Preface
At the 2001 American College of Physicians (ACP) Annual Conference, a new teaching format to aid physician learning, “Clinical Pearls,” was introduced. Understanding 3 qualities of physician learners allowed the design of Clinical Pearls to be created. First, we physicians enjoy learning from cases. Second, we like concise practical points that we can use in our practice. Finally, we take pleasure in problem solving. (Source: Disease a Month)

Clinical pearls in psychiatry
A 26-year-old patient comes to see you in follow-up for an emergency department visit for chest pain. This came on while she was driving and was experienced as an abrupt onset of a pounding heart, a sense she was choking and could not catch her breath, and a fear she would die. She pulled her car over and called emergency services from her car and was transported by ambulance to the emergency department. By that point, symptoms had improved. Blood pressure was 118/70, pulse was 70. ECG showed normal sinus rhythm. (Source: Disease a Month)

Foreword
October is liver awareness month as designated by the American Liver Foundation. Toward this end, we are presenting this month ׳s issue on a vexing condition associated with a variety of underlying hepatic pathologies. Hepatorenal syndrome (HRS) is a unique progression of renal impairment initiated by fulminant liver failure usually associated with portal hypertension and ascites. Up to 40% of patients with cirrhosis will develop HRS with a median survival time of under 6 months. (Source: Disease a Month)

An EGD is done to screen for Barrett’s esophagus, and he is found to have a 4-cm area in the distal esophagus consistent with Barrett’s, with biopsies confirming intestinal metaplasia with goblet cells, withou...
