An unusual presentation of sub-capital neck fracture over the old intertrochanteric femur fracture with dynamic hip screw
Publication date: Available online 24 May 2016 Source:Apollo Medicine Author(s): Siva V. Reddy, G.S. Asati, Chandan S. Kanwar Although sub-capital neck fracture of femur over an old healed trochanter fracture with dynamic hip screw implant in situ is not uncommon, but can be an unexpected outcome for both surgeon and patient. Many series of case reports have been published so far in the literature, but the majority of them were in relatively old and female age groups with poor bone stock. Herewith, we are presenting an unusual occurrence of this type of entity in a 58-year healthy Indian male, which is managed wi...

Association of serum gamma glutamyl transferase with lipid abnormalities in end stage renal disease

Unusual case of total lung collapse due to endobronchial tuberculosis – A case report
Publication date: Available online 6 April 2016 Source:Apollo Medicine Author(s): V. Vinod Kumar An 18-year-old female with history of fever and loss of weight was found to have a total lung collapse during imaging. Flexibronchoscopy was consistent with endobronchial tuberculosis and the patient was started on anti-tuberculous treatment. She responded well to treatment and the collapsed lung opened up spontaneously in 4 weeks. This case is presented here due to unusual presentation of endobronchial tuberculosis as a total lung collapse. (Source: Apollo Medicine)

Arthroscopic retrieval of bullet from knee
We describe a rare case report of an intra-articular bullet caused by misfiring of a gun and its early arthroscopic retrieval. The 45-year-old policeman recovered well from surgery and had complete range of motion (0–130°) with no synovitis and minimal pain. Early arthroscopic-assisted retrieval can help achieve a satisfactory result and long-term outcome. (Source: Apollo Medicine)

A lesion to learn: Stroke mimics
Publication date: Available online 26 March 2016 Source:Apollo Medicine Author(s): Mahir Meman, Pushpendra Nath Renjen, Dinesh M. Chaudhari Acute ischemic stroke with neurological deficit is a very debilitating condition, especially in younger patients. IV thrombolysis is the only effective treatment available in most of the centers across India. But delay in hospitalization and bleeding complications are major limitations. In addition to that, stroke mimics are another big problem. Correct identification of stroke mimics needs clinical expertise and imaging studies. Multiple studies indicate safety of thrombolys...

Circulating irisin is increased in type 2 diabetes mellitus and correlates with fasting glucose levels
Conclusion This result indicates that T2D is associated with an irisin release in order to compensate the insulin resistance observed in the skeletal muscle on diabetes, and that the poor glycaemic control contributes for enhancement of this state. (Source: Apollo Medicine)

A case of ocular myasthenia: An eye-opener
We report a 32-year-male patient presenting with an acute onset total ophthalmoplegia. Ice-cooling test, edrophonium test and the repeated nerve stimulation test were found to be normal. A diagnosis of myasthenia gravis was made after a positive anti-acetylcholine receptor antibody test and treatment with pyridostigmine showed gradual improvement over 11 days. Isolated ocular myasthenia is a well-known entity but sudden onset complete ophthalmoplegia is rarely reported in literature. Our case not only puts forward the atypical presentation of myasthenia gravis but also is a reminder of the pitfalls of the conventional diag...

Candida utilis causing neonatal Candidemia – A case report and literature review
We report a fatal case of neonatal sepsis caused by C. utilis. Treatment was initiated with fluconazole. Despite resuscitative measures, the infant succumbed on day four of admission in the neonatal intensive care unit. To our knowledge, this is the first case report of C. utilis causing neonatal Candidemia in India. Our report and review highlight the importance of speciation and choice of antifungal therapy for successful clinical management of such cases the need of such important clinical and epidemiological data. Statins and stroke
Publication date: Available online 14 March 2016 Source:Apollo Medicine Author(s): Dinesh M. Chaudhari, Pushpendra Nath Renjen, Mahir Memon Stroke is a heterogeneous condition with a number of possible etiologies. Patients with different underlying stroke pathophysiologies may variably respond to specific preventive treatment strategies. Statin outcome trials in subjects with established coronary artery disease, hypertension, diabetes, or at high vascular risk have shown a consistent 17–21% relative reduction in the risk of incident stroke. The benefits observed with statins appear to be greater than what might...

Hormone use and stroke
Publication date: Available online 14 March 2016 Source:Apollo Medicine Author(s): Pushpendra Nath Renjen, Dinesh M. Chaudhari, Mahir Meman Strokes are an important cause of disability and death among older women. Because many women use hormone therapy for the control of perimenopausal symptoms and to prevent osteoporosis after menopause, establishing whether such therapy has other health effects is of considerable clinical importance. Overall, 55% of strokes occur in women, and women account for nearly 60% of all stroke-related deaths. Women appear to be protected from heart disease and stroke before menopause. ...

Publication date: Available online 3 March 2016 Source:Apollo Medicine Author(s): Kübra Ozgok-Kangal, İbrahim Arziman, Günalp Uzun, Şenol Yildiz Carbon monoxide (CO) poisoning is an important cause of mortality and morbidity worldwide. Because of non-specific symptoms of CO poisoning, a high vigilance in the acute care settings is required to diagnose CO poisoning. CO poisoning in the workplace is less recognized. Herein, we present two patients who have poisoned with CO at work but left undiagnosed at their initial admission to emergency department. We further discuss the importance of history taking in th...
