Late onset interstitial pneumonia induced by bepridil: A case report
Publication date: February 2015 Source:Journal of Arrhythmia, Volume 31, Issue 1 Author(s): Manabu Matsumoto , Masanobu Ohga , Shumpo Uemura A 63-year-old man was prescribed bepridil for paroxysmal atrial fibrillation in May 2011. He was referred to our hospital with the chief complaint of slight dyspnea in October 2012. Radiography and computed tomography indicated diffuse bilateral peribronchial infiltration. Since an examination of the bronchial alveolar lavage fluid revealed inflammatory lymphocytes and a drug lymphocyte stimulation test was strongly positive for bepridil, he was diagnosed with bepridil-induced ...

Prominent J-wave and T-wave alternans associated with mechanical alternans in a patient with takotsubo cardiomyopathy
Publication date: February 2015 Source:Journal of Arrhythmia, Volume 31, Issue 1 Author(s): Rintaro Hojo , Seiji Fukamizu , Takeshi Kitamura , Kota Komiyama , Yasuhiro Tanabe , Tamotsu Tejima , Mitsuhiro Nishizaki , Harumizu Sakurada , Masayasu Hiraoka A 74-year-old woman with takotsubo cardiomyopathy developed polymorphic ventricular tachycardia during the acute phase. She exhibited prominent J-wave and T-wave alternans preceding ventricular tachycardia. These abnormalities disappeared after recovery from myocardial stunning. (Source: Journal of Arrhythmia)

Relation between frequency of activated partial prothrombin time measurements and clinical outcomes in patients after initiation of dabigatran: A two-center cooperative study
Conclusions In this retrospective study, the frequency of aPTT measurements after dabigatran initiation might have been dependent on patient characteristics. However, frequent aPTT measurements did not lead to a reduction in adverse clinical events. (Source: Journal of Arrhythmia)

The optimal setting of complex fractionated atrial electrogram software in substrate ablation for atrial fibrillation
Conclusion These data suggest that setting #1 was generally superior to the default setting, whereas setting #2 was optimal for excluding areas not requiring ablation. The optimal CFAE software setting was a voltage of 0.05–0.30mV and an interval parameter of 40–70ms. (Source: Journal of Arrhythmia)

Effect of intravenous amiodarone on QT and T peak–T end dispersions in patients with nonischemic heart failure treated with cardiac resynchronization-defibrillator therapy and electrical storm
Conclusions Intravenous amiodarone suppressed the electrical storm and decreased the QT and Tp–e dispersions in patients treated by using CRT with a defibrillator. (Source: Journal of Arrhythmia)

Successful transjugular extraction of a lead in front of the anterior scalene muscle by using snare technique
We report a case of and successful device removal in a 79-year-old man with implantable cardioverter-defibrillator infection. Right phrenic nerve paralysis was evident on chest radiography. The lead was in front of the anterior scalene muscle, close to the left phrenic nerve. Therefore, extraction carried a risk of bilateral phrenic nerve paralysis. The lead was successfully extracted from the right internal jugular vein by using the snare technique. No complications occurred, and the extraction was successful. (Source: Journal of Arrhythmia)

Feasibility and accuracy of a new mobile electrocardiography device, ER-2000®, in the diagnosis of arrhythmia
Conclusions A rhythm strip obtained using ER-2000® is accurate in diagnosing arrhythmia, despite some differences in the detailed morphology of the QRS and T-wave, and the pacing spike compared to those obtained by the 12-lead ECG. (Source: Journal of Arrhythmia)

Dramatic improvement of refractory anemia caused by mechanical hemolysis in a patient with hypertrophic obstructive cardiomyopathy using dual-chamber pacing
Publication date: Available online 2 January 2015 Source:Journal of Arrhythmia Author(s): Yosuke Miwa , Kyoko Soejima , Toshiaki Sato , Kenichi Matsushita , Junsuke Sueoka , Nobuyuki Takayama , Hideaki Yoshino A 59-year-old woman with hypertrophic obstructive cardiomyopathy was admitted to our institution with worsening heart failure. She developed mechanical intracardiac hemolysis due to left ventricular outflow tract (LVOT) obstruction. Despite surgical myectomy, the LVOT pressure gradient (PG) remained high and hemolytic anemia recurred. A dual-chamber implantable cardioverter defibrillator was then implanted...

Malignant conversion of benign right ventricular outflow track ventricular tachycardia 18 years post-ablation
Publication date: Available online 30 December 2014 Source:Journal of Arrhythmia Author(s): Wendy H. Gerstein , Neal S. Gerstein , Andrea Sandoval , Michael B. West This case report describes the rare phenomenon of malignant conversion of benign right ventricular outflow tract ventricular tachycardia into idiopathic ventricular fibrillation 18 years after successful ablation, in the absence of any type of heart disease. We review the current literature looking at predictors for this event, with the conclusion that there are no reliable risk predictors available. Until clear guidelines exist, we suggest patients be ...

Relationship between plasma dabigatran concentration and activated partial thromboplastin time in Japanese patients with non-valvular atrial fibrillation
Conclusions There was a significant correlation between plasma dabigatran concentrations and aPTT. Additionally, in daily clinical practice in Japan, plasma dabigatran concentrations and aPTT reached a peak in the 4-h post administration range. Considering the pharmacokinetics of dabigatran, aPTT can be used as an index for risk screening for excess dabigatran concentrations in Japanese patients with NVAF. (Source: Journal of Arrhythmia)
