Electronic Medical Record Reporting Enhances Proactive Psychiatric Consultation
Numerous studies have demonstrated that psychiatric and substance use issues in general hospital inpatients result in increased length of stay and associated costs. Additional studies have demonstrated that proactive consultation models in psychiatry can effectively address these problems. Selecting patients for proactive interventions is less well studied. Progressive encephalomyelitis with rigidity and myoclonus syndrome presenting as catatonia
In this case report, we discuss a patient who was initially diagnosed with catatonia, and was later found to have GAD-65 associated progressive encephalomyelitis with rigidity and myoclonus (PERM) syndrome, which is a variant of an autoimmune condition known as stiff person syndrome. Her catatonia was initially treated with benzodiazepines and ECT, and later in her hospitalization course was treated with steroids, IVIG, and rituximab. Fibroids and Antipsychotics
Leiomyoma can lead to significant morbidity in reproductive-aged women, causing symptoms that range from bleeding and pain to infertility. Despite recent progress made regarding tumor mitogens and nonsurgical treatments, the exact pathogenesis of fibroids remains unknown. Prolactin has been implicated as a mitogen growth factor in leiomyoma. Antipsychotic medications, in lowering dopamine levels and thereby removing the inhibition of prolactin secretion, can increase serum prolactin levels. Given this relationship, it stands to reason that antipsychotics may have a deleterious effect on myometrial smooth muscle cells. (Source: Psychosomatics)
