Impact of left ventricular end diastolic pressure guided hydration on prevention of contrast induced nephropathy post cardiac catheterization
Conclusions LVEDP-guided hydration is superior to standard hydration in prevention of CIN. Hydration can be done based on LVFP in patients with pre-procedure normal LVF and in patients with pre-procedure elevated LVFP but not in those patients with inconclusive LVFP in which hydration should be guided by the invasively measured LVEDP. Increased left atrial stiffness in patients with atrial fibrillation detected by left atrial speckle tracking echocardiography

Diabetic patient with three-vessel disease and left main involvement. Surgery yes, but not always
We report the case of a 61-year old active smoker and diabetic man with atypical symptoms and positive treadmill test. The coronary angiography revealed a severe three-vessel disease and distal left main involvement (SYNTAX score=49). As the patient refused to follow heart team indication to undergo coronary bypass grafting, a percutaneous coronary intervention was successfully performed with intra-aortic balloon counterpulsation support and intravascular ultrasound optimization. Protective role of single versus multiple remote ischemic preconditioning in elective percutaneous coronary interventions
