Risk perception of cardiovascular diseases among individuals with hypertension in rural Malaysia
Conclusions Our study shows that respondents who were not on medications, unaware of their hypertension status and those who had uncontrolled hypertension tended to underestimate (lower CvRPS) their risk for CVD. Improving their CvPRS through a concerted health education may lead to better therapeutic behaviour and outcomes. (Source: Heart Asia)

An evaluation of the 25 by 25 goal for premature cardiovascular disease mortality in Taiwan: an age-period-cohort analysis, population attributable fraction and national population-based study
Conclusions Cigarette smoking is the primary target in the prevention of cardiovascular disease. Through the stringent control of smoking, the goal of a 25% reduction in premature mortality from cardiovascular disease may be achieved before 2025 in Taiwan. (Source: Heart Asia)

A dairy worker with fever and an abnormal echocardiogram
Description of the case A 38-year-old male presented with history of progressively increasing dyspnoea of 25 days duration. He gave history of low -grade fever associated with malaise and weight loss over the preceding 6 months. He worked in the dairy industry in the Middle East and returned to India owing to his illness. On clinical examination, he was found to be tachypneic and cachectic. Jugular venous pressure was raised with a prominent ‘a’ wave. There was a short early diastolic murmur over the aortic area. His blood investigations, including renal and liver function tests, were normal. Three se...

Cardiovascular risk in an HIV-infected population in India
Conclusion CVD at younger ages among Asian Indian people living with HIV appear to be an imminent risk for morbidity. Stepping up of preventive services including screening services and prescription of statins are important strategies that must be considered. (Source: Heart Asia)

Implications of prescribing a fixed-dose combination in clinical cardiology practice: a retrospective observational study using a single medical centre database in Korea
Conclusions FDC discontinuation was common among patients attending the cardiology outpatient clinic. Our analyses suggest that FDC discontinuation in patients at high ASCVD risk may have an impact on CV event rates. (Source: Heart Asia)

Outcomes after mitral valve surgery for rheumatic heart disease
Conclusion In this large prospective cohort study we have demonstrated that adjusted long-term survival following RHD mitral valve repair surgery in Australia is no different to replacement and no different to non-RHD. Interpretation of valve surgery outcome requires careful consideration of patient factors that may also influence survival. (Source: Heart Asia)

Spectrum of cardiovascular diseases in six main referral hospitals of Ethiopia
Conclusion Our study shows that chronic rheumatic valvular heart disease is the most common cardiovascular diagnosis among patients seen at cardiology clinics of six referral/teaching hospitals in the country, followed by congenital heart diseases. Hypertensive and ischaemic heart diseases also accounted for a significant proportion of the cases. Therefore, strategies directed towards primary and secondary prevention of acute rheumatic fever as well as prevention of risk factors for hypertension and ischaemic heart disease may need to be strengthened. Development of local clinical practice guidelines in the real world: an evolving scene in China
Introduction Clinical practice guidelines play a vital and indispensable role in every healthcare system. In China, the comprehensive reform of the healthcare system was initiated in 2009 with the ultimate goal of universal health coverage.1 The approach to boost government funding to subsidise health expenditures in the form of government health insurance schemes inevitably led to higher demands for healthcare services and a sharp rise in health expenditures.2–4 This dramatic transformation of healthcare delivery and utilisation highlights the important consideration of ensuring that the health system in China is ab...

Non-compaction of left ventricular myocardium in sub-Saharan African adults
Conclusion Contrary to previous beliefs, NCLV may not be less common in black Africans than in other ethnic subgroups. Clinicians in Africa should be made aware of NCLV so that it can be diagnosed at earlier stages. (Source: Heart Asia)

Pregnancy-related conditions and premature coronary heart disease in adult offspring
Conclusions Offspring of mothers with complications during pregnancy have an increased risk for premature ACS in adulthood, which may be of particular relevance in populations in transition, where the incidence of both pregnancy-related morbidity and CVD are high. (Source: Heart Asia)

Rheumatic heart disease with a mitral blood cyst: three-dimensional echocardiographic imaging
A 22-year-old woman presented with a 4-month history of dyspnoea on exertion and palpitations. She was in New York Heart Association class II at presentation. She had no history of fever, joint pains or rash. On examination, she had a grade II pansystolic murmur at the apex and an early diastolic murmur at the aortic area. Laboratory investigations revealed anti-streptolysin O titre of <200 units with normal erythrocyte sedimentation rate and C reactive protein levels. On two-dimensional echocardiographic examination, the mitral valve showed mild leaflet thickening with a mobile globular cystic structure with ...

Challenges and opportunities in accessing trial data: an example from statin primary prevention trials
The objectives were to assess the effects, both harms and benefits, of statins in people with no history of CVD. Our results showed that statins reduced all-cause mortality (OR=0.86, 95% CI 0.79 to 0.94) and fatal and non-fatal cardiovascular events (RR=0.75, 95% CI 0.70 to 0.81) with no increased risk of adverse events (RR=1.00, 95% CI 0.97 to 1.03).1 Despite consistent findings of the benefits of statins for primary prevention of CVD, the validity of these results has been challenged by some authors.2 Observational studies have demonstrated associations between statin exposure and adverse outcomes, such as impaired cognit...
