Gender differences in the use of primary prevention ICDs in New Zealand patients with heart failure
Conclusion Perioperative complications were significantly more common in women referred for ICD/CRT-D. Although there has been a significant increase in ICD implantation rates, gender differences in the use of these devices still exist in New Zealand, in keeping with the demographics of ischaemic heart disease and systolic dysfunction between genders. Impact of regionalisation of a national rheumatic heart disease registry: the Ugandan experience
Objectives Rheumatic heart disease (RHD) remains a major driver of cardiovascular morbidity and mortality in low-resource settings. Registry-based care for RHD has been advocated as a powerful tool to improve clinical care and track quality metrics. Data collected through an RHD registry may also reveal epidemiological and geospatial trends, as well as insight into care utilisation. Uganda established a central RHD registry at the country’s only tertiary cardiac centre in 2010. In 2014 RHD care and registry enrolment expanded to the Western region and in 2015 to the North. Here, we examine the geographical distributi...

Cancer antigen-125 and risk of atrial fibrillation: a systematic review and meta-analysis
Conclusion Ca-125 was significantly higher in patients with AF than in those in sinus rhythm, and high Ca-125 is predictive of AF occurrence. However, the high heterogeneity observed means there is an uncertainty in the relationship between Ca-125 and AF, which needs to be confirmed by larger prospective studies. (Source: Heart Asia)

Rheumatic heart disease across the Western Pacific: not just a Pacific Island problem
Some of the highest recorded rheumatic heart disease (RHD) prevalence and mortality rates are from the World Health Organization’s Western Pacific Region (WPR). RHD burden has been well documented in much of the WPR subregion of Oceania, but less is known about RHD outside the Pacific Islands and Australasia. We aimed to review RHD burden in WPR outside Oceania to identify countries with high RHD burden and those with contemporary data gaps. We searched the peer-reviewed literature for English-language primary studies published between 1980 and April 2017 that reported RHD prevalence or mortality in the 13 WPR countr...

Iatrogenic radial arteriovenous fistula
A 62-year-old female underwent successful primary percutaneous coronary intervention for an acute anterior wall myocardial infarction via the right radial artery access. The patient presented 3 months later for a routine follow-up. There was a dilated superficial vein at the level of the distal right radial artery, with a palpable thrill at the access site (figure 1A). Subsequent Doppler ultrasound imaging demonstrated a radial arteriovenous fistula with a neck of 2 mm (figure 1B; see online supplementary video 1)supplementary video 1). The brachial angiography showed a right radial artery to venous fistula ...

'Caterpillar right ventricle: unusual manifestation of a rare disease
A 66-year-old man presented with abdominal distension and pedal oedema for the past 10 years. He had history of right heart failure on several occasions in the past, and one of these admissions prompted referral to a cardiac specialist. On examination, he had markedly elevated jugular venous pressure with prominent ‘y-descent’, a tricuspid regurgitation murmur, gross ascites and pedal oedema. A full blood count, routine biochemical screen and serum bicarbonate levels were normal. Right ventricular angiogram revealed a ‘caterpillar’-like aneurysm of the right ventricle (RV) apex (figure 1 arrow) with...

Proctors in interventional cardiology: go beyond mentoring and monitoring!
The last decade has seen exciting advances in interventional cardiology. These innovations have tremendous potential for offering attractive alternatives to the existing surgical treatments of some cardiac diseases and have also identified novel methods of percutaneous intervention for hitherto pharmacologically managed conditions. Many of these advances have been driven by the introduction of implantable devices, such as the percutaneous aortic valve and left atrial appendage closure device. Introduction of a new technology to physicians as well as adequately training the interventionist in the science and technique of in...

Renal artery revascularisation: can we predict who benefits?
Atherosclerotic renal artery stenosis (ARAS) is commonly seen in individuals with vascular disease in other territories, that is, coronary and cerebrovascular disease.1 Epidemiological data suggest that ARAS is present in ~7% of patients over the age of 65 years and in more than half of the patients with evidence of atherosclerosis elsewhere such as abdominal aortic aneurysm, peripheral vascular disease and multivessel coronary artery disease. The overall goals of therapy in ARAS is normalisation of blood pressure, improvement of renal function, and reduction of the risk of cardiovascular and cerebrovascular even...

Changes in the prevalence and social distribution of cardiovascular disease and risk factors in India
It is evident that cardiovascular disease (CVD) and its associated risk factors comprise a major and increasing burden of death and disability in India.1 In 2015, the Global Burden of Disease (GBD) study reported that ischaemic heart disease (IHD; the most common form of CVD) was the leading cause of premature mortality in India, after previously being ranked fifth a decade earlier.2 Over the decade from 2005 to 2015, premature IHD mortality increased by 11% compared with the next three leading causes: neonatal preterm birth, neonatal encephalopathy, and lower respiratory infection which, over the same period, each exhibit...

Trend in prevalence of coronary artery disease and risk factors over two decades in rural Punjab
Conclusions In a rural population of Punjab, the prevalence of several CAD risk factors like sedentary lifestyle, hypertension, diabetes, obesity and hypercholesterolaemia increased over 20 years. These changes in risk factors were associated with a modest increase in prevalence of CAD. (Source: Heart Asia)

Prevalence and pattern of cardiovascular risk factors in a population in India
Conclusions In our population in semiurban India, one in three adults have a major cardiovascular risk factor, with low control rates. There is a large burden of undiagnosed cardiovascular risk factors and a large gap between treatment and control, which may be explained by lack of treatment intensification. (Source: Heart Asia)

Is white rice consumption a risk for metabolic and cardiovascular outcomes? A systematic review and meta-analysis
