotic Urological Surgery
We present a review on multiple analgesic techniques available to prevent and treat pain specifically caused after by urological robotic surgery; this article will explain the mechanism of pain pathways involved in laparoscopic procedures and review current evidence pertaining to systemic and regional analgesia methods.Curr Urol 2015;9:5-11 (Source: Current Urology)

Effect of Pneumoperitoneum on Renal Function and Physiology in Patients Undergoing Robotic Renal Surgery
Laparoscopic and minimally-invasive robotic access has transformed the delivery of urological surgery. While associated with numerous desirable outcomes including shorter post-operative stay and faster return to preoperative function, these techniques have also been associated with increased morbidity such as reduced renal blood flow and post-operative renal dysfunction. The mechanisms leading to these renal effects complex and multifactorial, and have not been fully elucidated. However they are likely to include direct effects from raised intra-abdominal pressure, and indirect effects secondary to carbon dioxide absorptio...

Evaluation of Thymidylate Synthase Expression in Prostate Cancer
In this study, we investigated expression of TS in prostate cancer and its prognostic significance. Seventy-five prostatic tissue specimens were obtained from patients who had undergone prostate biopsy for diagnosis of prostate cancer. We analyzed the cancerous tissue specimens for TS expression using immunohistochemistry. TS expression was significantly increased in patients with bone metastasis. No relationship was found between expression of TS and the other clinicopathological findings. Because TS expression could be used as a prognostic parameter in patients with prostate cancer, an accurate prediction of prognosis mi...

Low Oxalobacter Formigenes Colonization is Associated with Reduced Bone Mineral Density in Urinary Stone Forming Patients
Introduction: Lower bone mineral density (BMD) and reduced Oxalobacter formigenes colonization are common findings in urolithiasis patients. But none of the studies conducted investigated the relationship between decreased bone mineral density and reduced Oxalobacter colonization. Here we evaluated the relation between BMD and O. formigenes colonization in urolithiasis patients. Materials and Methods: 50 stone formers (48.9 ± 11.9 years) and 50 control (47.2 ± 13.4 years) adult male subjects were included in the study. Alterations in O. formigenes colonization were determined as absolute O. formigenes count from fecal sa...

Robotic Partial Cystectomy for Primary Urachal Adenocarcinoma of the Urinary Bladder
We present the largest reported consecutive series on robotic partial cystectomy in the management of patients with primary urachal adenocarcinoma. Eight patients with primary urachal adenocarcinoma of the urinary bladder underwent a robotic partial cystectomy. The mean operative time, including trocar placement as well as robotic docking and closure was 184 minutes (range 130-240 minutes). The mean console time was 120 minutes (range 70-170 minutes). The mean estimated blood loss was 50 ml. There were no conversions to open surgery. The mean hospital stay was 4 days (range 3-7 days). Drain removal was performed at postope...

Is a Reassuring MAG-3 Diuretic Renal Scan Really Reassuring?
Conclusion: In pediatric patients with persistent antenatally detected hydronephrosis, a reassuring MAG-3 can allow for safe observation in the vast majority. Only one of 24 renal units with follow-up progressed to obstruction. All patients treated without surgery had complete resolution of their hydronephrosis. Observation with serial ultrasounds may be an appropriate alternative to operative management, even in patients with moderate to severe hydronephrosis in the presence of a reassuring or indeterminate MAG-3 diuretic scan.Curr Urol 2014;8:178-182 (Source: Current Urology)

Risk Factors and Significance of Gentamicin Resistant Positive Urine Culture in Patients Undergoing Urological Surgery: A Short Report
We assessed patients who had pre-operative urine that grew gentamicin-resistant bacteria but were given gentamicin prophylaxis because urine result was not available. Our aim was to identify postoperative-sepsis rates, risk factors to acquire resistant-bacteria, and to optimize our prophylactic regime. Total 4,933 pre-operative urine-samples were reviewed and those positive for E.coli, Klebsiella or Proteus (n = 979) were analysed. Forty-four (4.4%) had gentamicin-resistant bacteria. Of those, 8 were immunosuppressed, 38 (86%) had a recent urological procedure and 29 (66%) had received recent antibiotics. Eighteen (41%) ha...

Intravesical Therapies for Radiation Cystitis
Conclusion: There is a paucity of data in recent literature regarding the management of radiation cystitis, at least by means of intravesical agents. The replenishment of the glycosaminoglycan layer of the bladder mucosa and the administration of botulinum toxin show promise, but need further investigation.Curr Urol 2014;8:169-174 (Source: Current Urology)
