Clinical diagnosis of heart failure in nursing home residents based on history, physical exam, BNP and ECG: Is it reliable?
Publication date: Available online 9 November 2016 Source:European Geriatric Medicine Author(s): M.A.M.J. Daamen, H.-P. Brunner-la Rocca, F.E.S. Tan, J.P.H. Hamers, J.M.G.A. Schols Introduction Heart failure (HF) is highly prevalent in nursing home residents. To diagnose HF properly, echocardiography is recommended by cardiology guidelines. In nursing homes, the nursing home physician (NHP) usually diagnoses HF based on history, clinical examination, electrocardiography (ECG) and levels of B-type natriuretic peptide only, which is in contrast to the cardiologist who probably always will apply for an echocardiogram. ...

Comparison of the AIMS65 and Glasgow Blatchford score for risk stratification in elderly patients with upper gastrointestinal bleeding
Conclusions For the elderly patients with UGIB, the AIMS65 score is superior to GBRS in predicting inpatient mortality, and the GBRS is superior in predicting rebleeding and blood transfusion. Both scores are similar in predicting the composite clinical endpoint. (Source: European Geriatric Medicine)

Factors associated with frailty in community-dwelling elderly population. A cross-sectional study
Conclusions Factors identified as associated with frailty may alert healthcare professionals and help them to identify subjects at risk fragilization. Good control over underlying diseases and pain, rationalizing use of medications, optimizing nutritional status and body weight, promoting physical activity and improving social support may contribute to preventing or even reverting frailty. However, these hypotheses need to be tested. (Source: European Geriatric Medicine)

Detecting malnutrition and predicting mortality in the Spanish oldest old: Utility of the Controlling Nutritional Status (CONUT) score compared with the Mini Nutritional Assessment (MNA) score
Conclusions When compared, the MNA score detected a higher proportion of individuals at risk for malnutrition than that detected by the CONUT. Furthermore, MNA was more effective for predicting 4-year mortality rates than the CONUT. (Source: European Geriatric Medicine)

Fracture risk and bone health following a stroke are inadequately considered by physicians: A UK survey of practice
Conclusions We found marked heterogeneity in the approach of UK stroke physicians to the assessment of fracture risk and management of bone health in stroke patients with overall under-appreciation of fracture risk and low levels of assessment. Our findings support the need for clear guidelines regarding fracture risk assessment and bone health in patients who have experienced a stroke. (Source: European Geriatric Medicine)

Effects of preventive home visits on older people's use and costs of health care services: A systematic review
Conclusions Of the high number of studies investigating efficacy of PHVs on older people, only a few studies explore economic effects. PHVs do not provide overall savings to health care costs, but some interventions might offer some cost-neutral positive effects on functioning, QOL and/or mortality. More studies are needed to clarify the effective aspects of the programs and cost-effectiveness of the PHVs. (Source: European Geriatric Medicine)

A scoping review of music and anxiety, depression and agitation in older people with dementia in residential facilities and specialist care units
Conclusions There is insufficient evidence of the effect of music therapy on anxiety, depression and agitation in this population. Future studies should consider how specific musical interventions can improve anxiety, depression and agitation in older adults with dementia in residential facilities and specialist care units.
