CONCLUSION: Specific conceptual and methodological work is required to develop a better understanding of TRs in psychiatric settings. Further research should include intervention studies and might support a stronger emphasis on TRs in training and supervision.

Early detection and treatment of schizophrenia: how early?
CONCLUSION: We are at the moment in an ethical dilemma between either diagnosing and treating this disorder too late or too early. The only way and prerequisite for solving this dilemma is a more reliable identification of individuals at risk and the beginning disease process.

The Australian experience of deinstitutionalization: interaction of Australian culture with the development and reform of its mental health services.
CONCLUSION: The effects of competition for limited resources between core mental health service delivery and the shift to a population-based public health approach (to prevention of mental illness and promotion of mental health), leaves our services vulnerable to doing neither particularly well. The recent loss of momentum of these reforms, due to failure of governments to continue to drive and fund them adequately, is causing the erosion of their considerable achievements.

Evaluating a community-based mental health service focusing on severe mental illness. The Verona experience.
CONCLUSION: Developing community-based mental health care requires a series of coordinated actions that need to be pursued over a long period of time. Service evaluation, including costs assessment, is an important part of this exercise.

Social psychiatry and public mental health: present situation and future objectives. Time for rethinking and renaissance?
CONCLUSION: A re-thought and re-conceptualized social and societal psychiatry with focus on public mental health must have a renaissance. Needs of people with severe mental illness.
CONCLUSION: Unmet needs are a strong predictor of less favourable health perceptions and a lower quality of life. Needs are to a certain extent changeable, and thus amenable by treatment or mental health interventions. The prevalence of (un)met needs should be an outcome criterion for the evaluation of mental health service systems and for cross-cultural comparison.

The Canadian National Outcomes Measurement Study in Schizophrenia: overview of the patient sample and methodology.
CONCLUSION: The CNOMSS provides demographic, clinical and treatment-related information about a large Canada-wide sample of psychiatric patients. The following three articles in this issue of Acta Psychiatrica Scandinavica explore issues related to medication, quality of life and resource utilization.

Medication decisions and clinical outcomes in the Canadian National Outcomes Measurement Study in Schizophrenia.
CONCLUSION: There were some regional differences noted in both utilization of types of antipsychotic medications and hospitalization rates. In this sample of stable out-patients over 70% who started on monotherapy with clozapine, risperidone, olanzapine and quetiapine remained on the same medication over the 2-year study period.

Outcome on quality of life in a Canadian national sample of patients with schizophrenia and related psychotic disorders.
CONCLUSION: Within a naturalistic sample of schizophrenia patients treated and followed in routine care the overall QoL showed an improvement over time but this improvement was not influenced by the type of medication prescribed.

Resource utilization in a Canadian national study of people with schizophrenia and related psychotic disorders.
CONCLUSION: Treatment with risperidone or quetiapine was associated with the lowest overall costs when compared with olanzapine or clozapine.

A framework for evidence-based mental health care and policy.
CONCLUSION: It is important to promote bridging strategies among these new fields in order to enhance communication and information transfer between the different parts involved in mental health decision making: i) clinicians and epidemiologists, ii) data analysts, iii) care policy makers and other end-users.
