Cataract surgery training using surgical simulators and wet-labs: Course description and literature review
Publication date: Available online 5 February 2018Source: Saudi Journal of OphthalmologyAuthor(s): Saeed AlwadaniAbstractCataract surgery is the most common surgery to face the ophthalmology training resident. To facilitate achieving surgical competency and reduce complication rates, wet laboratories and surgical simulators are used in surgical disciplines worldwide. We developed a simulator and wet-lab course that aims to build the microsurgery skills of trainees and improve safety during real surgical procedures. Herewith, we describe the standardized hands-on course that incorporates these tools for advanced training. A...

Non-axial proptosis secondary to pneumosinus dilatans of the maxillary sinus
Publication date: Available online 8 February 2018Source: Saudi Journal of OphthalmologyAuthor(s): Rakan S. Al-Essa, Saad A. Alsaleh, Adel H. AlsuhaibaniAbstractPneumosinus Dilatans (PD) is a rare condition characterized by abnormal enlargement of one or more paranasal sinuses that can lead to different functional and cosmetic presentations. Here we report right non-axial proptosis in a 47-year-old female secondary to pneumosinus dilatans of the maxillary sinus with superior bowing of the orbital floor. Eyelid molluscum contagiosum presenting as a giant solitary ulcerating mass
We report a case of an eyelid molluscum contagiosum in a 5-year-old boy with no risk factors and a 2-week history of a large localized ulcerating mass of the left upper eyelid. The mass was totally excised, and diagnosis of molluscum contagiosum was confirmed in the histopathology study. Microscopic examination revealed enlarged keratinocytes, and acquired eosinophilic Henderson-Patterson inclusion bodies were also detected. Such ulcerating solitary lesions can be misdiagnosed as infected epidermal cysts, keratoacanthoma, or infected chalazions; therefore, molluscum contagiosum should be considered in the differential diag...

Just another metastatic carcinoid tumour to the uveal tract
We describe the clinic, image, and histopathologic features of a well differentiated neuroendocrine carcinoma (carcinoid tumour) metastatic to choroid and ciliary body in a 52-year-old Mexican Mestizo man. The ophthalmologic examination showed an inferior choroidal mass accompanied by exudative retinal detachment. Ultrasound B-Scan study revealed a diffuse thickened choroid with overlying serous retinal detachment, ultrasound A-Scan revealed a high internal reflectivity solid lesion. Ultrasound biomicroscopy (UBM) evidenced a dome shaped ciliary body mass, presumptive diagnosis was uveal tract metastatic disease. Scleral f...

Unilateral central retinal vein occlusion as a first manifestation of diabetes mellitus in a ten-year-old girl
We present a case of CRVO as the first manifestation of diabetes mellitus in a ten-year-old girl. The associated macular edema was managed successfully with a single injection of Ranibizumab. Ophthalmologists should consider the possibility of diabetes mellitus in pediatric cases of CRVO. (Source: Saudi Journal of Ophthalmology)

Does the addition of hyaluronidase improve the quality of peribulbar anesthesia in cataract surgery? – A randomized double blinded study
ConclusionsHyaluronidase is not an essential adjuvant for peribulbar block for cataract surgeries. Appropriate deposition of a smaller volume of anesthetic agent and adequate ocular massage provide adequate and safe anesthesia. (Source: Saudi Journal of Ophthalmology)

Terminology of Peters’ anomaly variants: Summary of histopathological findings in 6 corneas and detailed clinicopathological correlation in 2 cases
Publication date: Available online 8 March 2018Source: Saudi Journal of OphthalmologyAuthor(s): Hind M. Alkatan, Huda Al Dhaheri, Mosa Al HarbyAbstractPeters’ anomaly accounts for the highest type of Anterior Segment Dysgenesis (ASD). The main features of Peters’ anomaly are: congenital corneal opacity centrally, defect in the posterior stroma and absence of Descemet’s membrane and the endothelium. However, this condition has wide clinical and histopathological variations in appearance, associations and severity. In this case series, we summarize 6 corneas in 5 Saudi cases of Peters’ anomaly (and describe 2 in deta...

Customized 3D printing: A novel approach to migrated orbital implant
Publication date: Available online 15 March 2018Source: Saudi Journal of OphthalmologyAuthor(s): Tarjani V. Dave, Gayatri Gaur, Nikhil Chowdary, Dhruv JoshiAbstractThis paper describes a novel approach to treat migrated orbital implants post socket surgery. Implant migration may hinder the final aesthetic outcome of a custom ocular prosthesis. Once an implant migrates within the orbit there tends to be fibrosis around the implant. This fibrosis does not allow for centeration of the implant during repeat surgery. Hence treatment of a migrated implant traditionally involves implant removal with dermis fat grafting. Dermis fa...

Spontaneous breakage of intracorneal ring segments (ICRS) 8 years after implantation
We present a case of spontaneous in situ breakage of intracorneal ring segments (ICRSs) 8 years after their implantation in a patient with keratoconus. The patient presented to our clinic with a red and painful right eye that had not improved despite topical steroids and antibiotics. The decision was made to explant the broken ICRSs from the cornea and send them for laboratory analyses, by which manufacturer defects were excluded. It is noteworthy that corneal curvature re-steepening was observed 4 months after ICRS removal despite the performance of crosslinking (CXL) 1 year prior to ICRS implantation. (Source: Saud...
