Publication date: Available online 23 June 2014 Source:Taiwan Journal of Ophthalmology Author(s): Kui-Yueh Lin , I-Hua Wang , Jieh-Ren Jou , Hai-Jui Chu , William Wei , Shwu-Huey Lee , Szu-Yuan Lin Chronic inflammatory demyelinating polyneuropathy (CIDP) is a condition that mainly affects the peripheral nervous system; however, the central nervous system has also been involved in rare cases. Herein, we describe the case of a 33-year-old man with CIDP who presented with progressively blurred vision and pain with eye movement in both eyes for 1 month. Bilateral inferior oblique muscle paresis after posterior subtenon injection of triamcinolone acetonide
We report a case of simultaneous bilateral inferior oblique muscle paresis in a 60-year-old man who developed it after receiving intravitreal injections of bevacizumab and posterior subtenon injections of triamcinolone acetonide for the treatment of diabetic macular edema. The patient complained of excessive pain during the injections, which were performed at the 6 o'clock position in both eyes. According to the literature, inferior oblique muscle paresis after posterior subtenon injections is rare. This complication may be prevented by proper selection of the injection site and paying attention to any unusual patient com...

Major challenges in vitreoretinal surgery
Publication date: Available online 26 July 2014 Source:Taiwan Journal of Ophthalmology Author(s): Yijun Hu , Shibo Tang Since the first vitrectomy surgery was used for treatment of vitreoretinal diseases, surgical techniques and instrumentation have been rapidly improved in the past decades. However, there are complicated vitreoretinal diseases that cannot be successfully treated, even with state-of-the-art surgeries. The outcomes of some complicated cases are still poor due to different reasons and debates still remain in some areas regarding what are the best treatments. There is still a lack of full understanding ...

Management of hypotony after glaucoma filtering surgery
Publication date: Available online 26 July 2014 Source:Taiwan Journal of Ophthalmology Author(s): Pei-Kang Liu , Han-Yi Tseng , Kwou-Yeung Wu A 39-year-old male with open angle glaucoma in both eyes visited our clinic. The intraocular pressure (IOP) of both eyes fluctuated between 15 mmHg and 25 mmHg. The best corrected visual acuity (BCVA) was 20/20 in the right eye and 20/100 in the left eye. He underwent trabeculectomy with an adjunctive agent (mitomycin C; concentration, 0.2 mg/mL) smoothly on the right eye. After the removal of releasable sutures, the filtering bleb was prominent over the superior limbus for...

General skin erosions developed 2 hours after surgery. The patient's family doctor diagnosed that she was allergic to seafood. One month later, she underwent phacoemulsification surgery in the other eye. After surgery, she developed Stevens–Johnson syndrome with general skin lesions, erythema nodosa, genital mu...

Ocular adnexal lymphoma: Five case reports and a literature review
This article reports the clinical course and treatment of ocular adnexal lymphoma based on a retrospective review of five cases with a histologically approved ocular adnexal lymphoma at Kaohsiung Veterans General Hospital over 10 years. Extranodal B-cell lymphoma in the orbit, lacrimal gland, eyelid, or conjunctiva was found in these patients. Four of them were female, and they were aged 45–64 years. All patients were also consulted with hematologists for possible systemic involvement and therapeutic plan. The patient with retrobulbar and orbital apex involvement received systemic chemotherapy. The patient with lacrimal ...

Ocular post-transplant lymphoproliferative disorder
We report a case of an iris tumor with mutton-fat keratic precipitates in a young patient after liver transplantation surgery. A 6-year-old girl underwent liver transplantation for congenital biliary atresia and was subsequently immunosuppressed with oral cyclosporine. We examined her 5 years after transplantation because of a “white nodule in her left eye,” which had been detected by her father one day before visiting our clinic. Ophthalmological examinations revealed symmetric visual acuity and normal afferent papillary reflex. Slit-lamp examination revealed a depigmented iris nodule approximately3 × 2 mm with mutto...

Postoperative orbital fluid accumulation masquerading as subperiosteal orbital abscess: A case report and literature review
Publication date: September 2014 Source:Taiwan Journal of Ophthalmology, Volume 4, Issue 3 Author(s): Chao-Yun Chi , Yih-Jeng Tsai , Cheng-Hui Lin , Yu-Chi Lin We herein present the case of a postoperative orbital fluid accumulation in a 1.5-year-old boy masquerading as subperiosteal orbital abscess (SPA). The patient was admitted to our hospital under the impression of orbital cellulitis with SPA. He subsequently underwent endoscopic sinus surgery and received external drainage for decompression. A significant amount of fetid odor abscess was drained from the subperiosteal space. However, a new episode of eyelid s...

Amantadine-related corneal edema and endothelial cell loss: Four case reports
Publication date: September 2014 Source:Taiwan Journal of Ophthalmology, Volume 4, Issue 3 Author(s): Chia Ching Lin , Chieh Yin Cheng , Pei Shin Hu , Chang Ping Lin , Shiuh Liang Hsu Amantadine is widely used in treating influenza A, hepatitis, Parkinson's disease, and fatigue in multiple sclerosis. In the past, only a few case reports have demonstrated that amantadine is associated with corneal edema, endothelial dysfunction, and other corneal comorbidity. We herein present four cases with reversible corneal edema and endothelial loss after taking amantadine, including two cases with delayed presentation of corn...

Undercorrection after photorefractive keratectomy in Wilson's disease
Publication date: September 2014 Source:Taiwan Journal of Ophthalmology, Volume 4, Issue 3 Author(s): Yung-Sung Lee , Yi-Ming Su , Hsin-Chiung Lin A 34-year-old woman with Wilson’s disease and prominent Kayser–Fleischer rings had uncomplicated photorefractive keratectomy (PRK) in both eyes. Her preoperative refraction was −3.25 −2.50 × 180 in the right eye and −3.25 −1.50 × 180 in the left eye. The corneal topographic pattern showed symmetric bow tie with-the-rule astigmatism, and central corneal thickness was 550 μm in both eyes. Three months postoperatively, her refraction was −1.00 −...

Comparison of transpalpebral tonometer with Goldmann applanation tonometer
Publication date: September 2014 Source:Taiwan Journal of Ophthalmology, Volume 4, Issue 3 Author(s): Ashim Kumar Chakraborty , Mousumi Majumder , Santanu Sen Transpalpebral tonometer, a new generation intraocular pressure (IOP) measuring instrument is nowadays used for self-tonometry. It is convenient and noninvasive and seems suitable for IOP measurement at home, as recommended by several authors. Apart from its use for self-tonometry, it has been reported that transpalpebral tonometer is more accurate in determining the IOP in thinned cornea after photorefractive procedures when compared with Goldmann applanation...
