Use of DOACs in real-world challenging settings: a Delphi Consensus from Italian cardiologists.
CONCLUSIONS: DOACs should represent the first-line anticoagulation therapy in non-valvular AF patients in the majority of challenging settings, underexplored by literature. Caution in their prescription is needed in case of severe renal impairment. Dose choice should follow the SmPC, although this is matter of debate in borderline patients. Helicobacter pylori and cardiovascular disease: update 2019.
Authors: Mladenova I Abstract Many studies have been performed concerning the potential role of Helicobacter pylori (H. pylori) in different extra-gastric diseases. Ischemic heart disease (IHD) remains the leading cause of mortality in developed countries. The traditional cardiovascular risk factors could not predict all cases of IHD. Hence, the scientists explore other potential etiologic factors, especially infections. H. pylori infection has been suspected to have a role in the pathogenesis of atherosclerosis. However, after 25 years from the first description, the role of the bacterium in the pathogene...

Exercise and atrial fibrillation: how health turns harm, and how to turn it back.
Authors: Tirapu L, San Antonio R, Tolosana JM, Roca-Luque I, Mont L, Guasch E Abstract Exercise is, together with diet, a powerful health-promoting habit. However, an association of intense physical activity with the onset of atrial and ventricular arrhythmias, and sudden death has been described. Although initially questioned, the atrial pro-arrhythmic role of endurance physical activity is now well accepted in the scientific community. Atrial fibrillation is common among endurance athletes, being a source of morbidity in otherwise healthy young and middle-aged individuals. The mechanisms for its developm...

Implications of the new 2017 American College of Cardiology/American Aeart Association Guidelines for Hypertension.
Authors: Aronow WS Abstract Automated validated devices should be used for measuring blood pressure (BP). The 2017 American College of Cardiology (ACC)/American Heart Association (AHA) hypertension guidelines recommend that a systolic BP between 120-129 mm Hg with a diastolic BP less than 80 mm Hg should be treated with lifestyle measures. These guidelines recommend treatment with lifestyle measures plus BP lowering drugs for secondary prevention of cardiovascular events in persons with clinical cardiovascular disease and an average systolic BP of ≥130 mm Hg or an average diastolic BP ≥ 80 mm Hg. These...

Non vitamin K oral anticoagulants versus antiplatelets in embolic stroke of undetermined source: most updated evidence.
Authors: Chatterjee S, Dubey S, Lahiri D, Ray BK Abstract Recent trial data have expanded the horizons of newer indications of non vitamin K oral anticoagulants (NOAC). Most recently they are being evaluated for use in embolic stroke of undetermined source (ESUS). ESUS are particularly known for their recurrences. So, identifying the causes and treating those etiological factors are the keys to secondary prevention of ESUS. Although traditional experts still opine for the use of antiplatelets for secondary prevention of ESUS as for other causes of embolic stroke, there are still room for improvement in del...

Relationships between left ventricular geometry (sphericity index) and remodeling in dilated cardiomyopathy.
CONCLUSIONS: To summarize, changes in sLVd are crucial for changes in LV geometry, whereas lLVd has a negligible effect on this process. The presence of LVRR was not always associated with an improvement in SI and its absence with increase in SI. Since the assessment of SI is less complex than LVRR, SI as a significant LVRR predictor can be useful part of a regular echocardiography examination. Ablation, rate or rhythm control strategies for patients with atrial fibrillation: how do they affect mid-term clinical outcomes? A retrospective cohort study.
CONCLUSIONS: AF ablation is able to decrease the arrhythmic episodes but does not offer a statistically significant protection against the composite of death, disabling stroke, severe bleeding and cardiac arrest In the mid-term follow-up. Effectiveness of the ECG method in the correct positioning of PICC type central venous catheters in patients with atrial fibrillation.
Authors: Albertini F, Struglia M, Faraone V, Fioravanti R, Boursier Niutta S Abstract The peripherally inserted central catheters (PICCs) are third generation silicone or polyurethane catheters, used as venous accesses for long- and medium-term care, and recommended in therapies that require venous access for more than six days. Current implantation methods recommend the radiological method, using standard chest RX in antero-posterior projection, as a method of assessing the correct positioning of the catheter tip. The observational study evaluated the effectiveness in terms of reliability of the intracavi...

Recurrent retinal vein thrombosis: prevention with Aspirin, Pycnogenol ®, ticlopidine, or sulodexide.
CONCLUSIONS: Pycnogenol® is the only product able to control edema and this may reduce the incidence of recurrent RVT. This retrospective registry indicates that Aspirin, Pycnogenol®, ticlopidine an sulodexide reduce recurrent RVT without side effects. Larger studies should be planned to involve a wider range of conditions, diseases and risk factors associated with RVT and to its recurrence. Validation of a new score for outcome prediction in patients with heart failure with reduced ejection fraction.
CONCLUSIONS: Our prognostic model, named OSR HF Risk Score, is a simple, quick, inexpensive tool for predicting patient outcome in 5 years. It might be used as an adjunctive tool in outpatients evaluation alongside more complex scores. PMID: 30919603 [PubMed - as supplied by publisher] (Source: Minerva Cardioangiologica)

Hybrid ablation for atrial fibrillation: the importance of achieving transmurality and lesion validation.
Authors: Hassan SM, Hong K, Rosati F, Glover B, Redfearn D, Enriquez A, Bisleri G Abstract Therapeutic ablation for atrial fibrillation (AF) has evolved significantly with progressive advancements in technology and surgical instruments. With the goal of minimizing surgical morbidity while maintaining the benefits of the traditional Cox-Maze procedure, surgical ablation for AF has undergone significant modifications. Most recently, an increased understanding of substrate complexity, predominantly in patients with persistent or long-standing persistent AF, has led to the development of a synergistic hybrid a...
