Laparoscopic or open distal pancreatectomy- for benign and malignant lesions of the pancreas?
Authors: Bausch D, Wellner U, Keck T Abstract Minimally-invasive techniques were adopted relatively late for pancreatic procedures due to their complexity. Today, minimally-invasive distal pancreatectomy is the procedure of choice for the treatment of benign or premalignant pancreatic lesions due to the advantages inherent to laparoscopy. Data on the efficacy of minimally-invasive distal pancreatectomy in oncologic surgery is still limited and larger prospective series are needed prior to its widespread use for the treatment of pancreatic cancer. This review addresses various aspects of distal pancreatecto...

Radiotherapy and breast reconstruction.
Authors: Rocco N, Catanuto G, Nava MB Abstract Indications for post mastectomy radiotherapy are widening to a larger group of mastectomy patients and breast reconstruction rates following mastectomy are rising all over the world, this leading breast surgeons to face breast reconstruction in a growing number of patients who will undergo radiotherapy Increased morbidity is associated with al forms of breast reconstruction in the setting of post-mastectomy radiotherapy and in patients who previously underwent chest wall radiotherapy. Different attitudes towards breast reconstruction in the radiotherapy settin...

Hybrid breast reconstruction.
We present a new reconstructive system encompassing prostheses and fat grafting as integrated tools to improve the final reconstructive outcome. Fat grafting in our view is a pre-operatively planned part of the reconstruction. The final volume we achieve will be made up of fat and implants in a variable percentage according to patient's body contour and breast morphology. We used the "Hybrid Breast Recontruction" technique in more than 350 reconstructions, both immediate and delayed, two-stage and DTI, without experiencing higher loco-regional recurrence and complication rates when compared with conventional two-stage reco...

The argument for POEM in treatment of achalasia.
This report assess the current knowledge of technical issues of POEM, outcomes after POEM and the status of POEM centers in the U.S.A., and presents the argument for surgeons to participate in the practice of POEM. PMID: 29366317 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Safety and feasibility analysis of combination therapy of laparoscopic left hepatectomy and choledochoscopy for calculus of the left intrahepatic duct.
CONCLUSIONS: Combination therapy of laparoscopic left hepatectomy and choledochoscopy is worth being promoted in clinical practice of treatment for calculus of intrahepatic duct with various advantages, such as significant effect, small trauma, high safety and reliability, rapid recovery after operation and few recurrences. The use of laparoscopy for locally advanced rectal cancer.
CONCLUSIONS: Locally advanced rectal cancers require multidisciplinary management including neoadjuvant chemo-radiotherapy and surgery. Preoperative re-staging may be useful to plan the best surgical procedure. Minimally invasive surgery, especially laparoscopy, can provide adequate outcomes in selected patients with locally advanced rectal cancer with the advantage of faster recovery compared to open surgery. However, the overall evidence is poor, and further studies are needed to elucidate the role of minimally invasive surgery for cT4 rectal cancer. Intrathoracic stomach in hiatal hernia: the role of laparoscopic repair.
CONCLUSIONS: Laparoscopy plays a major role in the treatment of intrathoracic stomach and regarding most aspects of the treatment. All available techniques have their advantages and disadvantages, and the decision on how to repair the intrathoracic stomach, remains a tailored based decision. The surgical management of obesity.
Authors: Torres-Landa S, Kannan U, Guajardo I, Pickett-Blakely OE, Dempsey DT, Williams NN, Dumon KR Abstract Obesity is a spreading epidemic associated with significant morbidity and mortality with a prevalence of over 36% worldwide. In the face of a growing epidemic, increasing medical costs, and the disappointing limitations of medical and lifestyle modification bariatric surgery was found to consistently lead to significant weight loss and improvement in obesity-associated comorbidities when compared to non-surgical interventions. Bariatric procedures fall within three basic categories: restrictive pro...

Curative effect of knee arthroplasty on patients with osteoarthritis and analysis of long-term joint function of these patients.
CONCLUSION: Both the arthroscopy debridement and the knee replacement have good curative effect. The curative effect of knee replacement was better, postoperative knee joint activity was obviously improved, the long-term recovery of knee joint function was good, and knee replacement can reduce the pain of the patients. Laparoscopic vs. open approach for pT3/pT4 colorectal cancer in the elderly. Ten-year experience in a single center.
CONCLUSIONS: Laparoscopy is as safe as open surgery for pT3/pT4 CRC and it is often feasible, though in old patients higher conversion rates should be anticipated than in younger ones. PMID: 29154512 [PubMed - as supplied by publisher] (Source: Minerva Chirurgica)

Propensity score-matched comparison between complete mesocolic excision and classic right hemicolectomy for colon cancer. An Italian and Egyptian experience.
CONCLUSIONS: Right colon cancer patients undergoing CME benefitted from more oncological adequacy, with no increased postoperative complication rate, a decreased locoregional recurrence rate, and a better long-term outcome than patients operated on with the conventional procedure.
