Analysis of patient selection policy and pattern of recurrence after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for colorectal carcinomatosis.
CONCLUSIONS: By a selection policy based on patient, disease extension and primary tumor factors, a median survival higher than 50 months can be expected. Most patients will eventually recur, mainly in the peritoneum. The pattern of recurrence suggests a potential role for more effective intraperitoneal therapies and repeat surgical treatments. Comparing outcomes after treatment of rectal cancer over a long-lasting follow-up between patients who were offered to surgery alone and to surgery with neoadjuvant therapy. A single center survey up to 240 months.
CONCLUSIONS: Overall, these results showed comparable outcomes between both groups over such a long lasting follow-up. This time frame might be used more extensively for increasing our knowledge of RC biological behaviour as well. PMID: 29397634 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Use of pain medication before and after lumbar discectomy: longitudinal analysis of a nation-wide cohort.
CONCLUSIONS: The greater decline in the use of pain medication after discectomy was associated with a sharp rise of that use within six months before surgery. This suggests that lumbar discectomy may benefit especially those with acute or subacute pain within the six-month window. Pringle maneuver in robotic liver surgery: preliminary study.
CONCLUSIONS: Our results show that liver resections with robotic technique can be performed safely even without systematic Pringle maneuver. PMID: 29397637 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

OTSC ® Proctology vs. fistulectomy and primary sphincter reconstruction as a treatment for low trans-sphincteric anal fistula in a randomized controlled pilot trial.
CONCLUSIONS: Our results suggest that OTSC® Proctology is an effective and safe treatment in achieving permanent closure of the internal fistula opening in selected patients, with excellent results in terms of pain, postoperative incontinence, healing time, and days of hospitalization. Staple line reinforcement during sleeve gastrectomy: with a new type of reinforced stapler.
CONCLUSIONS: During laparoscopic sleeve gastrectomy, the use of Reinforced stapler statistically reduces the operative time and staple line bleeding. No significant difference is evidenced in terms of reduction of staple line leaks with this reinforced stapler. PMID: 29397639 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Video-Assisted Anal Fistula Treatment (VAAFT) in the management of complex anal fistula: single center experience of 224 consecutive patients.
CONCLUSIONS: VAAFT seems to be a safe and effective technique for treating primary and recurrent perianal fistula, providing a very good healing rate without sphincters impairment and allowing a very quick return to normal activities. Study on the effect of perfusion weighted imaging on guiding the operation implementation for non-enhanced glioma.
CONCLUSIONS: Magnetic resonance perfusion weighted imaging can display vessel growth and distribution within non-enhanced gliomas before surgery, and effectively evaluate the histopathological grade and grade of malignancy, and provide accurate guidance for tumor resection during surgery. Acute diverticulitis and surgical treatment.
Authors: Roig JV, Sánchez-Guillén L, GARCíA-Armengol JJ Abstract Diverticulitis is a common condition in industrialized countries and an important cause of hospital admissions. Its growing trend is a challenge for the surgeons who perform emergency surgery, because approximately 15-25 percent of the patients will require surgery, being the surgical management of complicated acute diverticulitis controversial. The past decade has seen a paradigm shift in the treatment of sigmoid diverticulitis based on new epidemiological studies and refinement of surgical techniques that has produced a reassessment of o...

The role of laparoscopic distal pancreatectomy in elderly patients.
CONCLUSIONS: LDP is safe and feasible if compared to ODP in selected elderly patients with body and tail pancreatic tumors. Less blood loss and shorter hospital stay are the most evident advantages of minimally invasive approach. Randomized controlled trilas and high-volume centers prospective studies with long-term outcomes are necessary to consider laparosocpy a standard of care. Minimally invasive robotic surgery may represent an interesting alternative to laparosocpy especially for splee-preserving procedures. Laparoscopic repair of perforated peptic ulcer: review.
CONCLUSIONS: LPR showed similar or better results than OR in terms of morbidity, mortality, operation time and hospital stay, caution is needed as the OR patients tend to be more shocked or with higher ASA at presentation. The higher leakage rate after LPR should be addressed in the training of the laparoscopic surgeons or other safety measures should be developed and employed.
