Serial changes in delayed focal hippocampal lesions in patients with transient global amnesia.
Authors: Ueno H, Naka H, Ohshita T, Wakabayashi S, Matsumoto M Abstract The etiology of transient global amnesia (TGA) is not well understood. MR studies, including studies using diffusion-weighted imaging (DWI), have been used to investigate the pathophysiology of TGA, and focal hippocampal lesions have been detected in some studies. The aim of this study was to investigate serial changes in MR images from the patients with TGA. In seven TGA patients, serial MRI scans (from the same day of the onset to several days after the onset of symptoms) using a 1.5-T MR unit were prospectively evaluated. In four pa...

Norovirus gastroenteritis accompanied by ischemic colitis: a case report.
Authors: Zenda T, Kaneko S, Noriki S Abstract A 57-year-old woman was admitted to our hospital because of frequent bloody diarrhea, vomiting, and abdominal cramps. While the patient was diagnosed with norovirus gastroenteritis based on the presence by chance of positive norovirus antigen in the stool samples, endoscopic as well as pathological examinations demonstrated left-sided ischemic colitis. The patient soon recovered solely by supportive treatment. Although it is believed that patients with viral gastroenteritis do not develop bloody diarrhea, the present case suggested that conditions of viral gast...

Trans-differentiation of a duodenal phenotype on duodenal transplantation of different normal tissues in F344 rats.
Authors: Watanabe H, Kominami Y, Gon R, Hayashi M, Nishiki M, Sasaki A, Shiraishi M, Ueda S, Ochiya T, Kamiya K Abstract The mechanisms regulating stem cell differentiation and self-renewal are largely unknown. Our ultimate goal is to be able to regulate somatic stem cell differentiation and proliferation. In the present study the ability of trans-differentiation was studied when different normal tissue types were transplanted into the duodenum in rats. Pieces of ear (skin), bladder, trachea, diaphragm, pyloric gland, and forestomach from 8-week old GFP-F344 rats were transplanted into the duodenum of F344...

Regression of a primary pulmonary adenocarcinoma after zoledronic acid monotherapy.
Authors: Nagao S, Hattori N, Fujitaka K, Iwamoto H, Ohshimo S, Kanehara M, Ishikawa N, Haruta Y, Murai H, Kohno N Abstract Bisphosphonates are widely used for the treatment of metastatic skeletal tumors and hypercalcemia resulting from malignant tumors. Zoledronic acid (ZOL), a third-generation bisphosphonate agent, was recently demonstrated to show synergistic antitumor activity of ZOL when combined with chemotherapy in lung cancer patients. However, whether ZOL exerts direct antitumor activity on lung cancer remains unclear. Here, we report an atypical case encountered while treating a 57-year-old woman ...

Portal-systemic shunt between the inferior mesenteric vein and inferior vena cava in a patient with hepatic encephalopathy: successful occlusion by balloon-occluded retrograde transvenous obliteration.
We report a patient with hepatic encephalopathy due to a large IMV-IVC shunt who was successfully treated by balloon-occluded retrograde transvenous obliteration. The procedure involved a combination of 11 metallic coils and 5 ml of 5% ethanolamine oleate with iopamidol as the sclerosing agent. After complete obliteration of the shunt, his symptoms disappeared. At 2-years follow-up he was free of clinical symptoms, the size of his liver had slightly increased, and his liver function was preserved. Advanced esophageal cancer with situs inversus totalis successfully treated with chemoradiotherapy followed by esophagectomy: case report.
We report a case of advanced esophageal cancer successfully treated with neoadjuvant chemoradiotherapy followed by esophagectomy in a 53-year-old man with situs inversus totalis. Upper gastrointestinal endoscopy in a clinical examination revealed a tumor in the lower third of the esophagus, and moderately differentiated squamous cell carcinoma was diagnosed from the biopsy findings. He was referred to us and the disease was diagnosed as esophageal cancer (clinical T3N1M0, cStage III) after further evaluation. According to the therapeutic strategy of our department, neoadjuvant chemoradiotherapy was commenced. The regimen w...

Control of dry weight and tube feeding improved the general condition of a hemodialysis patient: report of a case.
Authors: Nakatani H, Shoji H, Shibata M, Yoshida M, Tadokoro Y, Ogura N, Inoue Y, Hanazaki K, Hamada S, Kawamura A Abstract Patients on long-term hemodialysis are at risk of developing malnutrition, and poor nutrient intake is an important factor in this. In the present case, we encountered a 55-year-old Japanese woman with end-stage renal failure and a past history of schizophrenia. Severe systemic edema was observed. Hemodialysis was started, but after one year she suddenly became unable to consume food orally, despite provision of a dietary plan by the nutrition support team (NST). Tube feeding was event...

Norovirus gastroenteritis accompanied by marked elevation of transaminases.
Authors: Zenda T, Miyamoto M, Kaneko S Abstract A 56-year-old woman was admitted because of frequent watery diarrhea, vomiting, and abdominal cramps. An examination of the stool for norovirus antigen was positive, and a blood examination revealed a marked elevation of liver enzymes. Liver dysfunction, as well as symptoms related to gastroenteritis, was ameliorated solely by supportive treatment. Although liver injury concurrent with norovirus gastroenteritis is rarely documented and its pathogenesis remains unknown, clinicians should consider liver injury as one of the possible extra-intestinal manifestati...

Six-year experience of permanent prostate brachytherapy for clinically localized prostate cancer.
This study evaluated the efficacy of PPB in these patients. The median patient age was 69 years (range 53 to 82 years), the median prostate-specific antigen (PSA) value before biopsy was 6.75 ng/ml (range 3.5 to 47.9 ng/ml), and the median prostate volume was 23.1 ml (range 10.1 to 57 ml). The median follow-up was 37 months (range 1 to 72 months). The serum PSA levels decreased continuously after PPB throughout the entire follow-up period in 97% of patients without neoadjuvant hormonal therapy. No relapse occurred during the follow-up period in patients at low risk. Our 6-year experience suggests that PPB is effective for ...
