Safety and efficacy of transcatheter left atrial appendage closure using the Watchman device in Egyptian patients with nonvalvular atrial fibrillation
Conclusion LAA closure with the Watchman device can be safely performed, and may be a reasonable alternative to consider for patients at high risk for stroke but with contraindications to systemic oral anticoagulation or with high risk of bleeding. (Source: The Egyptian Heart Journal)

Clinical perspectives and evidence of diastolic stress test in heart failure with preserved ejection fraction
Publication date: Available online 27 June 2015 Source:The Egyptian Heart Journal Author(s): Evgeny Belyavskiy , Daniel A. Morris , Elisabeth Kraigher-Krainer , Martin Kropf , Aravind K. Radha Krishnan , Albrecht Schmidt , Burkert Pieske The diagnosis of heart failure with preserved ejection fraction (HFPEF) remains on the basis of echocardiographic analyses at rest. However, some patients with HFPEF have symptoms such as dyspnea only during exercise. Accordingly, echocardiographic analyses at rest could be insufficiently sensitive to identify these patients. In line, recent studies demonstrated that in some pat...

Impact of pre-infarction angina on angiographic and echocardiographic outcomes in patients with acute anterior wall myocardial infarction managed by primary percutaneous coronary intervention
Conclusions For patients suffering from a first attack of acute anterior wall STEMI, pre-infarction angina is associated with a better Killip class at presentation, better TIMI flow grade after PPCI, less incidence of TMP 0 flow grade. (Source: The Egyptian Heart Journal)

Adverse events in pediatric cardiac catheterization: Initial experience of Sohag university hospital
Conclusions Pediatric cardiac catheterization by well trained team of pediatric cardiologists in a new center is effective. The rate of occurrence of AEs approximated to those which are reported by various interventional pediatric cardiology centers around the world. The lower body weight and the second year of initial experience are risk factors for occurrence of AE. (Source: The Egyptian Heart Journal)

Frequency of masked hypertension and its relation to target organ damage in the heart
Conclusion MH is a common phenomenon and associated with subclinical target organ damage in the heart comparable to SH and significantly higher than WCH and true normotension. (Source: The Egyptian Heart Journal)

Catheter-induced dissection of a normal right coronary artery: Reappraisal of the underlying mechanisms
Publication date: Available online 9 June 2015 Source:The Egyptian Heart Journal Author(s): Wassam El Din HadadEl Shafey Iatrogenic coronary artery dissection during diagnostic coronary catheterization is a rare but life-threatening event with a reported incidence of less than 0.1%. The current case report addresses the issue of catheter-induced dissection of an apparently normal right coronary artery (RCA). I tried to explain the factors underlying its occurrence, in view of the current knowledge of the aortic root motion during the cardiac cycle, and the spatial orientation of the RCA ostium. Left atrial volume index as a predictor of left ventricular remodeling in patients with anterior STEMI treated with primary PCI
Conclusion LAVI is a simple and non-invasive echocardiographic marker that can predict the development of LVR in patients with anterior STEMI treated with PPCI. (Source: The Egyptian Heart Journal)

Prognostic value of IL6 in young adults presenting with acute coronary syndrome

Significance of ST-segment deviation in lead aVR for prediction of culprit artery and infarct size in acute inferior wall ST-elevation myocardial infarction
Conclusion In patients with inferior wall STEMI, ST-segment depression in aVR was more common in LCX infarcts than RCA infarcts. Patients with aVR depression had significantly larger infarctions, (estimated by peak creatine phosphokinase (CPK-MB) levels and transthoracic echocardiography) than patients without aVR depression. Metabolic approach to heart failure: The role of metabolic modulators
Publication date: Available online 22 April 2015 Source:The Egyptian Heart Journal Author(s): Giuseppe M.C. Rosano , Cristiana Vitale , Ilaria Spoletini Heart failure (HF) is a systemic and multiorgan syndrome with metabolic failure as fundamental mechanism. As a consequence of its impaired metabolism, other processes are activated in the failing heart, further exacerbating the progression of HF. Metabolic agents are a relatively new class of drugs that act through optimisation of cardiac substrate metabolism. Among the metabolic modulators, Trimetazidine (TMZ) and perhexiline are the only two agents with proven ant...

An unusual outcome of a right atrium wall abscess in an infant. A case report
Publication date: Available online 22 April 2015 Source:The Egyptian Heart Journal Author(s): F. Jebri , H. Msaaed , B. Melki , C. Oueslati , K. Hakim , R. Boussada Echocardiography is known as an accurate tool for the diagnosis of myocardial abscess (MA). Surgery resection is compulsory as therapy for this intracardiac mass. Though, we report the first case in an infant, of a right auricular MA without valvular involvement, diagnosed retrospectively in a occasion of a purulent pericarditis, leaving place spontaneously to a new cavity and displacing the tricuspid ring. Treatment is not codified and remains differ...

Worsening of left ventricular twist mechanics in isolated rheumatic mitral stenosis immediately after balloon mitral valvuloplasty
Conclusion LV twist mechanics are worsened in MS with a further worsening, immediately after BMV probably because of failure of the LV to adapt to the sudden increased preload. (Source: The Egyptian Heart Journal)
