Association between apparent temperature and acute coronary syndrome admission in Rasht, Iran
Conclusions The high apparent temperature is correlated with a higher ACS admission especially on the same day. These findings may have implications for developing intervention strategies to reduce and prevent temperature-related morbidity especially in the elderly. (Source: Heart Asia)

Economic burden of hospitalisation for congestive heart failure among adults in the Philippines
Conclusions The calculated healthcare-related hospitalisation cost for CHF in the Philippines in 2014 demonstrates the disparity between the actual cost and PhilHealth’s coverage. This implies a need for policymakers to review its coverage to make healthcare delivery affordable. (Source: Heart Asia)

Epidemiology of in-hospital cardiac arrest in Abu Dhabi, United Arab Emirates, 2013-2015
Conclusions The incidence of IHCA was higher and its outcomes were lower compared with other high-income/developed countries. Survival outcomes were better for patients who were younger and had a shockable rhythm, and similar between time of day and days of the week. These findings may help to inform health managers about the magnitude and quality of IHCA care in the UAE. (Source: Heart Asia)

Percutaneous closure of iatrogenic arteriovenous fistula after pacemaker implantation
A 76-year-old man presented with swelling of the left upper limb that started 1 year after pacemaker implantation. During the procedure, a single-pass VDD lead was introduced through an axillary vein puncture and was positioned in the right ventricular apex. Pitting oedema of the left upper limb was seen, with reddish-brown skin pigmentation, scaling and thickening (figure 1A). Subclavian angiography showed a connection between the left axillary artery and vein (figure 1B). A self-expanding covered stent was deployed across the fistula. Postprocedure angiogram showed minimal residual shunt (figure 2A, online supplementary ...

Barriers and facilitators to treatment among patients with newly diagnosed hypertension in Nepal
Conclusions A number of factors emerged as barriers and facilitators to hypertension treatment. This information can be useful in designing appropriate health interventions to improve hypertension management. (Source: Heart Asia)

ST segment elevation myocardial infarction with normal coronary arteries
Case presentation A middle-aged patient presented to the emergency department with intermittent chest pain of 4-hour duration. The patient had been recently diagnosed with metastatic adenocarcinoma of the colon and was receiving 5-fluorouracil (5-FU)-based chemotherapy at the time of presentation. The ECG at presentation showed 1 mm ST segment elevation in leads II, III and aVF, with reciprocal changes in leads aVL, V1 and V2 (figure 1A). Serum cardiac troponin I level was elevated at 0.11 ng/mL (normal: 0.00–0.02 ng/mL). The patient was given sublingual nitrate and loading doses of aspirin, clopidogrel and atorvasta...

Audit of defibrillators at an urban public sector hospital
Conclusion Compliance with regard to the availability of defibrillator checklist books, conducting and recording of daily defibrillator checks, timely service maintenance of defibrillators and identification of critical device problems was suboptimal in this study. There is a need for ongoing training of hospital staff as well as the establishment of systems to prevent potential adverse consequences due to device failure. (Source: Heart Asia)

Incidence and predictors of 30-day mortality in patients with ventricular septal rupture complicating acute myocardial infarction
Conclusions In this retrospective cohort of patients with AMI, the occurrence of VSR was not rare and carried a considerable risk of 30-day mortality. Advanced age, previous cerebrovascular accident and surgical repair influenced the risk for 30-day mortality. (Source: Heart Asia)

Cystic mediastinal mass
Case presentation A 32-year-old woman with no other medical history presented with 1-month history of fever, weight loss and dyspnoea. On examination she had elevated jugular venous pressure and tachycardia. Her chest X-ray posterioranterior view (figure 1A) showed a rounded mass in the right cardiophrenic angle obscuring the right atrial margin, producing a ‘silhouette’ sign. Echocardiography showed a large cystic mass with thickened pericardium, lateral to the right atrium, causing right atrial compression (figure 1B). CT image of the chest showed a cystic lesion compressing the right atrium with thickened pe...

Clinical implication of LAVI over A' ratio in patients with acute coronary syndrome
Conclusion LAVI/A' was related to plasma BNP levels in patients with ACS, particularly in those with NSTE-ACS. This index was useful for predicting cardiac events in patients with ACS. (Source: Heart Asia)

Inverse association of ApoB and HSP60 antibodies with coronary artery disease in Indian population
Conclusion This study shows that higher antibodies to ApoB and HSP60 proteins are less often associated with ACS and that these antibodies are inversely associated with inflammatory Th17 cells. (Source: Heart Asia)

Influence of comorbidities and clinical prediction model on neurological prognostication post out-of-hospital cardiac arrest
Background Survival with good neurological function post out-of-hospital cardiac arrest (OHCA), defined as cerebral performance category (CPC) 1–2, ranges from 1.6% to 3% in Asia. We aim to study the influence of comorbidities and peri-OHCA event factors on neurological recovery and develop a model that can help clinicians predict neurological function among patients with post-OHCA admitted to the hospital. Methods This was a retrospective cohort study. All patients admitted post-OHCA from 1 January 2011 to 31 December 2015 to a tertiary centre were identified through the hospital OHCA registry. Patients w...

Extended trans-septal versus left atrial approach in mitral valve surgery: 1017 patients experience
Conclusion In our opinion, EVTSA to the mitral valve is safe and reproducible. It gives excellent exposure of the mitral valve under all circumstances without any significant increase in cross-clamp or bypass time, postoperative arrhythmia, heart block/pacemaker rate or bleeding. Vasculitis of ascending aorta detected on FDG PET/CT in a patient with fever of unknown origin
A 59-year-old man was admitted for fever of unknown origin. Fever was associated with chills. His medical record revealed the history of aortic valve replacement 11 years earlier, as well as enterococcal endocarditis 4 months prior to the current admission. The patient was found to have normal left ventricular size with borderline systolic function, left ventricular hypertrophy, right ventricle at the upper limit of normal size, mild systolic dysfunction and mild transvalvular aortic insufficiency on transthoracic echocardiography; while no vegetations were observed on transesophageal echocardiography. Sequential blood cul...

A challenging broad-complex tachycardia
A 53-year-old man presented with chest pain, palpitations and presyncope, without history of overt cardiac disease. The patient was alert. His heart rate was 206 beats per minute, and his blood pressure was 100/50 mm Hg. An intravenous bolus of amiodarone 150 mg was administered in the emergency department. His ECGs preamiodarone and postamiodarone are shown in figure 1. Echocardiography showed low-normal left ventricular systolic function. Figure 1 (A) ECG of index arrhythmia. (B) ECG following amiodarone. Question What should the next diagnostic test be? Referral for electrophysiology study. Referral ...
