Axillary surgery in breast cancer: the beginning of the end.
Authors: Dumitru D, Khan A, Catanuto G, Rocco N, Nava MB, Benson JR Abstract Axillary surgery in breast cancer patients has shifted from more extensive to minimalist approaches with re-evaluation of the risks versus benefits of available treatment options which are increasingly tailored to individual patient characteristics.A radical axillary node dissection is rarely indicated nowadays due to several factors including screening with detection of small node negative cancers, introduction of targeted node sampling, less reliance on information from nodal staging for adjuvant therapy decision making and evid...

DCIS management in the modern era.
Authors: Khan A, Dumitru D, Catanuto G, Rocco N, Nava MB, Benson J Abstract Ductal carcinoma in situ (DCIS) has been the subject of much controversy since the advent of population based breast screening programmes. An increasing number of asymptomatic women are being diagnosed with this condition and there is uncertainty over the best treatment algorithm for this condition if treatment is to be considered at all. Different subtypes of DCIS show innate differences in developmental pathways and biological behaviour. This is not only determined by pathological subtypes but there is increasing understanding of...

Non-operative management of rectal cancer: future perspectives turning into reality?
Authors: DE Manzini N, Bellio G, Corleone P, Troian M Abstract The management of rectal cancer has considerably changed over the last decades and complete response to neoadjuvant chemoradiotherapy is becoming a common clinical entity. There is still no consensus on the definition of complete response to neoadjuvant treatment prior to surgery. Treatment programs are mostly heterogeneous and non-randomized. In addition, techniques to diagnose complete response are still unclear and there is no uniformity in surveillance modality of those patients managed without operative intervention. We review the most rec...

How to reduce surgical complications in rectal cancer surgery using fluorescence techniques.
CONCLUSIONS: Real time intraoperative ICG fluorescent angiography (FA) is a safe and feasible technique to guide the surgeon in intraoperative decision-making process. ICG FA seems to reduce AL rates following rectal surgery for cancer. However large well-designed RCTs are needed to provide evidence for its routine use. Visual probing of rectal neoplasia-near-infrared interrogation of primary tumours and secondary lymph nodes.
Authors: Khokhar HA, Loughman E, Khogali M, Mulligan N, O'Shea DF, Cahill RA Abstract Laparoscopic and endoscopic colorectal intervention and operations have their basis in real-time, image-based decision-making and step-by-step sequenced technical progress. The capacity to visualise accurately malignant disease wherever it may be including within the primary lesion and its draining lymph node basin as well as at potential sites of metastatic harbour (i.e. peritoneum, liver and lung) would allow more accurate surgery at the time of operation and enable personalised, stratified surgical intervention. In add...

Post-surgical recurrence of HCC along resection margin treated by percutaneous US-guided ablation.
CONCLUSIONS: HCC recurrence along the surgical margin can be safely and effectively treated by percutaneous therapy, despite the misconception of the surgical margin as a hostile location. Percutaneous treatment may bridge the patient for additional therapy. PMID: 29471619 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Augmented reality for breast imaging.
CONCLUSIONS: Augmented breast imaging (ABI) to the breast can improve clinical outcomes, giving an enhanced view of the structures to work on. It should be further studied to determine its utility in clinical practice. PMID: 29471620 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Oncoplastic breast reduction in conservative surgery.
Authors: Rancati A, Angrigiani C, Dorr J, Aquaviva J, Nava MB, Catanuto G, Rocco N, Rancati A, Curutchet P Abstract Advances in reconstructive breast surgery with new materials and techniques now allow us to offer patients the best possible cosmetic results without the risks associated with oncological control of the disease. These advances, in both oncological and plastic surgery, have led to a new field, oncoplastic breast surgery, which enables us to undertake large resections and, with advance planning, and prevent subsequent deformities. This is particularly important when more than 30% of the breast ...

Development of epidermal growth factor receptor targeted therapy in pancreatic cancer.
Authors: Qing L, Qing W Abstract The epidermal growth factor receptor (EGFR) family are a series of important cancer therapeutic targets involved in cancer biology. These genes play an important role in tumor biological characteristics including angiogenesis, cell survival, invasion and glucose metabolism. In recent years, progresses have been achieved upon the cellular and molecular biological characteristics of EGFR and its role in cancer development based on the study of tumor specimens and experimental animal model. EGFR(HER1/ErbB) is overexpressed in over sixty percent of triple-negative breast cancer...

Conventional cytology vs. immunocytochemistry of intraoperative peritoneal washes in gastric cancer patients subjected to gastrectomy, clinical correlates and association with overall survival.
