Since July 2016, Drs Relhan et al have either individually or collectively written 2 editorials and 3 letters in this journal opposing the practice of routine intracameral antibiotic prophylaxis (ICAP) for cataract surgery. We agree that ICAP remains controversial because of the paucity of level 1 evidence. Javitt's oft-cited editorial elegantly summarizes the multiple reasons that a prospective, randomized, placebo-controlled trial is so difficult, expensive, impractical, and potentially unethical to now perform. Thank you to Drs Blair and Shapiro for their letter. Their comment about equipoise in the evaluation of controversies in retinopathy of prematurity (ROP) management could not be more germane. By citing an US Food and Drug Administration warning, the authors note possible danger in the use of general anesthesia in young children, a point in favor of intravitreal bevacizumab (IVB). This warning should be elaborated for the interested reader.1 The US Food and Drug Administration recommends against prolonged anesthesia (>3 hours) in children (Source: Ophthalmology)

Like Dattilo et  al,1 I also find stereoacuity to be a useful test in the neuro-ophthalmology clinic. It is a sensitive test for lesions of the chiasm and can also help diagnose patients with functional vision loss. However, I do not completely understand the line of reasoning they used with regard to traditional stereopsis and reverse stereopsis (RS). (Source: Ophthalmology)

We read with interest the article by Sung et  al.1 In this observational study, the authors concluded that the most distinct difference between the 2 subgroups in the cluster analysis was trabecular–ciliary process angle (TCA) and that the position of the ciliary body is important in subclassifying primary angle closure. We thank the author for the comments on our article1 and wish to clarify a few points, recognizing that Figure  1 (in the original article) may oversimplify the mechanisms leading to stereopsis. We greatly appreciate the interest Dr Li has shown in our article.1 We agree with the first comment that examinations can be influenced by observers' subjectivity. However, examinations can also be influenced by interexaminer variability if multiple examiners perform the analysis. Lin et  al2 reported that interexaminer measurements related to the ciliary process were variable and recommended that measurement should be performed by the same observer. Corneal Collagen Crosslinking for Ectasia after Refractive Surgery
The development of corneal ectasia has been recognized as a complication after LASIK since the first case report from Seiler and Quurke1 in the late 1990s. The clinical picture of iatrogenic corneal ectasia resembles that of progressive keratoconus with reduced corneal biomechanical strength, increasing central corneal steepness, irregular refractive errors, and decreasing visual acuity.
