Sclerotherapy for placenta accreta: Some concerns
Malag ón Reyes et al [1] proposed a novel procedure to reduce bleeding during cesarean hysterectomy for placenta accreta (accreta, increta, and percreta). Vessel-sclerosing polidocanol solution was injected through an umbilical vein cannula and the bilateral internal hypogastric arteries were ligated, f ollowed by hysterectomy, leading to a relatively small amount of bleeding [average: 2.7 L (range: 2.5–3.0)]. Response to the comments of Dr Shigeki Matsubara
The authors celebrate the interest of an expert in this field. In relation to the first point in the surgical technique of our article, we decannulate the umbilical vein after the extraction of the fetus, so that there would be no fetal complications due to sclerosis. By contrast, the extension of sclerosed maternal vessels with the polidocanol dose we used has not been quantified. The association of enchondromatosis with malignant transformed chondrosarcoma and ovarian juvenile granulosa cell tumor (Ollier disease)
Ovarian juvenile granulosa cell tumor has an interesting association with multiple enchondromatosis (Ollier disease and Maffucci syndrome) and should be considered a leading diagnosis when an ovarian mass is found in young patients with these conditions. Besides the association with nonskeletal malignancies, there is a high risk of malignant transformation of enchondroma in chondrosarcoma as was also the case in this instance. Unusual clinical presentations of choriocarcinoma: A systematic review of case reports
Choriocarcinoma (CC) is a highly malignant tumor originating in the trophoblastic tissue. The clinical presentation of CC is so much varied that every case may be one of its kinds and thus can be a diagnostic challenge. Numerous case reports have been published in various journals regarding the unusual clinical presentations of this cancer. Maternal dyslipidemia during pregnancy may increase the risk of preterm birth: A meta-analysis
Epidemiological studies have reported an inconsistent relationship between maternal lipid levels and preterm birth (PTB). We performed this meta-analysis to evaluate the association between maternal dyslipidemia and PTB. Overall, three nested case-control studies and eight cohort studies were eligible. Effect estimates [odds ratio(OR)/relative risk] were pooled using a fixed-effects or a random-effects model. Meta-analysis of validity of echogenic intracardiac foci for calculating the risk of Down syndrome in the second trimester of pregnancy
The objective of this study is to assess the validity of echogenic intracardiac foci for Down syndrome detection in the second trimester ultrasound scan. A systematic search in major bibliographic databases was carried out (MEDLINE, EMBASE, CINAHL). Twenty-five studies about echogenic intracardiac foci were selected for statistical synthesis in this systematic review. Those 25 considered to be relevant were then subjected to critical reading, following the Critical Appraisal Skills Programme criteria, by at least three independent observers.
