Clinical hypertension: Blood pressure variability
Hypertension (HTN) is a global epidemic, and continues to be a leading cause of cardiovascular and cerebral complications with major economic implications.1,2 In the United States (US), the total crude prevalence of HTN over 2013 –14 has been estimated to be 33.5% in adults aged 20 and over.2 HTN is an established strong predictor of overall mortality, cardiovascular morbidity and mortality, and end stage kidney disease,1 and has been recognized as the cause of death (primary or contributing) in an estimated 410,000 deaths in the US in the year 2014. Primary Headache Disorders Part I- Migraine and the Trigeminal Autonomic Cephalalgias
In Primary Headache Disorders, Part 1, we discuss three of the primary headache disorders using the headache definitions from ICHD-III (Beta): Migraine, with and without aura; its pathophysiology and treatment are discussed. We then discuss the Trigeminal Autonomic Cephalalgias (TACs), including Cluster Headache and Hemicrania Continua, two more primary headache disorders, as well as the other TAC Headaches. We discuss pathophysiology as well as diagnosis, treatment, and pharmacotherapeutic management of these headache diatheses. (Source: Disease a Month)

Primary Headache Disorders- Part 2: Tension-type headache and medication overuse headache
In Part 2 of Primary Headache disorders, we discuss the fourth Primary Headache Disorder, Tension-Type Headache (TTHA). We are again using the ICHD-III (Beta) definitions of such headaches, taking into consideration episodic and chronic TTHA, as well as the presence or absence of pericranial muscle tenderness. We discuss the pathophysiology and pharmacotherapeutic treatment of TTHA, and the aspects of the Myofascial Pain Syndrome that enhance and help the development of TTHA. We then discuss Medication Overuse Headache (MOH), itself a Secondary headache disorder, but one that is extremely important as it assists with the c...

Clinical hypertension: Blood pressure variability
As the leading risk factor in cardiovascular disease, the management and treatment of hypertension has been at the forefront in primary care preventive medicine. Oftentimes, a mean blood pressure measurement obtained in a clinic setting has been used as the most important therapeutic metric in making management decisions; this may lead to an overly simplistic and erroneous conclusions due to variations during the day and night. Thus, due to oscillations during the day, measurement of a single clinic-based blood pressure is an inconsistent metric for a definable goal and therefore has often been problematic in hypertension ...

A comprehensive review and update on Crohn's disease
The term inflammatory bowel disease (IBD) refers principally to two major categories of chronic relapsing inflammatory intestinal disorders: Crohn's disease (CD) and ulcerative colitis (UC). In the United States, it is currently estimated that about 1.5 million people suffer from IBD, causing considerable suffering, mortality and economic loss every year. Yet the cause of IBD is unknown, and until we understand more, prevention or cure will not be possible. There is a lot of variation in the incidence and prevalence of CD based on geographic region, environment, immigrant population, and ethnic groups. (Source: Disease a Month)
