Clinical Decision Support for Perioperative Information Management Systems
Clinical decision support (CDS) systems are being used to optimize the increasingly complex care that our health care system delivers. These systems have become increasingly important in the delivery of perioperative care for patients undergoing cardiac, thoracic, and vascular procedures. The adoption of perioperative information management systems (PIMS) has allowed these technologies to enter the operating room and support the clinical work flow of anesthesiologists and operational processes. Constructing effective CDS systems necessitates an understanding of operative work flow and technical considerations as well as ac...

Modeling Anesthetic Developmental Neurotoxicity Using Human Stem Cells
Mounting preclinical evidence in rodents and nonhuman primates has demonstrated that prolonged exposure of developing animals to general anesthetics can induce widespread neuronal cell death followed by long-term memory and learning disabilities. In vitro experimental evidence from cultured neonatal animal neurons confirmed the in vivo findings. However, there is no direct clinical evidence of the detrimental effects of anesthetics in human fetuses, infants, or children. Development of an in vitro neurogenesis system using human stem cells has opened up avenues of research for advancing our understanding of human brain dev...

Cerebral Oximetry and Cardiac Arrest
Cerebral oximetry is a Food and Drug Administration–approved technology that allows monitoring of brain oxygen saturation in accessible superficial brain cortex regions, which are amongst the most vulnerable in regard to ischemic or hypoxic injury. Since most oxygen in the area of interest is located in the venous compartment, the determined regional brain oxygen saturation approximately reflects the local balance between oxygen delivery and oxygen consumption. Major systemic alterations in blood oxygen content and oxygen delivery will be accompanied by corresponding changes in regional brain saturation. This systema...

The Distinct Role of Palliative Care in the Surgical Intensive Care Unit
This article will explore palliative care, identifying patients/families who benefit from palliative care services, how palliative care complements SICU care, and opportunities to integrate palliative care into the SICU. Palliative care enhances the SICU team’s ability to recognize pain and distress; establish the patient’s wishes, beliefs, and values and their impact on decision making; develop flexible communication strategies; conduct family meetings and establish goals of care; provide family support during the dying process; help resolve team conflicts; and establish reasonable goals for life support and r...

Current Concepts in Diagnosis and Perioperative Management of Carcinoid Heart Disease
Carcinoid tumors are neuroendocrine tumors with a very unpredictable clinical behavior. In the setting of hepatic metastases, the tumor’s release of bioactive substances into the systemic circulation results in carcinoid syndrome: a constellation of symptoms among which cutaneous flushing, gastrointestinal hypermotility, and cardiac involvement are the most prominent. Cardiac manifestations, also known as carcinoid heart disease, are secondary to a severe fibrotic reaction which frequently involves the right-sided valves and may extend towards the subvalvular apparatus leading to valve thickening and retraction. Left...

Electrophysiology Procedures: Weighing the Factors Affecting Choice of Anesthesia
Invasive electrophysiologic procedures have evolved and increased in frequency significantly over the past 2 decades. The complexity and nature of the various procedures offered have also changed, and complex ablations for atrial fibrillation and ventricular tachycardia have become commonplace. These procedures often require the services of an anesthesiologist. An understanding of the specific nature and challenges of these procedures may be helpful in planning the optimal anesthetic and patient management. A paired review of these issues has been written from the standpoint of a practicing anesthesiologist. This review is...

Anesthesia for Electrophysiology Studies and Catheter Ablations
Providing anesthesia in nonoperating room locations is becoming increasingly common. The field of electrophysiology and its patient population are growing, resulting in a greater need for anesthesiologists in the electrophysiology laboratory. The procedures are complex and of long duration with patients who have multiple comorbidities. The electrophysiology laboratory is a unique situation in that arrhythmias are sought and sometimes even provoked so that they may be eliminated. The focus of this review will be on anesthesia for electrophysiology studies and catheter ablation. (Source: Seminars in Cardiothoracic and Vascular Anesthesia)

Anesthesia for Liver Transplantation
Orthotopic liver transplantation is the only definitive treatment for end-stage liver disease. More than 6000 procedures are performed in the United States annually with excellent survival rates. The shortage of donor organs leads to continued interest in techniques to enlarge the potential donor pool. Patients presenting for liver transplant suffer from important cardiovascular, respiratory, renal, neurological, and gastroenterological comorbidity. In the Western world, liver failure is increasingly caused by steatohepatitis, and transplant candidates are thus becoming older and more comorbid. The role of the transplant a...
